We have described an 81-year-old woman with symptoms of gastric outlet obstruction in whom upper gastrointestinal endoscopy showed an extrinsic mass compressing the superior aspect of the postbulbar duodenum. This was confirmed by an upper gastrointestinal series, and CT revealed migration of a known, previously asymptomatic gallstone through the duodenal wall, forming a cholecystoduodenal fistula.

The surgery of the last ileal ansa in case of non-neoplastic pathology bears some problems of technical nature, bound to the remarkable anatomic variabilities in the arterial blood supply to terminal ileum. The authors report the observation of three cases of non-neoplastic pathology of the last ileal ansa, treated with different technique in relation to the type of arterial blood supply remarked. In a case an ileum resection with termino-terminal anastomosis was performed, and in the other two cases a resection of the last ileal ansa, followed by an ileo-caecal reimplantation, was effected.

Colonic diverticulitis may result in single or multiple fistulae communicating either internally or externally. We report an unusual case of fistulous communication between the colon and the left hip joint secondary to diverticulitis, with subsequent septic arthritis and osteomyelitis of the proximal femur.

Carcinoma developed in an excluded segment of small bowel in an 80-year old male patient, 40 years following the onset of regional enteritis and 33 years after by-pass of the terminal ileum. Within the past four years, four patients with malignancy occurring in excluded segments of the bowel in Crohn's disease have been reported from the institution. Bouveret's syndrome is a rare complication of cholelithiasis. It is characterized by a gallstone entering the intestine through a cholecystoenteric fistula, impacting the duodenum and causing gastric outlet obstruction. Rarely, it presents with hematemesis and melena. Ureteral iatrogenic lesions occur with variable frequency in the different types of surgery, mainly in uncomplicated abdominal hysterectomy. This paper contributes 29 patients (2 male and 27 female) with several types of lesions, secondary to gynaecological, intestinal and vascular surgery. The series produced 16 cases of clamping, 6 ureterovaginal fistulae, 6 ureteral sections (5 total and 1 partial) and 1 ureterouterine fistula. The terminal ureter was the most frequently damaged. The diagnostic and therapeutical methods used in this type of lesions are analyzed.

Ten patients with various stages of enterovesical fistulization in Crohn's disease are discussed. These fistulae can develop at different times in the course of the disease, but may also cause the presenting symptoms. Cystoscopy and radiologic examination rarely visualizes the fistulae. Conservative treatment by medication can sometimes suppress the symptoms, but cannot cure. Surgical treatment, which can consist of resection of intestine but need not involve resection of parts of the bladder, gives good results at least so far as the urinary infection is concerned.

Vesicoenteric fistulas represent challenging clinical problems and may be characterized by difficulty in diagnosis as well as controversy in management. We present our experience with 56 patients with vesicoenteric fistulas. The results of our series were compared with a report of 1,100 in the literature during the past 20 years. Ehlers-Danlos syndrome (EDS) Type IV results in inadequate production of type III collagen and can lead to devastating vascular, gastrointestinal, and uterine complications. A case is presented of spontaneous colonic perforation in a patient with EDS Type IV complicated by multiple intestinal fistulas managed with porcine small intestinal submucosa. Early success with this technique would suggest additional applications in a variety of complicated gastrointestinal problems.

Gallstone ileus is an uncommon form of bowel obstruction. It can occur whenever a stone passes through the common bile duct or a cholecystoenteric fistula. When a stone is in the intestinal track it can either traverse the entire colon to be voided spontaneously or obstruct the small or more rarely the large intestine. We report a case of recurrence of gallstone ileus in a young patient with Crohn's disease. Clinical findings, diagnosis and treatment are presented.

Two female patients with enterovesical fistula complicating Crohn's disease are presented. Case 1: A 40-year-old woman having a 10-year history of diarrhea presented with vesical irritability of a three months duration. Following a difficult nephrectomy for pyonephrosis, two large calculi in the left pelvic ureter were not removed. The upper of these two calculi finally found its way into the bladder after ulcerating into the sigmoid colon and thence into the bladder. The calculi and the residual ureter were removed and the fistulae closed, with a successful outcome.
