Medial Patellofemoral Ligament Repair or Medial Advancement: Is There a Role?

Patellar instability is one of the most prevalent knee disorders, with dislocations occurring in 5 to 43 cases per 10,000 annually. Traumatic patellar dislocation can result in significant morbidity and is associated with patellofemoral chondral injuries and fractures, medial soft tissue disruption, pain, and reduced function, and can lead to patellofemoral osteoarthritis. Chronic and recurrent instability can lead to deformation and incompetence of the medial soft tissue stabilizers. Despite recent gains in understanding the pathoanatomy of this disorder, the management of patients with this

Familial patellar dislocation associated with t(15;20) (q24;q13.1).

Patellar instability is a common debilitating injury affecting young active individuals. It accounts for approximately 3% of all knee injuries. We report a family, of which five members across three generations, who suffered from autosomal dominant familial recurrent patellar dislocation as well as short stature. All of them have recurrent patellar dislocations before the age of 15. The affected patients in all three generations have been genetically screened. Genotypical evaluation revealed a balanced translocation of chromosomes 15 and 20.

Outcomes and Predictors of Popliteal Artery Injury in Pediatric Trauma.

BACKGROUND: Popliteal artery injury (PAI) is a rare occurrence in pediatric patients with significant consequences. Delays in diagnosis lead to severe complications such as lifelong disability and limb loss. We sought to identify outcomes and clinical predictors of PAI in the pediatric trauma population. METHODS: The Pediatric Trauma Quality Improvement Program (2014-2016) was queried for patients ≤17 years old with PAI. Patient demographics and outcomes were characterized. A comparison of patients sustaining blunt versus penetrating PAI was performed. A multivariable logistic regression anal

Severe varus and valgus deformities treated by total knee arthroplasty.

We report a prospective case-controlled study which compared the outcome of knee replacement in seriously deformed and slightly deformed knees. There were 51 knees with varus or valgus deformity greater than 20 degrees matched for age, gender, disease, type of prosthesis and time of operation with a control group in which the alignment of the leg was within 5 degrees of normal. The clinical outcome at a mean 5.5 years was similar in the two groups. Some deformity persisted in 14 patients in the first group, 13 of whom were initially in valgus. These patients had a significantly poorer mean cli

[Limitation in subtalar motion in a patient nine years after treatment for medial subtalar dislocation].

Subtalar dislocations are rare injuries. They may cause serious complications in the long-term. A 27-year-old professional soldier developed medial subtalar dislocation of the right foot due to fall from height and was treated with closed reduction under general anesthesia and below-knee cast fixation for six weeks. At the end of a nine-year follow-up, he began complaining of occasional pain around his ankle. There was minimal loss in subtalar movements. He had no stiffness or instability. The ankle-hindfoot score was 90 according to the American Orthopaedic Foot and Ankle Society clinical rat

A Rare Cause of Popliteal Artery and Vein Transection: A Case Report and Literature Review.

We present a rare occurrence of popliteal vascular injury due to blunt trauma. The patient had an isolated blunt lower extremity trauma. The patient subsequently experienced moderate tenderness and non-expanding hematoma at the popliteal fossa, reduced range of motion at the knee, and diminished distal pulses. X-rays showed a patella dislocation and tibial plateau non-displaced fracture but no knee dislocation. CT angiography showed an abrupt non-opacification of the distal portion of the popliteal artery with an overlying large hematoma. Surgical exploration was performed which revealed a con…

Common peroneal nerve injuries: results with one-stage nerve repair and tendon transfer.

The authors report their experience in the treatment of common peroneal nerve (CPN) injuries using a one-stage procedure of nerve repair and tibialis posterior tendon transfer. A series of 45 patients with traumatic injury and graft repair of the CPN is presented. From 1988 to 1991, the six patients elected for surgery had only nerve repair: five ultimately did not recover, while muscle contraction in the remaining patient was graded M1-2. Since 1991, nerve surgery in our clinic was associated with tendon transfer procedures (39 cases) which were followed by a satisfactory reinnervation rate.

[Reconstruction of hip joint function: old fracture-dislocation of hip joint complicated with deformed healing of upper 1/3 of fractured femur--a case report].

OBJECTIVE: To explore a method of reconstruction of hip joint function after deformed healing of the upper 1/3 of fractured femur as a complication of old fracture dislocation of hip joint. METHODS: A patient with loss of function in hip joint and fusion of knee joint was treated with lock for femur intra-medullary fixation in April 1997. RESULTS: Before operation, the diseased hip joint lost its most functions and the entire lower extremity was disabled because the knee joint had been fused. One year after operation, the follow-up examination revealed that the patient could walk by crutches

Femoral tunnel enlargement after medial patellofemoral ligament reconstruction: prevalence, risk factors, and clinical effect.

BACKGROUND: In recent years, significantly more attention has been focused on the role of the medial patellofemoral ligament (MPFL) in patellar stability, and MPFL reconstruction has become a mainstay of surgical treatment of episodic patellar dislocations. Although previously described in detail after reconstruction of the anterior cruciate ligament, tunnel enlargement has not been investigated after MPFL reconstruction. HYPOTHESES: (1) Femoral tunnel enlargement occurs after MPFL reconstruction. (2) Patella alta, trochlear dysplasia, and tunnel malposition are risk factors for tunnel enlarg

Muscles may contribute to shoulder dislocation and stability.

Joint instability is one of the most common human afflictions, affecting not only the hip, knee, and shoulder but all joints of the body. Surgical repair for joint instability is done commonly, yet has not been able to restore function consistently. Pain, recurrence of instability, joint stiffness, muscle injury, and degenerative joint disease are all too common sequelae. Because the glenohumeral joint is most commonly dislocated, efforts have been directed to understanding shoulder instability. Study has included not only the static restraints (capsulolabrum, articular surfaces, intracapsular

First-Time Patellofemoral Dislocation: Risk Factors for Recurrent Instability.

Patellofemoral instability is a complex problem, which can be difficult to manage. The purpose of this study was to describe the demographics of patients with a first-time patellofemoral dislocation, and identify risk factors for recurrent instability. This was a single institution, institutional review board-approved, retrospective review of >2,000 patients with a patellar dislocation between 1998 and 2010. Inclusion criteria are as follows: (1) no prior history of patellofemoral subluxation or dislocation of the affected knee; (2) X-rays within 4 weeks of the initial instability episode; and

Pigmented villonodular synovitis causing painless chronic knee swelling in an adolescent.

It is uncommon for an adolescent to present with a painless chronic knee effusion without a history of trauma. Acute knee effusions are usually caused by internal derangement, patellar dislocations, or fractures. The differential diagnosis of a chronic knee effusion must include the more uncommon diagnoses of juvenile rheumatoid arthritis, septic arthritis, reactive arthritis, synovial osteochondromatosis, synovial hemangioma, synovial sarcoma, or pigmented villonodular synovitis. Radiographic imaging, plain radiographs, and magnetic resonance imaging in addition to a biopsy specimen are most

Injuries to the hip joint in frontal motor-vehicle crashes: biomechanical and real-world perspectives.

Hip fractures and dislocations in frontal crashes are of substantial concern to clinicians and automotive safety engineers because of the frequency at which hip injuries occur and the associated potential for long-term disability. Impacts to the flexed knees of unembalmed cadavers under loading conditions similar to those that occur in frontal crashes of newer-model vehicles indicate that the hip is the weakest part of the knee-thigh-hip complex and that hip injury tolerance is reduced by hip flexion and adduction from a typical driving posture. These results are being used to develop new knee

The Ali Krogius procedure for treatment of patellofemoral instability should be regarded as obsolete even in skeletally immature patients.

BACKGROUND: Several interventions are established for treating patellofemoral instability in adults. Fewer exist for pediatric patients without damaging the epiphysis. The Ali Krogius (AK) method is currently still being used. Most studies are not current and report varying results in small patient population. The aim of this study is to determine the long-term results of the AK method. METHODS: In this monocentric, retrospective study design, 33 knees in 33 patients who received the AK procedure for recurrent patellar dislocation were assessed. The average age was 20.8 years (range 6-40). Th

[Finite element analysis of the effect of knee movable unicompartmental prosthesis insertion shape and mounting position on stress distribution in the knee joint after replacement].

In unicompartmental replacement surgery, there are a wide variety of commercially available unicompartmental prostheses, and the consistency of the contact surface between the common liner and the femoral prosthesis could impact the stress distribution in the knee after replacement in different ways. Medial tibial plateau fracture and liner dislocation are two common forms of failure after unicompartmental replacement. One of the reasons is the mismatch in the mounting position of the unicompartmental prosthesis in the knee joint, which may lead to failure. Therefore, this paper focuses on the

Ipsilateral Knee Reoperation Rates Following Patello-Femoral Arthroplasty: Analysis of a Statewide Database.

BACKGROUND: Patello-femoral arthroplasty (PFA) is a mainstay treatment of isolated patello-femoral osteoarthritis. Despite comparable outcomes to total knee arthroplasty, the reoperation rate following PFA remains poorly characterized in the literature. This study aimed to determine the cumulative incidence and temporal patterns of ipsilateral knee reoperation following primary PFA. METHODS: A retrospective cohort study was conducted using a statewide all-payer claims database. Adults undergoing primary PFA between 2017 and 2024 were identified (N = 3,715). The primary outcome was time to fir
