A Randomized, Controlled Trial of ZMapp for Ebola Virus Infection.

BACKGROUND: Data from studies in nonhuman primates suggest that the triple monoclonal antibody cocktail ZMapp is a promising immune-based treatment for Ebola virus disease (EVD). METHODS: Beginning in March 2015, we conducted a randomized, controlled trial of ZMapp plus the current standard of care as compared with the current standard of care alone in patients with EVD that was diagnosed in West Africa by polymerase-chain-reaction (PCR) assay. Eligible patients of any age were randomly assigned in a 1:1 ratio to receive either the current standard of care or the current standard of care plus

Comprehensive control of human papillomavirus infections and related diseases.

Infection with human papillomavirus (HPV) is recognized as one of the major causes of infection-related cancer worldwide, as well as the causal factor in other diseases. Strong evidence for a causal etiology with HPV has been stated by the International Agency for Research on Cancer for cancers of the cervix uteri, penis, vulva, vagina, anus and oropharynx (including base of the tongue and tonsils). Of the estimated 12.7 million new cancers occurring in 2008 worldwide, 4.8% were attributable to HPV infection, with substantially higher incidence and mortality rates seen in developing versus dev

The membrane as a target for controlling hypervirulent Clostridium difficile infections.

OBJECTIVES: The stationary phase of Clostridium difficile, which is primarily responsible for diarrhoeal symptoms, is refractory to antibiotic killing. We investigated whether disrupting the functions of the clostridial membrane is an approach to control C. difficile infections by promptly removing growing and non-growing cells. METHODS: The bactericidal activities of various membrane-active agents were determined against C. difficile logarithmic-phase and stationary-phase cultures and compared with known antibiotics. Their effects on the synthesis of ATP, toxins A/B and sporulation were also

Epidemiology, surveillance and control of Nipah virus infections in Malaysia.

The outbreak of Nipah virus, affecting pigs and pig-farm workers, was first noted in September 1998 in the north-western part of peninsular Malaysia. By March 1999, the outbreak had spread to other pig-farming areas of the country, inclusive of the neighbouring country, Singapore. A total of 283 human cases of viral encephalitis with 109 deaths were recorded in Malaysia from 29 September 1998 to December 1999. During the outbreak period, a number of surveillances under three broad groups; Surveillance in Human Health Sector, Surveillance in Animal Health Sector, and Surveillance for the Reserv

Treatment and control of chlamydial and rickettsial infections in sheep and goats.

Small ruminants are susceptible to several chlamydial and rickettsial infections. Some of them, such as Ehrlichia ruminantium, have a great impact on the sheep and goat industry while others, such as Coxiella burnetii, are important zoonotic agents. This review focuses on measures of treatment and control for the following organisms: Chlamydophila abortus (formerly Chlamydia psittaci immunotype 1), Coxiella burnetii, Anaplasma ovis, Anaplasma phagocytophilum, and Ehrlichia ruminantium.

Mechanisms controlling anaemia in Trypanosoma congolense infected mice.

BACKGROUND: Trypanosoma congolense are extracellular protozoan parasites of the blood stream of artiodactyls and are one of the main constraints on cattle production in Africa. In cattle, anaemia is the key feature of disease and persists after parasitaemia has declined to low or undetectable levels, but treatment to clear the parasites usually resolves the anaemia. METHODOLOGY/PRINCIPAL FINDINGS: The progress of anaemia after Trypanosoma congolense infection was followed in three mouse strains. Anaemia developed rapidly in all three strains until the peak of the first wave of parasitaemia. T

Intermittent antiretroviral therapy in patients with controlled HIV infection.

BACKGROUND: To assess the safety of a drug-sparing treatment regimen in patients with high CD4 cell counts and controlled HIV replication under antiretroviral therapy. METHODS: An open-label, non-inferiority study involving 403 adults with CD4 cell counts of 450 x 10(6) cells/l or greater and plasma HIV-1-RNA levels less than 200 copies/ml, randomly assigned to switch to an 8-week off, 8-week on regimen or to continue their antiretroviral regimen. The primary endpoint was the proportion of patients reaching a confirmed CD4 cell count less than 300 x 10(6) cells/l. RESULTS: Over 96 weeks, the

Epidemiology and control of trematode infections in cattle in Zimbabwe: a review.

In this paper the main epidemiological aspects of the major domestic ruminant trematode infections in Zimbabwe are reviewed and discussed with regard to the available options for control. Seasonal occurrence of amphistomes, Fasciola gigantica and Schistosoma mattheei are considered both in the definitive and intermediate hosts. The regional distribution of the trematodes is reviewed in relation to the distribution patterns of their snail intermediate hosts. Based on the epidemiological features of the trematodes, practical control measures are suggested.

Bacterial infection in chronic rhinosinusitis: a controlled paired analysis.

BACKGROUND: The aim of this study was to determine if pathogenic bacteria are involved in the pathogenesis of chronic rhinosinusitis (CRS). METHODS: A consecutive series of adult patients with unilateral sinus disease determined by unilateral radiographic involvement or unilateral purulent secretions was microbiologically studied. Aerobic and anaerobic bacterial and fungal cultures were obtained during endoscopic sinus surgery from purulent secretions or tissue culture. Positive culture rates were compared between the diseased sinus and the contralateral nondiseased (control) sinus to determi

[Principles for the control of sexually transmitted infections in tropical areas].

Owing to their frequency and their own complications, sexually transmitted infections (STI) constitute a very classical health problem, especially in developing areas. The interest for those diseases has been on the rise since the beginning of the AIDS pandemic. Conventional sexually transmitted diseases (such as syphilis, chancroid, infections due to Neisseria gonorrhoeae and to Chlamydia trachomatis) are potent cofactors of HIV transmission. It has been established that the treatment of patients with STI was able to lower the HIV incidence at the community level. Today the fight against STI

Multiple sclerosis and antecedent infections: a case-control study.

OBJECTIVES: To determine whether there is an excess of respiratory tract infections in the 5-week, 3-month, and 12-month periods before MS symptom onset and if there is an association between MS and a history of infectious mononucleosis (IM). BACKGROUND: The etiology of MS remains unknown, but infection is frequently suggested as a putative etiologic agent. Epidemiologic studies have produced inconsistent evidence for an etiologic role of respiratory tract infections (RTI) and IM in MS. METHODS: The authors performed a case-control study using the General Practice Research Database from the

Ectopic pregnancy and genital infections: a case-control study.

This case-control study was carried out to evaluate the significance of genital infections as risk factors in ectopic pregnancy. The study population consisted of 86 consecutive patients with ectopic pregnancy and two age- and parity-matched control groups of women with normal early pregnancy; those with planned pregnancy (I; 65 pairs) and those having legal abortion (II; 51 pairs). Histories of salpingo-oophoritis in the two groups (22% against 5%, P less than 0.05), and of cervical Chlamydia trachomatis infection (9% against 0%, P less than 0.05) were more common in patients with ectopic pre

Efficacy of ivermectin in controlling Strongyloides westeri infections in foals.

Twenty-eight foals whose dams were treated IM with ivermectin (200 micrograms/kg of body weight) on the day of parturition were compared with 35 foals whose dams were administered only the vehicle. The effect of ivermectin on the vertical transmission of Strongyloides westeri and foal heat diarrhea was determined by a comparison of results obtained in the 2 groups. Foals from treated mares had significantly fewer S westeri eggs per gram of feces from 17 to 28 days postpartum. There were no differences observed in the frequencies of severity of foal heat diarrhea between the treated and control
