Surgical removal of a third molar at risk for mandibular pathologic fracture: case report and clinical considerations.

Surgical removal of mandibular deeply impacted third molars may be challenging and could be associated with several possible risks. Intra- or postoperative mandibular fracture during or after third molar extraction is a major though rare complication. Before starting the surgical removal of a mandibular deeply impacted third molar, various predisposing factors of mandibular fracture should be considered and kept in mind: adult age, male sex, deep inclusion, tooth ankylosis, and associated bony pathology. The aim of this article is to report and discuss the surgical management of a mandibular d

Superolateral dislocation of the intact mandibular condyle associated with panfacial fracture: a case report and literature review.

Superolateral dislocation of the intact mandibular condyle (SDIMC) is rare. This case report focuses on a 15-year-old teenager who was involved in a motor vehicle accident as well a literature review regarding the SDIMC. Clinical examination demonstrated a diffuse edema in the midfacial area and a left lateral deflection of the mandible, including an open bite and a crepitation in the symphyseal region. Three-dimensional computed tomography scans were taken, which presented a superolateral dislocation of the left mandibular condyle as well as panfacial fracture. The patient was set in intermax

Pathological mandibular fracture after simple molar extraction in a patient with osteogenesis imperfecta treated with alendronate.

Osteogenesis imperfecta (OI), is a heterogeneous group of inherited disorders of connective tissue characterized by bone fragility. Patients with this disease frequently suffer fractures, over 80% of the extremities due to the more intensive mechanical load. Fractures of the facial bones occur very infrequently. Several studies have proved that bisphosphonate therapy may be effective in reducing fracture risk in OI patients. We report here an unusual case of pathological mandibular fracture following simple molar extraction in an adult patient with OI type I and oral alendronic acid treatment.

[Development of treatments and results of mandibular fractures from 1950 to 1978 at the University Center Hospital in Lyon].

Over the period extending from 1950 to 1978, the treatment of mandibular fractures at the CHU of Lyon followed a dual evolutionary course. On the one hand, fractures of the dental part of the bone were treated more and more often surgically via the endobuccal approach, by osteosynthesis using miniature screwed plates. On the other hand, fractures of the region of the condyle benefited from mechanotherapy according to professor Delaire's technique. These therapeutical trends have led to frank improvement in the outcome of these patients, both from the functional and esthetical points of view.

External Fixation for War-Related Mandibular Fractures in a Resource-Limited Setting: A Retrospective Study of 91 Patients.

Background/Objectives: War-related mandibular injuries result in extensive soft-tissue damage, severe comminution, and bone loss, and are associated with high rates of infection and delayed healing. No universally accepted management protocol exists for these injuries. External fixation is commonly used in this context, particularly when internal fixation is unavailable or contraindicated. This study aimed to analyze injury patterns, treatment outcomes, and complications of war-related mandibular fractures treated with external fixation as a primary and definitive stabilization method in a res…

Mandibular Angle Fractures and Third Molar Management: A 10-Year Retrospective Study on Clinical Outcomes and Complication Patterns.

Mandibular angle fractures represent one of the most frequent facial injuries and are often associated with the presence of a third molar within the fracture line. Whether to extract or retain the third molar during surgical treatment remains a topic of ongoing debate. This retrospective study reviewed 218 patients treated for mandibular angle fractures between 2015 and 2025 at the "Department of Maxillofacial Surgery, Umberto I General Hospital, Sapienza University of Rome," to describe clinical outcomes and complication patterns associated with third molar management. All patients had radiol

Remodelling of mandibular condylar head after fixation of fractures with ultrasound activated resorbable pins: A retrospective case series.

The aim of this study was to assess whether the resorption of poly-lactic acid pins, used for condylar head fracture fixation, adversely affect remodelling of the condylar head. A retrospective review of patients was performed that underwent ORIF of CHF with ultrasound activated resorbable poly-lactic acid pins and had CT/CBCT scans of the condyle at least 18 months after surgery, at which point the fixation material was expected to be resorbed. We reviewed the size, shape and position of the condylar head and compared this to the normal side and compared this to the results of conservative ma
