Why Serum Magnesium Fails: A Narrative Review of Magnesium Biochemistry, Compartmental Exchange, and Endpoint Selection for Supplementation Trials.

Magnesium is an obligate cofactor in more than 600 enzymatic reactions, yet population-level inadequacy is widespread and rarely detected. The reason is largely methodological. Serum magnesium, used in routine care and in most supplementation trials, holds less than one percent of total body magnesium and is homeostatically defended by renal reabsorption and bone exchange, so individuals with substantial tissue depletion may present with values inside the reference range. This narrative review addresses a specific problem: the biochemical endpoints of oral magnesium supplementation trials are …

Intravenous Magnesium Sulphate in Standing Horses: Effects on Physiological Parameters, Plasma Concentration of Magnesium and Nociceptive Threshold Tests☆.

A bolus of 50 mg kg -1 MgSO4 (treatment Mg) or the same volume of saline (treatment S) was infused over 15 minutes in 5 adult healthy horses. T0 was the end of the infusion. Physiological parameters were recorded throughout the study period. Measurements of electrical, thermal, and mechanical nociceptive thresholds were performed at the pelvic limbs at baseline (before T0), and at specific timepoints. Blood samples were taken at fixed timepoints before, during and until 12 hours after the infusion. For statistical analysis, the 95% confidence intervals (CI's) for the differences in nociceptive

Role of protein phosphatase magnesium-dependent 1A and anti-protein phosphatase magnesium-dependent 1A autoantibodies in ankylosing spondylitis.

OBJECTIVE: Although ankylosing spondylitis (AS) is driven by immune-mediated processes, little is known about the presence and role of autoantibodies in this disease. This study was undertaken to investigate whether autoantibodies occur in and are involved in AS. METHODS: We performed human protein microarray analysis of sera derived from patients with AS or other autoimmune disorders to identify autoantibodies associated specifically with AS, and identified autoantibody targeting of protein phosphatase magnesium-dependent 1A (PPM1A) in AS. We performed enzyme-linked immunosorbent assay (ELIS…

The role of magnesium in otology - A narrative review.

Magnesium is believed to have a positive and protective effect on inner ear disorders. We conducted a systematic literature search on the topic. The mesh terms "magnesium", "tinnitus", "sudden hearing loss", "vestibular disorders", "ototoxicity", and "otoprotection" were used for the Medline search. Based on this approach, a narrative review with a structured literature search was conducted. There is good evidence that supplementation with >200 mg magnesium daily protects against the development of age-related hearing loss. A similar protective effect can be assumed for protection against nois…

Magnesium and nerve injury: Mechanisms and applications.

Magnesium is a vital mineral that plays an important role in recovery from nerve injury recovery by inhibiting excitotoxicity, suppressing inflammatory effects, reducing oxidative stress, and protecting mitochondria. The role of magnesium ions in the field of nerve injury repair has garnered substantial attention. This paper aims to review the mechanisms of action and potential applications of magnesium in nerve injury repair. Magnesium ions, as key neuroregulatory factors, substantially alleviate secondary damage after nerve injury by inhibiting N-methyl-D-aspartate receptors, regulating calc…

Importance of Magnesium Status in COVID-19.

A large amount of published research points to the interesting concept (hypothesis) that magnesium (Mg) status may have relevance for the outcome of COVID-19 and that Mg could be protective during the COVID disease course. As an essential element, Mg plays basic biochemical, cellular, and physiological roles required for cardiovascular, immunological, respiratory, and neurological functions. Both low serum and dietary Mg have been associated with the severity of COVID-19 outcomes, including mortality; both are also associated with COVID-19 risk factors such as older age, obesity, type 2 diabet…

Coordination of Phosphate and Magnesium Metabolism in Bacteria.

The majority of cellular phosphate (PO4-3; Pi) exists as nucleoside triphosphates, mainly adenosine triphosphate (ATP), and ribosomal RNA (rRNA). ATP and rRNA are also the largest cytoplasmic reservoirs of magnesium (Mg2+), the most abundant divalent cation in living cells. The co-occurrence of these ionic species in the cytoplasm is not coincidental. Decades of work in the Pi and Mg2+ starvation responses of two model enteric bacteria, Escherichia coli and Salmonella enterica, have led to the realization that the metabolisms of Pi and Mg2+ are interconnected. Bacteria must acquire these nutri…

Magnesium for T cells: strong to the finish!

'Popeye, the Sailor' cartoons taught children that eating spinach boosts strength and helps defend against bullies. Lötscher and colleagues report that dietary deficiency of magnesium ions (Mg2+), against which eating spinach is an excellent antidote, impairs the activity of a key adhesion molecule, LFA-1, and hinders the ability of CD8+ T cells to grapple with assorted bullies, such as tumors and bacteria.

The Role of Magnesium in the Pathogenesis of Metabolic Disorders.

Magnesium (Mg) is an essential nutrient for maintaining vital physiological functions. It is involved in many fundamental processes, and Mg deficiency is often correlated with negative health outcomes. On the one hand, most western civilizations consume less than the recommended daily allowance of Mg. On the other hand, a growing body of evidence has indicated that chronic hypomagnesemia may be implicated in the pathogenesis of various metabolic disorders such as overweight and obesity, insulin resistance (IR) and type 2 diabetes mellitus (T2DM), hypertension (HTN), changes in lipid metabolism…

Magnesium and the Brain: A Focus on Neuroinflammation and Neurodegeneration.

Magnesium (Mg) is involved in the regulation of metabolism and in the maintenance of the homeostasis of all the tissues, including the brain, where it harmonizes nerve signal transmission and preserves the integrity of the blood-brain barrier. Mg deficiency contributes to systemic low-grade inflammation, the common denominator of most diseases. In particular, neuroinflammation is the hallmark of neurodegenerative disorders. Starting from a rapid overview on the role of magnesium in the brain, this narrative review provides evidences linking the derangement of magnesium balance with multiple sc…

Magnesium: The overlooked electrolyte in blood cancers?

Magnesium is an important element that has essential roles in the regulation of cell growth, division, and differentiation. Mounting evidence in the literature suggests an association between hypomagnesemia and all-cause mortality. In addition, epidemiologic studies have demonstrated that a diet poor in magnesium increases the risk of developing cancer, highlighting its importance in the field of hematology and oncology. In solid malignancies, hypomagnesemia at diagnosis portends a worse prognosis. However, little is known about prognosis in patients with hypomagnesemia and blood cancers in ge…

Magnesium deficiency and oxidative stress: an update.

Magnesium deficiency (MgD) has been shown to impact numerous biological processes at the cellular and molecular levels. In the present review, we discuss the relationship between MgD and oxidative stress (OS). MgD is accompanied by increased levels of OS markers such as lipid, protein and DNA oxidative modification products. Additionally, a relationship was detected between MgD and a weakened antioxidant defence. Different mechanisms associated with MgD are involved in the development and maintenance of OS. These mechanisms include systemic reactions such as inflammation and endothelial dysfun…
