Repair of anterior meniscofemoral ligament-posterior horn junction tear of the lateral meniscus: a report of four different cases.

Lateral meniscus posterior horn tears are frequently associated with pivoting trauma and anterior cruciate ligament injuries, but they encompass heterogeneous and incompletely characterized patterns. We present four cases of tears at the junction of the anterior meniscofemoral ligament (MFL) and the posterior horn of the lateral meniscus. Two patterns were identified: an isolated tear and a combined tear associated with an oblique radial tear of the lateral meniscus. These lesions mimic root tears and are often not detected by magnetic resonance imaging and may compromise anterolateral rotatio…

Biomechanical Roles of the Popliteomeniscal Fascicles and Meniscofibular and Meniscotibial Ligaments in Stabilizing the Lateral Meniscus Posterior Horn: A Cadaveric Study.

BACKGROUND: The posterolateral corner (PLC) structures restraining the mobility of the lateral meniscus posterior horn (LMPH) have been described in greater anatomic detail in recent years. However, biomechanical evidence regarding their functional role remains limited. HYPOTHESIS/PURPOSE: The purpose was to investigate the biomechanical roles of the popliteomeniscal fascicles (PMFs), meniscofibular ligament (MFibL), lateral meniscotibial ligament (LMTL), and posterior meniscotibial ligament (PMTL) in stabilizing the LMPH. It was hypothesized that transecting the PMFs would increase mobility …

Protective Effects of Lindera obtusiloba Leaf Extract on Osteoarthritis in Mouse Primary Chondrocytes and a Medial Meniscus Destabilization Model.

Osteoarthritis (OA) is a degenerative joint disorder characterized by progressive articular cartilage degradation, leading to pain, stiffness, and impaired mobility. This study investigated the anti-osteoarthritic effects of Lindera obtusiloba (LO) leaf extract in primary cultured chondrocytes and a mouse model of destabilization of the medial meniscus (DMM)-induced OA. Mouse primary chondrocytes were treated with IL-1β and various concentrations of LO leaf extract (50-150 μg/mL), and analyzed by RT-PCR, Western blotting, and ELISA. For the in vivo experiments, male C57BL/6 mice underwent DMM …

Tendon-suture complex shows better repair outcomes in the reconstruction of the medial meniscus posterior root : a biomechanical study.

AIMS: This study aimed to biomechanically compare three medial meniscus posterior root tear (MMPRT) repair techniques to support their clinical application. METHODS: A total of 18 porcine knee joints and 12 bovine extensor tendons were allocated into three groups: transtibial pullout repair (TPR), tendon reconstruction (TR), and tendon-suture complex (TSC). Each sample underwent cyclic loading (1 to 30 N, 1,000 cycles), followed by failure testing. Displacement, elongation, stiffness, ultimate load, and failure modes were assessed. RESULTS: Significant mean differences were detected between …

Epigenetic Dynamics in Meniscus Cell Migration and its Zonal Dependency in Response to Inflammatory Conditions: Implications for Regeneration Strategies.

Meniscus injuries pose significant challenges in clinical settings, primarily due to the intrinsic heterogeneity of the tissue and the limited efficacy of current treatments. Endogenous cell migration is crucial for the healing process, yet the regulatory mechanisms of meniscus cell migration and its zonal dependency within the meniscus are not fully understood. Thus, this study investigates the role of epigenetic mechanisms in governing meniscus cell migration under inflammatory conditions, with a focus on their implications for injury healing and regeneration. Here, we discovered that a proi…

Effect of loading direction and anatomical location on the ultimate tensile stress, fracture toughness, and failure patterns of knee meniscus.

BACKGROUND: Rupture of the knee menisci is a common injury that can have implications for other conditions, such as osteoarthritis. The fracture toughness of soft tissue (Jc) is a mechanical property that characterizes its resistance to tear extension. To date, Jc of the meniscus has not been quantified. METHODS: Cyclic tensile tests were conducted on meniscus samples to determine Jc and explore its characteristics. Initially, the study investigated the impact of an initial notch on the ultimate tensile stress. This allowed for an understanding of how the presence of a notch affects its struc…

Biomechanical Analysis of Meniscotibial Ligament Tenodesis to Treat Meniscal Extrusion in the Setting of Posterior Medial Meniscus Root Repair.

BACKGROUND: Meniscal extrusion often persists after a medial meniscus root repair. If the meniscus is extruded, the function of the meniscus as a load-sharing device and secondary knee stabilizer is compromised. HYPOTHESIS: It was hypothesized that repairing the meniscotibial ligament (MTL) would decrease meniscal extrusion in the settings of both an isolated MTL tear and a repaired medial meniscus root while also improving medial compartment contact mechanics. STUDY DESIGN: Controlled laboratory study. METHODS: Ten fresh-frozen cadaveric knees (mean age, 50.5 years) were tested in 5 condit…

Common Peroneal Nerve Injury Due to an All-Inside Repair of the Posterior Horn of Lateral Meniscus: A Case Report.

CASE: An 18-year-old man sustained a peroneal nerve (PN) injury during an all-inside repair of the posterior horn of the lateral meniscus from the medial portal. Although he could dorsiflex his ankle actively after emergence from general anesthesia, he had a foot drop on the day after surgery. Exploration of the PN at 5 months postoperatively revealed that the nerve was entrapped by the suture. Fifteen months after a nerve repair using a sural nerve graft, he recovered from the foot drop. CONCLUSION: This case report highlights the risk of PN injury during an all-inside repair of the posterio…

Lateral meniscus with tears or with histologic calcification does not increase the risk of lateral osteoarthritis after medial unicompartmental arthroplasty.

PURPOSE: Meniscal tears or histological meniscal calcifications (in the absence of radiological chondrocalcinosis) are frequent in osteoarthritis. Whether lateral meniscal lesions influence clinical outcomes after medial unicompartmental knee arthroplasty (UKA) is unknown. METHODS: We analyzed 130 patients (130 knees) with medial unicompartmental knee arthroplasties between 2005 and 2015. These 130 knees had full articular cartilage thickness in the lateral compartment and no radiological chondrocalcinosis on preoperative radiographs. The lateral meniscus was analyzed with preoperative MRI an…

Medial meniscus extrusion is invariably observed and consistent with tibial osteophyte width in elderly populations: The Bunkyo Health Study.

We reported that the full-length width of medial tibial osteophytes comprising cartilage and bone parts correlates with medial meniscus extrusion (MME) in early-stage knee osteoarthritis (OA). However, no data exist on the prevalence of MME and its relationship with osteophytes in the elderly population. 1191 elderly individuals (females 57%; 72.9 years old on average) in the Bunkyo Health Study underwent standing plain radiograph and proton density-weighted MRI on knee joints. MRI-detected OA changes were evaluated according to the Whole-Organ Magnetic Resonance Imaging Score. A new method of…

Evaluation of the All-Inside Technique for the Repair of Lateral Meniscus Root Tears at 1 Year After ACL Reconstruction.

BACKGROUND: The presence of a lateral meniscus root tear (LMRT) in patients with an anterior cruciate ligament (ACL) tear makes the knee more unstable and increases the risk of osteoarthritis and osteonecrosis. An all-inside suture repair technique without bone tunnels has been proposed to treat LMRT. PURPOSE: To compare the 1-year postoperative findings between patients who underwent ACL reconstruction combined with LMRT repair (LMRT group) and patients who underwent isolated ACL reconstruction (control group). STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: The LMRT group consi…

Cost-Effectiveness Analysis of a Medial Meniscus Replacement Prosthesis for the Treatment of Patients with Medial Compartment Pain in the United Kingdom.

BACKGROUND: The most common intra-articular knee injury is a meniscal tear, which commonly occurs secondary to trauma following twisting or hyperflexion. Treatment options for meniscal tears can either be surgical or non-surgical, and range from rest, exercise, bracing and physical therapy to surgical intervention, including meniscal repair and partial meniscectomy. In patients with persistent pain following loss of meniscus tissue, treatment can include partial replacement or meniscal allograft transplantation. The NUsurface® prosthesis has been developed as a treatment option for patients ex…

Arthroscopic Repair of Radial Tears in the Junction of the Anterior Horn and Body of the Lateral Meniscus Using an All-Inside Device.

Surgical treatment of radial tears in the junction of the anterior horn and body of the lateral meniscus is indicated in the acute setting and with failure of nonoperative therapy. There are several options for arthroscopic repair; however, these are more technically difficult to perform because of the current surgical instruments available to address these tears. An outside-in technique is the current standard of care for this tear location and pattern. We present a technique for an all-inside side-to-side repair of the lateral meniscus for radial tears in the junction of the anterior horn an…

Associations between biomarkers and histological assessment in individual animals in a destabilization of the medial meniscus (DMM) model of osteoarthritis (OA).

To date, the use of biomarkers for assessing individual severity of osteoarthritis (OA) is limited, and the correlation of histological scores with biomarkers for individual animals in the destabilization of the medial meniscus (DMM) model of OA has not been well investigated. Accordingly, this study investigated how well representative biomarkers in the DMM model reflected specific changes in individual animals. Rats were randomly divided into the OA group and the sham group. OA model was established by destabilization of the medial meniscus (DMM). After 2,4,6,8,10 and 12 weeks (n=14, each we…

Medial meniscus extrusion is a determinant factor for the gait speed among MRI-detected structural alterations of knee osteoarthritis.

OBJECTIVE: Knee osteoarthritis (OA) is one of the most common causes for reduction in gait speed. Research into the mechanism of underlying knee OA pain and other symptoms such as the reduction in the gait speed is essential to development of disease-modifying treatments for knee OA. We examined the magnetic resonance imaging (MRI)-detected structural alterations in knee joints those were associated with gait speed in knee OA patients. DESIGN: In this cross-sectional study, structural alterations in knee joints of 74 knee OA patients (51 females; mean 72.2 years old) were evaluated by MRI, an

Temporomandibular joint imaging: current clinical applications, biochemical comparison with the intervertebral disc and knee meniscus, and opportunities for advancement.

Temporomandibular disorders encompass multiple pathologies of the temporomandibular joint that manifest as middle/inner ear symptoms, headache, and/or localized TMJ symptoms. There is an important although somewhat limited role of imaging in the diagnostic evaluation of temporomandibular disorders. In this manuscript, we provide a comprehensive review of TMJ anatomy, outline potentially important features of TMJ disc ultrastructure and biochemistry in comparison with the intervertebral disc and knee meniscus, and provide imaging examples of the TMJ abnormalities currently evaluable with MRI an…
