Individual and combined effects of somatic and mental disorders on reoperation risk after hip fracture surgery: a nationwide danish cohort study of 110,625 patients.

PURPOSE: Surgical complications after hip fracture surgery are common, particularly among patients with comorbidities. We examined the risk of reoperation concerning individual comorbidity groups, their combinations, and overall comorbidity burden. METHODS: Using nationwide Danish registry data, we identified all patients undergoing hip fracture surgery between 2004 and 2021. Comorbidities were analysed as: (1) 10 major comorbidity groups, (2) all pairwise combinations of the 10 groups, and (3) overall comorbidity burden based on the 10 groups. Reoperation risk was estimated with 95% confiden

[Characteristics of patients who received an indication of involuntary admission, with or without the involvement of a third party, in a Parisian psychiatric emergency unit].

BACKGROUND: The French mental health law, first enacted on July 5, 2011, introduced the possibility of psychiatric commitment in case of extreme urgency (imminent peril - ASPPI). The decision of involuntary admission can then be made by the hospital director based on a medical certificate, without the need of a third party request. This procedure was intended to be applied on an exceptional basis, but its use is steadily increasing against the other types of involuntary care. Our study aimed at comparing the characteristics of patients who had received an indication for involuntary admission d

Offense Characteristics of Incompetent to Stand Trial Defendants Charged With Violent Offenses.

The current study compared offender and offense characteristics of pretrial defendants found incompetent to stand trial (IST) against those described as general offenders by victims in the 2008 Bureau of Justice Statistics (BJS) survey and evaluated factors that differentiated IST defendants who allegedly used weapons from those who did not during the course of a violent offense. IST defendants were older and used "weapons" more frequently than those reported in the BJS survey; however, other characteristics, including use of firearms, did not differ. No demographic, clinical, or legal factors

How sick is the defendant? Forensic and emergency medical aspects at trial.

The evaluation of a defendant's ability to stand trial is a core task in the field of legal medicine as a supportive instrument of jurisprudence. Before the trial, the court-appointed expert informs him-/herself regarding existing somatic and psychological illness(-es) within the context of preparing an expert evaluation, then composing the written expert evaluation, which reflects the condition of the subject at the time of the examination and arrives at an assessment of the ability to stand trial. The decompensation or aggravation of illnesses - in particular in stressful situations, as is t

Infant and Early Childhood Mental Health Training in Psychiatry Residency: National Program Director Perspectives in Canada.

OBJECTIVE: Infant and early childhood mental health (IECMH) is increasingly recognized as relevant to psychiatric practice; however, its integration into psychiatry training in Canada remains unclear. This study examined how IECMH education is currently delivered within Canadian general psychiatry and child and adolescent psychiatry (CAP) residency programs. METHODS: Program directors (PDs) and associate PDs from all Royal College of Physicians and Surgeons of Canada-accredited General Psychiatry residency programs and CAP subspecialty programs were invited to complete an anonymous survey bet

Comparing the effects of jigsaw and lecture-based education on nurses' attitudes and screening competency toward delirium in the intensive care units: A randomized controlled trial.

BACKGROUND: Hospitalization in the ICU is closely related to unwanted complications, including delirium. This condition generally occurs in the form of acute changes in mental status, and frequent fluctuation courses. As delirium can be potentially prevented, providing appropriate education to nurses to deal with its associated risk factors and properly managing it seems to be imperative. MATERIALS AND METHODS: A randomized clinical trial with two parallel groups was done in two referral educational hospitals. In total, 75 ICU nurses were included. The nurses were randomly divided into, the j

"The Timeless Explosion of Fantasy's Dream": How State Courts Have Ignored the Supreme Court's Decision in Panetti v. Quarterman.

Multiple states have enacted statutes to govern procedures when a state seeks to execute a person who may be incompetent to understand why s/he is being so punished, an area of the law that has always been riddled with confusion. The Supreme Court, in Panetti v. Quarterman, sought to clarify matters, ruling that a mentally ill defendant had a constitutional right to make a showing that his mental illness "obstruct[ed] a rational understanding of the State's reason for his execution."However, the first empirical studies of how Panetti has been interpreted in federal courts painted a dismal pict

Applying the Project ECHO Model to Support Implementation and Sustainment of Cognitive Behavioral Therapy for Psychosis.

INTRODUCTION: Project Extension for Community Healthcare Outcomes (ECHO) is a teleconsultation model that leverages technology to sustain specialized interventions in underresourced settings. We present the application of the ECHO model to longitudinal training and consultation for community behavioral health providers learning to deliver cognitive behavioral therapy for psychosis, an evidence-based psychotherapy for individuals with psychotic disorders that has poorly penetrated the US mental health system. METHODS: We analyzed within-group change over practitioners' 6-month ECHO participati

Development of a multi-dimensional scale to measure trauma associated with child sexual abuse (MSCSA) and its ramifying impacts on children: A pilot study.

BACKGROUND: With the rising incidence of CSA in India and absence of culturally competent tool to assess the impact of trauma, there is a dire need for development of a comprehensive scale to assess the impact of trauma on children. Thus, the present study aims to develop a multi-dimensional CSA tool for children aged between 7 and 13 years. METHOD: Qualitative research method of FGD/Key Interviews with 4 group participants (Parents, counsellors, mental health and medical professionals) and in-depth interview with children having history of CSA was conducted along with item pooling from exist

Managing simple food allergy in community settings: A pilot study investigating a new model of care.

AIM: The prevalence of food allergy in Australia has increased, paralleled by an increase in waiting time to access tertiary paediatric allergy care. We aimed to test whether a new model of care, based on serum specific IgE testing, was feasible and acceptable to Australian families. METHODS: A prospective pilot intervention study was conducted in community paediatric practices within 20-40 km of The Royal Children's Hospital, Melbourne. Children ≤7 years with likely food allergy referred to the Department of Allergy and Immunology at RCH were included; children with anaphylaxis, drug allergy

Discharges against medical advice: a community hospital's experience.

OBJECTIVES: To understand the characteristics of patients who leave hospital against medical advice (known as "discharges against medical advice" [DAMA]) in a small community hospital and to study how these patients compare to current literature on the topic. To evaluate chart documentation pertaining to such discharges. METHODS: A retrospective chart audit was performed, covering a 2-year period, on patients who had discharged themselves against medical advice. The data were compared to the general patient population of the same period. Evaluation of DAMA documentation was also conducted by

Play and prosociality are associated with fewer externalizing problems in children with developmental language disorder: The role of early language and communication environment.

BACKGROUND: Children with developmental language disorder (DLD) are at higher risk of poorer mental health compared with children without DLD. There are, however, considerable individual differences that need to be interpreted, including the identification of protective factors. AIMS: Pathways from the early language and communication environment (ELCE, 1-2 years) to internalizing (peer and emotional problems) and externalizing (conduct problems and hyperactivity) problems in middle childhood (11 years) were mapped using structural equation modelling. Specifically, the role of indirect pathwa

Advancing Women's Mental Health Education in Psychiatry Residency Training Programs: Insights from a Trainee Needs Assessment.

This study assessed resident psychiatrist clinical confidence regarding women's mental health (WMH) topics and interest in receiving WMH education. The authors analyzed cross-sectional survey data from 152 Postgraduate year (PGY) 1-4 Residents in U.S. Adult Psychiatry Residency Training Programs between 12/3/2024-5/29/2025. Relative within-individual confidence in perinatal-focused WMH practice compared to general practice was assessed using paired Wilcoxon signed rank tests. Predictors of the relative confidence gap between perinatal and general practice were evaluated using multiple regressi

Contextual consideration of the emergence of alexithymia: A case study highlighting cross-cultural client factors and limited English proficiency.

Alexithymia, characterized by deficits in recognition or expression of emotional experiences, has been demonstrated to be associated with depressive symptoms. In psychotherapy, alexithymia can partly manifest as stunted, disfluent speech when an individual attempts to describe his or her subjective experiences. However, similarly stunted, disfluent speech can be observed in individuals with limited English proficiency who are not diagnosed with a depressive disorder. For individuals who present with both symptoms of depression and limited English proficiency, it can be difficult to determine i

Perceptions of Fetal Alcohol Spectrum Disorder (FASD) at a Mental Health Outpatient Treatment Provider in Minnesota.

Resulting from prenatal exposure to alcohol, Fetal Alcohol Spectrum Disorder (FASD) is characterized by deficits in adaptive and cognitive functioning. This disorder is typically accompanied by co-occurring disorders and conditions (e.g., mood, anxiety, psychosis, and substance use disorders). This complicated presentation of diverse symptoms makes the process of screening, assessing, and diagnosing FASD very difficult, limiting the likelihood that clients receive the treatment and services that they need. Although mental health care providers have an opportunity to intervene on behalf of clie

Community-Sourced Intervention Programs: Review of Submissions in Response to a Statewide Call for "Promising Practices".

This study was initiated to add to the nascent literature on locally-grown intervention programs in the youth mental health, child welfare, and juvenile justice service sectors, many of which demonstrate practice-based or community-defined evidence, but may not have been subjected to empirical evaluation. Characteristics of applications submitted in response to three public calls for additions to an inventory of research-supported intervention programs were reviewed on evidence for effectiveness, the use of key quality assurance (QA) elements (e.g., clearly specified training or integrity moni

A psycho-legal perspective on sexual offending in individuals with autism Spectrum disorder.

It is important to consider whether there are innate vulnerabilities that increase the risk of an individual with an autistic spectrum disorder (ASD), predominantly those defendants with a diagnosis of Asperger's Syndrome, being charged and convicted of a sexual offence. The significance of such can be readily seen in recent English case law, with judgments on appeal finding convictions unsafe where there have been a number of failings in the Judge's summing up. In this article, we will consider the gravity of Judges omitting to highlight a defendant's diagnosis of autism spectrum disorder and

Physician-Assisted Suicide: Considering the Evidence, Existential Distress, and an Emerging Role for Psychiatry.

Physician-assisted suicide (PAS) is one of the most provocative topics facing society today. Given the great responsibility conferred on physicians by recent laws allowing PAS, a careful examination of this subject is warranted by psychiatrists and other specialists who may be consulted during a patient's request for PAS. In this article, recent evidence regarding the implementation of PAS in the United States and The Netherlands is reviewed. Support is found for some concerns about PAS, such as the possibility that mental illness occurs at higher rates in patients requesting PAS, but not for

Evaluating the psycholegal abilities of young offenders with fetal alcohol spectrum disorder.

Individuals with a diagnosis of fetal alcohol spectrum disorder (FASD) experience a range of physical, cognitive, and behavioral deficits thought to interfere with their ability to competently navigate the arrest, interrogation, and trial process. This study examined the psycholegal abilities of young offenders with FASD, including their understanding and appreciation of Miranda rights, and adjudication capacities (factual knowledge of criminal procedure, appreciation of the nature and object of the proceedings, ability to participate in a defense and communicate with counsel).
