Increase in contralateral prophylactic mastectomy: echoes of a bygone era? Surgical trends for unilateral breast cancer.

OBJECTIVES: Surgical therapy for invasive breast cancer includes breast conservation therapy (BCT), unilateral mastectomy (UM), or bilateral mastectomy, including contralateral prophylactic mastectomy (CPM) with or without reconstruction (± R). The goal of this study was to determine factors associated with CPM. METHODS: A breast cancer database collected from 2000 through 2008 was retrospectively reviewed. Treatment groups analyzed included BCT, UM ± R, and CPM ± R. Variables were compared using ANOVA F-tests and chi-square tests. Multivariate analysis was performed using logistic regression…

Aesthetic outcome and oncological safety of nipple-areola complex replantation after mastectomy and immediate breast reconstruction.

Immediate breast reconstruction (IBR) has become an established procedure for women necessitating mastectomy. Traditionally, the nipple-areola complex (NAC) is resected during this procedure. The NAC, in turn, is a principal factor determining aesthetic outcome after breast reconstruction, and due to its particular texture and shape, a natural-looking NAC can barely be reconstructed with other tissues. The aim of this study was to assess the oncological safety as well as morbidity and aesthetic outcome after replantation of the NAC some days after IBR. Retrospective analysis of 85 patients rec…

Association of routine pretreatment magnetic resonance imaging with time to surgery, mastectomy rate, and margin status.

BACKGROUND: The benefit of breast MRI for newly diagnosed breast cancer patients is uncertain. This study characterizes those receiving MRI versus those who did not, and reports on their short-term surgical outcomes, including time to operation, margin status, and mastectomy rate. STUDY DESIGN: All patients seen in a multidisciplinary breast cancer clinic from July 2004 to December 2006 were retrospectively reviewed. Patients were evaluated by a radiologist, a pathologist, and surgical, radiation, and medical oncologists. RESULTS: Among 577 patients, 130 had pretreatment MRIs. MRI use increa…

Use of multiple drains after mastectomy is associated with more patient discomfort and longer postoperative stay.

BACKGROUND: Seromas constitute a common complication following surgery for breast cancer, and closed drainage is used routinely to reduce its incidence. The aim of this study was to evaluate the influence of number of drains on patient discomfort, seroma formation, and hospital stay during the immediate postoperative period after mastectomy for breast cancer. PATIENTS AND METHODS: Based on a retrospective review of our clinical database, 110 consecutive patients from January 2004 through January 2006 who had undergone a mastectomy and axillary clearance for breast cancer were sent a simple po…

Breast implants following mastectomy in women with early-stage breast cancer: prevalence and impact on survival.

BACKGROUND: Few studies have examined the effect of breast implants after mastectomy on long-term survival in breast cancer patients, despite growing public health concern over potential long-term adverse health effects. METHODS: We analyzed data from the Surveillance, Epidemiology and End Results Breast Implant Surveillance Study conducted in San Francisco-Oakland, in Seattle-Puget Sound, and in Iowa. This population-based, retrospective cohort included women younger than 65 years when diagnosed with early or unstaged first primary breast cancer between 1983 and 1989, treated with mastectomy…

Endoscopic-assisted subcutaneous mastectomy and axillary dissection with immediate mammary prosthesis reconstruction for early breast cancer.

BACKGROUND: Endoscopic surgery has been applied successfully in breast lump excision, breast augmentation, subcutaneous mastectomy for gynecomastia, and axillary dissection. Since subcutaneous mastectomy has been proven to be oncologically safe for early breast cancer, we have sought to develop a reproducible minimally invasive endoscopic-assisted technique to address this condition. METHODS: Between December 1998 and May 1999, endoscopic-assisted subcutaneous mastectomy and axillary dissection with immediate reconstruction using a mammary prosthesis was performed in nine patients with early …

Synchronous elective contralateral mastectomy and immediate bilateral breast reconstruction in women with early-stage breast cancer.

BACKGROUND: The role of elective contralateral mastectomy (ECM) in women with early-stage breast cancer who elect or require an ipsilateral mastectomy and desire immediate bilateral breast reconstruction (IBR) is an intellectual and emotional dilemma for both patient and physician. In an attempt to clarify the rationale for this approach, we reviewed our experience with ECM and IBR and evaluated operative morbidity, the incidence of occult contralateral breast cancer, and patterns of recurrence. PATIENTS AND METHODS: We retrospectively reviewed the records of 155 patients with primary unilate…

[Two cases of coronary artery bypass grafting following radical mastectomy using bilateral internal thoracic arterial grafts].

We report two cases of coronary artery bypass grafting following radical mastectomy using bilateral internal thoracic arterial grafts. One was a 68-year-old woman with angina pectoris, and the other was a 71-year-old woman with old myocardial infarction. Dissection of the internal thoracic arterial pedicles from the chest wall was not so difficult in both cases. The pedicles had good patency and each free flow of them was enough. They had no postoperative complications. It may be a useful method to use the internal thoracic arterial grafts for a case of coronary artery bypass grafting after ra…

Extreme Oncoplasty: Avoiding Mastectomy and Expanding the Limits of Breast Conservation-A Comparative Study of 718 Patients.

BACKGROUND: For many years, large, multifocal/multicentric tumors have routinely been treated with mastectomy. Extreme oncoplastic breast-conserving surgery attempts to achieve complete oncologic resection of large tumors while preserving the natural shape, symmetry, and function of breasts. This paper compares recurrence and survival probabilities for extreme and standard (≤50 mm) tumors. METHODS: A prospective oncoplastic database accrued between 2008 and 2025 was queried for patients with breast cancer treated with reduction mammoplasty and whole breast irradiation. In total, 340 patients …

Impact of Ethnicity on the Uptake of Risk Reducing Mastectomy in Unaffected BRCA1/2 and PALB2 Carriers in Canada.

BACKGROUND: While several studies have reported on factors influencing the uptake of risk-reducing mastectomy (RRM) in patients with germline pathogenic variants (gPV) in BRCA1/2, there remains a paucity of data regarding variations in uptake by ethnicity. METHODS: We conducted a retrospective cohort study of unaffected female patients with a confirmed gPV in BRCA1/2 or PALB2 between March 2000 to March 2023 at two academic institutions in Montreal, Canada. Ethnicity was self-reported as: White/European, Black/African/Caribbean, Asian/Southeast Asian, Hispanic/South/Central American, Middle E…

Impact of the American Society of Breast Surgeons' Guidelines on Genetic Testing and Contralateral Prophylactic Mastectomy Rates.

BACKGROUND: It is unclear whether the American Society of Breast Surgeons' (ASBrS) guideline to offer genetic testing (GT) to all patients with breast cancer (BC) impacted contralateral prophylactic mastectomy (CPM) rates. We sought to describe the trends of GT and CPM rates and to determine predictors of CPM uptake. METHODS: After retrospective review of two prospectively maintained institutional databases, we identified patients with unilateral stage 0-III BC who underwent surgery between January 2016 and July 2020. Trends in GT and CPM rates were described and multivariable logistic regres…

Initial oncological outcomes of nipple- and areola-sparing mastectomy in patients with breast cancer with nipple discharge.

OBJECTIVE: This study aimed to assess the oncological safety of nipple- and areola-sparing mastectomy (NSM) compared to mastectomy without preservation of the nipple-areola complex (NAC) in patients with breast cancer presenting with nipple discharge. METHODS: Clinical data were retrospectively reviewed for 93 patients who underwent NSM and 133 patients who underwent mastectomy without NAC preservation for non-metastatic breast cancer between March 2015 and August 2023 at two hospitals. All patients presented with bloody or serous nipple discharge. Clinicopathological characteristics of both …

Trends in Contralateral Prophylactic Mastectomies Before and After the American Society of Breast Surgeons Consensus Statement.

INTRODUCTION: In 2016, an American Society of Breast Surgeons (ASBrS) statement discouraged contralateral prophylactic mastectomy (CPM) in average-risk women with unilateral breast cancer. Despite evidence of no oncologic benefit and related attempts to discourage the practice, CPM remains prevalent. This study aims to assess CPM trends post-ASBrS statement and factors associated with these trends. METHODS: A retrospective cohort study of patients with primary unilateral breast cancer undergoing complete mastectomy at a single-tertiary center between January 2014 and December 2020 was perform…

Kaposi Sarcoma in the Context of Post-Modified Radical Mastectomy: A New Case Report and Brief Review.

Kaposi sarcoma is a human herpesvirus 8-associated angio-proliferative tumor arising from lymphatic endothelial cells. Four clinical subtypes are known: classic, epidemic, endemic, and iatrogenic. The development of Kaposi sarcoma and lymphedema may be interlinked, where each condition could potentially support the progression of the other. Post-mastectomy lymphedema is a commonly recognized complication following radical mastectomy. Angiosarcoma is the most frequently reported neoplasm in such a situation. We present a 72-year-old female who developed Kaposi sarcoma on the same side of mastec…

Effects of Physical Exercise and Motor Activity on Oxidative Stress and Inflammation in Post-Mastectomy Pain Syndrome.

It is estimated that 10-50% of interventions can generate persistent post-surgical pain. Chronic post-mastectomy pain is a condition persisting for at least three months after surgery. It has been shown that physical activity in the cancer patient allows the improvement of the pain symptom. The aim of this study was to evaluate the effects of physical activity on the intensity and interference of chronic pain in the quality of life of women underwent mastectomy needed for breast cancer removal. The secondary objective was to measure the effects of physical activity on inflammatory and oxidativ…
