Effect of rapid fluid resuscitation using crystalloids or colloids on hemostasis in piglets.

BACKGROUND AND OBJECTIVES: Impairment of blood coagulation is one of the main side effects of volume replacement, particularly if artificial colloids such as hydroxyethyl starch (HES) and gelatine preparations are used. This animal study aimed to evaluate the effect of a single fast intravenous crystalloid or colloid fluid bolus on blood coagulation as measured by rotation thromboelastometry (ROTEM). METHODS: Thirty-two anesthetized piglets were infused with a rapid 20 ml·kg(-1) fluid bolus of either normal saline (NS), 4% gelatine, 5% albumin or 6% HES 130/0.4 (n = 8 per group) over a period

Gastrointestinal toxicity due to bitter bottle gourd (Lagenaria siceraria)--a report of 15 cases.

Traditional medicine is widely practiced in tropical countries. Bottle gourd (Lagenaria siceraria) fruit juice is advocated as a part of complementary and alternative medicine. If the bottle gourd juice becomes bitter it is considered toxic. We report 15 patients, who developed toxicity due to drinking bitter bottle gourd juice. Patients presented with abdominal pain, vomiting, hematemesis, diarrhea and hypotension within 15 min to 6-h after ingestion of bottle gourd juice. Endoscopy showed esophagitis, gastric erosions, ulcers and duodenitis. Hypotension was treated with crystalloids and inot

Fluid administration and the kidney.

PURPOSE OF REVIEW: Intravenous fluids are widely administered in the ICU with the intention of preventing or ameliorating acute kidney injury (AKI). This review focuses on recent studies examining fluid administration and renal function in critical illness to critically examine conventional justifications for fluid administration. RECENT FINDINGS: Early, targeted, resuscitation of inadequate cardiac output in shock may have a beneficial effect on organ function and patient outcome. However, experimental evidence suggests the relationship between fluid administration and an increase in renal o

Colloids versus crystalloids as priming solutions for cardiopulmonary bypass: a meta-analysis of prospective, randomised clinical trials.

Using Cochrane methodology a review was performed of prospective randomized clinical trials comparing colloidal pump priming solutions for cardiopulmonary bypass. Dextrans were not considered. Database searches from 1966 through December 2002 delivered 265 articles. Seventeen studies finally met the eligibility criteria involving 997 patients. Summary odds ratio estimates from the 5 studies reporting mortality were 1.46 (n = 326; 95%-Confidence-Interval: .55 to 3.85; p = .49) for crystalloids against colloids and .74 (n = 150; 95%-Confidence-Interval: .17 to 3.36; p = .49) for albumin versus s

Effects of intravascular perfusion of cooled crystalloid solution on cold-induced brain injury using an extracorporeal cooling-filtration system.

BACKGROUND: We evaluated cerebral metabolic change during brain hypothermia with intravascular perfusion of cooled crystalloid solution using an extracorporeal cooling-filtration system and cerebroprotective effects of this hypothermia on brain injury in an animal model. METHOD: Microdialysis probes were implanted into the bilateral parietal cortices. A cold-induced brain injury was produced behind the microdialysis probe on the right parietal cortex. Immediately after injury in the cooled group (n=9), Ringer's solution cooled to 5 degrees C was infused into the right vertebral artery after o

Full-thickness burn and venous thrombosis following intravenous infusion of microwave-heated crystalloid fluids.

Use of microwave-heated crystalloid fluid has been recommended as one method of correcting hypothermia during resuscitation. We report a case of full-thickness burns and venous thrombosis after microwave-heated crystalloid was infused in the management of a ruptured aortic aneurysm. This case highlights the severity of the burn injury that can occur with infusion of heated fluids. Measuring the temperature of the fluid before the start of the infusion will avoid this complication.

Effect of crystalloid and colloid preloading on uteroplacental and maternal haemodynamic state during spinal anaesthesia for caesarean section.

We have studied the effects of crystalloid 1 litre (lactated Ringer's) or colloid 0.5 litre (hydroxyethyl starch) preloading in 26 healthy parturients undergoing elective Caesarean section under spinal anaesthesia. Maternal placental uterine artery circulation was measured using a pulsed colour Doppler technique with simultaneous measurement of maternal haemodynamics. A high incidence of maternal hypotension was observed during spinal anaesthesia in the crystalloid group (62%) but the incidence was lower in the colloid group (38%). Central venous pressure was increased significantly in both gr

Effects of crystalloid on lung fluid balance after smoke inhalation.

Inhalation injury occurs in 21% of flame burn victims who require large fluid volumes for resuscitation and have a mortality rate greater than 30%. This study was done to determine how vulnerable the smoke-injured lung is to fluid accumulation when crystalloids are infused rapidly. Mongrel dogs were exposed to smoke and 10% body-weight Ringer's lactate in three groups: (I) fluid only, (II) smoke only, and (III) smoke and fluid. The increase in wet-dry lung weight ratio was 2% in Group I, 28% in Group II, and 42% in Group III, consistent with pulmonary edema present only in Group III. The decre

Intravenous fluids in minor gynaecological surgery. Their effect on postoperative morbidity.

Postoperative morbidity was assessed in 100 patients who underwent minor gynaecological procedures. Fifty patients received intra-operative crystalloid (1000 ml compound sodium lactate solution) and the remaining fifty none. Identical short-acting intravenous anaesthetic techniques were used in both groups. There was no statistically significant difference between the groups in symptoms of nausea, vomiting, headache and drowsiness within the first 6 hours after operation. Patients who received intra-operative fluids exhibited a decreased incidence of dizziness within the first 6 hours and a de

Pulmonary effects of crystalloid and colloid resuscitation from hemorrhagic shock in the presence of oleic acid-induced pulmonary capillary injury in the dog.

The effects of resuscitation with crystalloid and colloid solutions in the presence of increased pulmonary capillary permeability were studied. Twenty-four hours after oleic acid administration, dogs were anesthetized and bled to produce hemorrhagic shock. One hour later, resuscitation was performed with saline, 5% albumin, or 6% hydroxyethyl starch solution to restore and then maintain cardiac output at pre-oleic acid values for 6 h. Dogs were recovered and, 24 h later, were reanesthetized for final measurements. Oleic acid administration resulted in increases in pulmonary artery pressure, pu

Breakdown of classical paradigms in lipid membranes. Part II: The permeability paradox.

Water soluble non electrolytes alter the density and the compressibility of the lipid membrane according to its ability to permeate. Glucose, a non-permeant, accumulates at the interphase binding to the PO2- groups promoting water extrusion and decreasing the specific volume and the compressibility without altering the vesicle size. Permeant as glycerol produces a lower change in agreement with its partial permeation. This behavior suggests a reduction in interfacial free volume and a restriction of area fluctuations while the membrane remains in the liquid-crystalline phase correlated with th

Modification of PMMA vertebroplasty cement for reduced stiffness by addition of normal saline: a material properties evaluation.

PURPOSE: Vertebral augmentation is an established treatment for patients with pathological vertebral compression fractures. These procedures typically employ a PMMA-based bone cement, which possesses a high compressive stiffness. Because of the increased risk of subsequent fractures after vertebral augmentations, there is a desire for reducing this stiffness. The goal of our study was to examine the influence of adding isotonic saline on the biomechanical properties of PMMA vertebroplasty cement. METHODS: A PMMA-based vertebroplasty cement was prepared according to the manufacturer's recommen
