Treatment of galactorrhea-amenorrhea.

A patient complaining of abnormal lactation and amenorrhea should be evaluated for a pituitary tumor. The work-up includes a thorough history and physical examination, serum prolactin and thyroid-stimulating hormone (TSH) determinations, and radiographic assessment of the pituitary gland. The treatment of choice for hyperprolactinemia leading to abnormal lactation is bromocriptine mesylate, even when small pituitary tumors are present. Surgery is reserved for patients who fail to respond to medical treatment and have tumors larger than 1 cm.

Intravenous sodium lactate administration in respiratory alkalosis secondary to severe brain injuries.

In acute cerebrovascular accidents the uptake of lactate by the brain tissue is enhanced, possibly in an attempt to buffer the increased local lactic acidosis. In this paper the results obtained by the administration of the lactate in 78 cases of hyperventilatory syndromes secondary to acute brain injuries, and the biochemistry of the cerebrospinal fluid and blood, are reported.

Use of brom-ergocryptine in treating amenorrhea/galactorrhea.

Seven women with amenorrhea and galactorrhea, and no evidence of pituitary adenoma, were treated with brom-ergocryptine for two to 24 weeks. All seven patients had the resumption of regular menses within eight weeks, but six patients required 24 weeks of therapy to completely cease lacting. Two of the five women attempting to conceive became pregnant. Four of the five nonpregnant patients had a total recurrence of their symptoms after discontinuing the drug. Brom-ergocryptine appears to be very effective in inhibiting prolactin secretion and thus reestablishing normal ovulatory menstrual cycle
