Cardiac Arrest and Extracorporeal Membrane Oxygenation in the Peripartum Period.

Maternal cardiac arrest occurs in 1 of 12,000 to 36,000 pregnancies and presents with unique etiologies and physiologic considerations. Pregnancy alters resuscitation dynamics, making key modifications to advanced cardiac life support protocols such as left uterine displacement, early and effective airway management, and consideration of perimortem cesarean delivery, crucial for successful resuscitation. Extracorporeal life support may be indicated in refractory cases and must account for unique maternal physiology, like increased oxygen consumption and cardiac output. Despite its complexity,

Palliative Care Needs in Pediatric Hematologic Oncology: Parent-Child Perspectives Across Stages.

CONTEXT: Children with hematologic malignancies undergo prolonged, intensive therapy that imposes physical, psychological, social, and spiritual challenges on patients and families. Although early palliative care is recommended, little is known about how these needs evolve across the illness course, especially in resource-limited settings. OBJECTIVE: To describe the dynamic palliative care needs of children with hematologic cancers and their families and to identify stage-specific opportunities for family-centered support. METHODS: A descriptive qualitative study was conducted at a national

Benefits of arrhythmia care coordinators.

Arrhythmias (abnormal heartbeats) are common but can be life threatening. Symptoms can be severe and include palpitations, dizziness, blackouts, breathlessness and even chest pain. These can be highly distressing for patients, causing them severe anxiety and depression if they are not well supported. Early diagnosis is essential to reduce mortality and improve quality of life--the most common arrhythmia--atrial fibrillation--can cause a stroke if left untreated or inadequately treated. While sudden cardiac death is less common in a young person, it is usually due to an inherited cardiac condit

Effect of an app-guided advanced cardiac life support algorithm on teamwork, non-technical skills, and workload in simulated in-hospital cardiac arrest: a randomized controlled feasibility trial.

BACKGROUND: In-hospital cardiac arrest carries high mortality despite advances in monitoring, rapid response systems, and adherence to advanced life support (ALS) guidelines. Digital decision-support tools, including mobile applications, are increasingly explored to improve guideline adherence, yet their impact on cognitive workload, team performance, and ALS performance remains uncertain. METHODS: We conducted a prospective, randomized, simulation-based controlled feasibility trial with 40 interprofessional teams (80 participants) comparing ALS performance with and without the use of the 'CP

Cardiac cachexia: the mandate to increase clinician awareness.

PURPOSE OF REVIEW: Heart failure is a frequent problem in an ageing population, associated with high rates of morbidity and mortality. Today, it is important to not only treat heart failure itself but also the related comorbidities. Among them, cardiac cachexia is one of the major challenges. It is a complex multifactorial disease with a negative impact on quality of life and prognosis. Therefore, prevention, early recognition and treatment of cardiac cachexia is essential. RECENT FINDINGS: Cardiac cachexia frequently presents with skeletal as well as heart muscle depletion. Imaging-based dia

Needs of people with dementia in long-term care: a systematic review.

With the aging of the population and the projected increase of dementia in the coming years, it is crucial that we understand the needs of people with dementia (PWD) in order to provide appropriate care. The aim of this study is to determine, using the best evidence possible, the care needs of PWD living in long-term care (LTC). A total of 68 studies, published between January 2000 and September 2010, were identified from six databases. From the selected studies, 19 needs of PWD were identified. The existing evidence suggests that psychosocial needs such as the need to engage in daily individu

A critical reappraisal of the ATLS classification of hypovolaemic shock: does it really reflect clinical reality?

AIM: The aim of this study was to validate the classification of hypovolaemic shock given by the Advanced Trauma Life Support (ATLS). METHODS: Patients derived from the TraumaRegister DGU(®) database between 2002 and 2010 were analyzed. First, patients were allocated into the four classes of hypovolaemic shock by matching the combination of heart rate (HR), systolic blood pressure (SBP) and Glasgow Coma Scale (GCS) according to ATLS. Second, patients were classified by only one parameter (HR, SBP or GCS) according to the ATLS classification and the corresponding changes of the remaining two p

Tension pneumopericardium: case report.

Pneumopericardium is defined as the presence of air within the pericardial space. It is an uncommon complication of blunt or penetrating chest trauma and may also occur iatrogenically. 'Simple' pneumopericardium produces no discernible circulatory compromise; however, simple pneumopericardium may progress rapidly to produce cardiovascular compromise or circulatory collapse, when it is termed 'tension' pneumopericardium. Tension pneumopericardium requires emergent drainage of the pericardial sac. Failure to achieve rapid effective drainage may result in cardiac arrest. Drainage of the pericardi

Ethical issues in clinical practice: cases and analyses.

Given the severity and complexity of the neurologic disorders that can affect infants and children, pediatric neurologists frequently encounter complex and challenging ethical issues. This discussion uses five cases to illustrate common ethical issues and dilemmas that are confronted by pediatric neurologists. These cases involve the provision of artificial nutrition and hydration to patients in vegetative or minimally conscious states, the use of mechanical ventilation for patients with spinal muscular atrophy, controversial therapies for autistic spectrum disorders, the use of medical inform

Posttraumatic stress disorder in women attending human immunodeficiency virus outpatient clinics.

This study examined posttraumatic stress disorder (PTSD) in human immunodeficiency virus (HIV)-positive women seeking medical care. Specifically, we examined traumatic life events, psychiatric treatment, social support, and demographic characteristics in relation to level of PTSD symptoms. We recruited and obtained informed consent from 47 ethnically diverse HIV-positive women from two HIV outpatient clinics in a county medical system. Forty-one of these women provided complete data on measures assessing demographics, traumatic life events, PTSD symptoms, social support, and psychotherapy/medi

Physicians' own preferences to the limitation and withdrawal of life-sustaining therapy.

While limiting and forgoing therapy at the end of life is now accepted on medical, ethical, moral and legal grounds, many Americans continue to die with heroic measures being taken to prevent their death. Recent studies have demonstrated that physicians frequently attend to their patients without knowledge of their preferences with regards to end-of-life issues. It is postulated that a physician's personal preferences with regard to the limitation and withdrawal of life support and active euthanasia would effect the discussion they had with their patients. The purpose of this study was to anal

Can the Routine Use of Patient-Reported Outcome Measures Improve the Delivery of Person-Centered Diabetes Care? A Review of Recent Developments and a Case Study.

PURPOSE OF REVIEW: In recent years, the recommendation for and use of patient-reported outcome measures (PROMs) in routine diabetes care has significantly increased. We review recent evidence and highlight key opportunities and challenges related to the active clinical use of PROMs to support person-centered diabetes care and focus areas for future research in the area. RECENT FINDINGS: Recent pragmatic studies support that integration of multi-dimensional PROMs for diabetes in clinical care as part of a care improvement strategy can be acceptable for and valued by people with diabetes (PWD)

Examining predictors of loneliness among Older Americans Act National Family Caregiver Support Program participants.

Family caregivers may be at particular risk for social isolation and loneliness. Multiple factors can impact caregivers' health and well-being outcomes, including loneliness. Guided by an adaptation of the Stress Process Model of Caregiving, this study uses the 2019 National Survey of Older Americans Act Participants (NSOAAP)-Family Caregiver Support module to inform efforts to reduce loneliness through family caregiver support programs. A hierarchical multiple regression model reveals that caregivers who report more loneliness are more likely to be female, Hispanic, living alone, not a child

Lessons from a research partnership in southwest England to understand community palliative care needs in rural, coastal and low-income communities.

BACKGROUND: The South West Peninsula (Cornwall, Devon, Somerset) has the highest proportion of over 65s (24.2%) and is the only English rural population with greater economic deprivation than in urban areas. Coastal populations have the worst health outcomes in England. Despite innovation among communities to support those with health and care needs in later life, recruitment to palliative care research in the region is low and there has been no evaluation of public health palliative care interventions. OBJECTIVES: A new South West Peninsula Palliative Care Research Partnership was funded for

[What role do nurses play in the "chronic kidney disease" care pathway?].

The introduction of the "chronic kidney disease" care package has strengthened the role of registered nurses (IDE) in the care pathway for adult patients. By becoming involved early in monitoring, prevention, coordination, and support, state-certified nurses and advanced practice nurses help improve patients' quality of life and slow the progression of the disease.
