Mortality and Surgery-Related Complications in Very Low and Extreme Low Birth Weight Infants Treated for Esophageal Atresia: A Multi-Center Cohort Study.

INTRODUCTION: Surgical esophageal atresia (EA) repair is a complex operation with high risk of perioperative complications. Infants born with very low or extreme low birth weight (VLBW or ELBW) might be even at higher risk for developing perioperative complications. However, studies about this specific patient group are scarce, resulting in a knowledge gap on the outcomes of these infants. This hampers identifying the optimal treatment strategy, but also good counselling of parents. The aim of this study was to evaluate the mortality and surgery-related complication rates of VLBW and ELBW infa

Effect of co-trimoxazole prophylaxis on morbidity and mortality of HIV-exposed, HIV-uninfected infants in South Africa: a randomised controlled, non-inferiority trial.

BACKGROUND: WHO guidelines recommend co-trimoxazole prophylaxis for HIV-exposed, HIV-uninfected infants. These guidelines date back to an era in which HIV testing of infants was impossible and mothers had poor access to antiretroviral treatment. To determine whether this guideline requires revision in the current era of effective prevention of mother-to-child transmission and early infant diagnosis programmes, we aimed to investigate whether receiving no co-trimoxazole prophylaxis is inferior to receiving co-trimoxazole prophylaxis in the resulting incidence of grade 3 or 4 common childhood il

IgM and IgA enriched polyclonal immunoglobulins reduce short term mortality in extremely low birth weight infants with sepsis: a retrospective cohort study.

BACKGROUND: Immunoglobulin supplementation is a debated strategy in fighting sepsis. We evaluated a polyclonal IgM and IgA enriched immunoglobulin (IgMeIVIG) preparation in reducing the short-term mortality in extremely low birth weight neonates (ELBW) with proven infection. METHODS: ELBW infants born from January 2008 to December 2014 were eligible for this retrospective case-control analysis if they were symptomatic and had a positive blood culture after 72 hours of life. Patients received antibiotic treatment with or without IgMeIVIG (intravenously, 250 mg/kg/day for 3 days) within the 24

Mortality, morbidity, length and cost of hospitalization in very-low-birth-weight infants in the era of National Health Insurance in Taiwan: a medical center's experience.

The impact of implementation of the National Health Insurance on the outcome, cost, and length of hospitalization of very-low-birth-weight (VLBW) infants is not clear in Taiwan. These data are important for the planning of medical care and regionalization in this area. This study was an attempt to examine these questions. We retrospectively collected mortality, morbidity, and length and cost of hospitalization data of VLBW (BW < 1500 g) infants between March 1995 and February 1998. There were totally 162 patients enrolled. The overall mortality rate was 21.6%; the birth weight (BW)-specific mo

Progress towards universal health coverage and inequalities in infant mortality: an analysis of 4·1 million births from 60 low-income and middle-income countries between 2000 and 2019.

BACKGROUND: Expanding universal health coverage (UHC) might not be inherently beneficial to poorer populations without the explicit targeting and prioritising of low-income populations. This study examines whether the expansion of UHC between 2000 and 2019 is associated with reduced socioeconomic inequalities in infant mortality in low-income and middle-income countries (LMICs). METHODS: We did a retrospective analysis of birth data compiled from Demographic and Health Surveys (DHSs). We analysed all births between 2000 and 2019 from all DHSs available for this period. The primary outcome was…

Learning From History About Reducing Infant Mortality: Contrasting the Centrality of Structural Interventions to Early 20th-Century Successes in the United States to Their Neglect in Current Global Initiatives.

UNLABELLED: Policy Points Current efforts to reduce infant mortality and improve infant health in low- and middle-income countries (LMICs) can benefit from awareness of the history of successful early 20th-century initiatives to reduce infant mortality in high-income countries, which occurred before widespread use of vaccination and medical technologies. Improvements in sanitation, civil registration, milk purification, and institutional structures to monitor and reduce infant mortality played a crucial role in the decline in infant mortality seen in the United States in the early 1900s. The c…

Higher PRBC transfusion volume and frequency are associated with higher risk of moderate-to-severe bronchopulmonary dysplasia or mortality in extremely preterm infants: A retrospective cohort study.

Several studies have reported inconsistent findings regarding the impact of packed red blood cell (PRBC) transfusions in extremely preterm infants (EPIs). Using a retrospective cohort study design, we investigated the relationship between different volumes and frequencies of PRBC transfusions and the morbidity and mortality in this vulnerable population. During the 3-year research period, our study encompassed 146 infants, ultimately enrolling 101 after a comprehensive screening process. Based on the medium volume of PRBC transfusions, 50 EPIs were classified into a higher-volume PRBC transfus

Effect of Probiotic Supplementation in Preventing Necrotizing Enterocolitis Sepsis and Mortality Among Preterm and Very Low Birth Weight Infants: A Systematic Review and Meta-Analysis.

Preterm and very low birth weight (VLBW) infants are highly vulnerable to gastrointestinal and systemic complications due to immature gut barriers, dysbiosis, and underdeveloped immunity, with necrotizing enterocolitis (NEC) and late-onset sepsis contributing significantly to morbidity and mortality. Probiotic supplementation has been proposed as a strategy to restore gut microbial balance, enhance gastrointestinal maturation, and reduce disease risk. This study systematically reviews the impact of probiotics on NEC, sepsis, mortality, feeding tolerance, hospitalization, and related physiologi
