Amylases in the Human Vagina.

Dominance of Lactobacillus species in vaginal communities is a hallmark of healthy conditions in the female genital tract. Key nutrients for … cteria in the vaginal community produce amylases that hydrolyze glycogen into simpler sugars (i.e., maltose and maltotriose). These sugars serve as "common goods" that sustain bacterial populations in vaginal communities. Given the temporal changes that are observed in the human vaginal microbiome, we expect the kinds of bacterial amylases produced will a …

Purification and partial biochemical characterization of a membrane-bound type II-like α-glucosidase from the yeast morphotype of Sporothrix schenckii.

The early steps of glycoprotein biosynthesis involve processing of the N-glycan core by endoplasmic reticulum α-glucosidases I and II which … n α1,3-linked disaccharide, over other substrates such as kojibiose (α1,2), trehalose (α1,1) and isomaltose (α1,6). Use of selective inhibitors of processing α-glucosidases such as 1-deoxynojirimycin, castanospermine and australine provided further evidence of the possible type of α-glucosidase. Accordingly, 1-deoxynojirimycin, a more specific inhibitor o …

Intravenous iron therapy results in rapid and sustained rise in myocardial iron content through a novel pathway.

BACKGROUND AND AIMS: Intravenous iron therapies contain iron-carbohydrate complexes, designed to ensure iron becomes bioavailable via the intermediary of spleen and liver reticuloendothelial macrophages. How other tissues obtain and handle this iron remains unknown. This study addresses this question in the context of the heart. METHODS: A prospective observational study was conducted in 12 patients receiving ferric carboxymaltose (FCM) for iron deficiency. Myocardial, spleen, and liver magnetic resonance relaxation times and plasma iron markers were collected longitudinally. To examine the h…

OBJECTIVE: To review the pharmacology, efficacy, and safety of ferric maltol (FM), an oral iron formulation, for iron deficiency anemia (IDA … IBD or CKD and IDA. Emerging data suggest that FM is noninferior to intravenous (IV) ferric carboxymaltose in patients with IBD and IDA. Prior to selecting FM over IV iron products, consideration should be given to time to normalization of Hb, ease of administration, cost, and tolerability. CONCLUSION: FM is a relatively safe, effective oral iron therap …

Financial impact of intravenous iron treatments on the management of anaemia inpatients: a 1 year observational study.

Background Intravenous (IV) iron preparations bypass the difficulties (malabsorption and side effects) associated with oral iron for the treatment of iron deficiency anaemia (IDA). Ferric carboxymaltose (FCM) can be administered as a single infusion over short periods of time but is more expensive than iron sucrose (IS) when the patients are hospitalized. Objectives To evaluate the appropriateness of FCM prescriptions and to establish the economic impact of this management (including disease coding) compared to the use of IV IS. Setting This study was conducted for inpatients in all department…

Effectiveness and safety of ferric carboxymaltose treatment in children and adolescents with inflammatory bowel disease and other gastrointestinal diseases.

BACKGROUND: The treatment of iron deficiency anemia in children with inflammatory bowel disease is a particular challenge and often insufficient. Absorption of orally given iron may be impaired by intestinal inflammation and treatment with oral iron may aggravate intestinal inflammation. This retrospective study is the first to describe the use of intravenous ferric carboxymaltose (FCM) in the pediatric setting. METHODS: All subjects who had received at least one dose of FCM intravenously in the observation period were included in this analysis with data collected for up to 3 months post last…

Attitudes towards ferric derisomaltose among Swiss patients with iron deficiency and their treating physicians: the prospective, observational Real-CHOICE study.

PURPOSE: Iron deficiency (ID) often leads to iron deficiency anemia (IDA). Oral iron supplements are frequently associated with gastrointestinal intolerance and poor adherence. Ferric derisomaltose (FDI), a single high-dose intravenous (IV) infusion, is another option. The real-world Real-CHOICE study investigated the attitudes of patients with IDA and ID without anemia (IDNA) towards IV iron therapy. Secondary objectives were the effectiveness and safety of FDI. METHODS: Real-CHOICE (NCT04318405) enrolled adults with IDA or IDNA in Switzerland. Custom questionnaires evaluated attitudes and t…

Ferric carboxymaltose use in pediatric kidney transplant recipients with iron deficiency.

BACKGROUND: Anemia is a common complication in pediatric kidney transplant recipients (KTR). Post-transplantation anemia (PTA) is multifactorial with iron deficiency being a frequent cause. Oral iron supplementation is often ineffective. Ferric carboxymaltose (FCM) is an intravenous iron formulation but its safety and efficacy in pediatric KTR has not yet been established. METHODS: This is a single-center, retrospective cohort study evaluating all consecutive KT patients diagnosed with iron deficiency (ID) and/or iron deficiency anemia (IDA), defined by KDIGO guidelines, who received FCM betw…

Severe FGF23-based hypophosphataemic osteomalacia due to ferric carboxymaltose administration.

Ferric carboxymaltose (FCM) is a novel iron formulation increasingly prescribed due to its effectiveness and fast infusion time. FCM administration can cause an asymptomatic hypophosphataemia secondary to fibroblast growth factor 23 (FGF23) dysregulation. In patients with chronic iron needs, however, a severe, long-lasting hypophosphataemia can lead to osteomalacia with associated bone pain. Lack of awareness of this complication results in delayed time to diagnosis and significant morbidity. We report a case of a patient with Crohn's disease and chronic iron-deficiency anaemia receiving multi…

The effect of intravenous ferric carboxymaltose on health-related quality of life in patients with chronic heart failure and iron deficiency: a subanalysis of the FAIR-HF study.

AIMS: Patients with chronic heart failure (CHF) show impaired health-related quality of life (HRQoL), an important target for therapeutic intervention. Impaired iron homeostasis may be one mechanism underlying the poor physical condition of CHF patients. This detailed subanalysis of the previously published FAIR-HF study evaluated baseline HRQoL in iron-deficient patients with CHF and the effect of intravenous ferric carboxymaltose (FCM) on HRQoL. METHODS AND RESULTS: FAIR-HF randomized 459 patients with reduced left ventricular ejection fraction and iron deficiency, with or without anaemia, …

Comparison of three methods used in assessment of carbohydrate absorption in malnourished children.

The results of oral lactose and sucrose tolerance tests were compared with jejunal disaccharidase activities and the absorption rates of lactose and sucrose from a perfused segment of jejunum in 8 malnourished children before and after treatment. Treated children, with nearly normal utilization of circulating glucose, showed a good correlation between the oral tolerance test results and the jejunal absorption rates of lactose and sucrose. Malnourished children often showed normal rises in blood glucose despite low disaccharidase activities and poor jejunal absorption rates of glucose, lactose,
