The patient presented for a refractive surgery evaluation with a manifest refraction of -0.50 -6.00 × 137 (20/40-1) in the right eye and -2.25 -2.25 × 30 (20/40-1) in the left eye. PURPOSE: To describe a novel technique to reshape the ectatic cornea by means of crescent keratectomy performed with an excimer laser using a mask.
METHODS: A crescent-shaped perforation at the base of the mask allowed the laser ablation to be directed only to the intended region, shielding the remaining cornea. This technique was performed in 3 eyes of 3 patients with keratoconus grade 2 to 3. Arcs of 180° and 360° had been performed depending on the severity of the keratoconus. Contact lens intolerance in keratoconus may be due to the formation of a proud nebula at or near the apex of the cone that gives rise to contact lens related abrasions. These nebulae do not flatten when contact lens wear is discontinued. We describe a simple technique of superficial keratectomy to remove proud nebulae in which the resulting defect healed quickly under a therapeutic hydrogel lens. UNLABELLED: The aim of this study is to find advantages of alcohol assisted epithelial flap creation before surface excimer--laser refractive ablation.
METHODS: One hundred eyes undergoing Epi-Lasek with 20% alcohol in the beginning of procedure. Mean preoperative spherical equivalent was - 3.44 D +/- 1.8 and/from -1.0 to - 8.0 D/spectacle--corrected visual acuity was from 1.0 to 0.8 in 100% eyes.
