Middle ear pathology can affect the ear-canal sound pressure generated by audiologic earphones.

OBJECTIVE: To determine how the ear-canal sound pressures generated by earphones differ between normal and pathologic middle ears. DESIGN: Measurements of ear-canal sound pressures generated by the Etymtic Research ER-3A insert earphone in normal ears (N = 12) were compared with the pressures generated in abnormal ears with mastoidectomy bowls (N = 15), tympanostomy tubes (N = 5), and tympanic-membrane perforations (N = 5). Similar measurements were made with the Telephonics TDH-49 supra-aural earphone in normal ears (N = 10) and abnormal ears with mastoidectomy bowls (N = 10), tympanostomy t

Oral steroids for the resolution of otitis media with effusion (OME) in children (OSTRICH): study protocol for a randomised controlled trial.

BACKGROUND: Otitis media with effusion (OME) is an accumulation of fluid in the middle ear affecting about 80 % of children by the age of 4 years. While OME usually resolves spontaneously, it can affect speech, behaviour and development. Children with persistent hearing loss associated with OME are usually offered hearing aids or insertion of ventilation tubes through the tympanic membrane. Oral steroids may be a safe and effective treatment for OME, which could be delivered in primary care. Treatment with oral steroids has the potential to benefit large numbers of children and reduce the burd

Perfusion Steroid via Ventilation Tube as Salvage Treatments for Sudden Sensorineural Hearing Loss.

Objectives: Intratympanic steroid injection (ISI) for sudden sensorineural hearing loss (SSNHL) is a relatively popular and effective method, but there is no standardized method for intratympanic steroids for the treatment of SSNHL and no consensus on how to deliver steroids to the middle ear. The purpose of this study was to compare 2 means of intratympanic steroid delivery as therapy for SSNHL. Methods: A retrospective chart review was performed for the period from November 2018 to October 2022 at our Department of Otorhinolaryngology-Head and Neck Surgery. Sixty patients with profound SSNHL

Micro-Computed Tomography-Based Three-Dimensional Anatomical Structure of the Region Around the Pterygoid Hamulus.

OBJECTIVE: Palatoplasty would involve the structures around the pterygoid hamulus. However, clinicians hold different opinions on the optima … muscles around the pterygoid hamulus, which play essential roles in the velopharyngeal function and middle ear ventilation. Currently, cleft palate repair has limited treatment options with proven successful outcomes.

Air and Bone Conduction Frequency-specific Auditory Brainstem Response in Children with Agenesis of the External Auditory Canal.

INTRODUCTION: Up to 80% of children have been affected by otitis media with effusion (OME) by the age of 4 years, but prevalence declines beyond 6 years of age. Non-purulent middle-ear infections can occur in children or adults after upper respiratory tract infection or acute otitis media. Half or more of cases resolve within 3 months and 95% within 1 year, but complications such as tympanic membrane perforation, tympanosclerosis, otorrhoea, and cholesteatoma can occur. METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the ef

Neuroepithelial cysts of the middle ear.

Neuroepithelial cysts are lesions of the central nervous system that have previously been reported in cerebral parenchyma, in the spinal cord, and within the ventricles in association with the choroid plexus. We describe 2 cases of neuroepithelial cysts of the middle ear. One was diagnosed after surgery for a retraction pocket and chronic otitis media complicated by an intraoperative cerebrospinal fluid (CSF) leak. The other produced bilateral spontaneous CSF otorrhea and mimicked the presentation of arachnoid granulations. Both cases were verified with immunohistochemical stains. Neuroepithel

Adenoidectomy for otitis media with effusion (OME) in children.

BACKGROUND: Otitis media with effusion (OME) is an accumulation of fluid in the middle ear cavity, common amongst young children. The fluid may cause hearing loss. When persistent, it may lead to developmental delay, social difficulty and poor quality of life. Management of OME includes watchful waiting, autoinflation, medical and surgical treatment. Adenoidectomy has often been used as a potential treatment for this condition. OBJECTIVES: To assess the benefits and harms of adenoidectomy, either alone or in combination with ventilation tubes (grommets), for OME in children. SEARCH METHODS:

Clinical Practice Guideline: The Treatment of Cleft Lip and Palate Deformities.

BACKGROUND: Cleft lip and palate deformities (CLPD) constitute one of the more common types of congenital malformation. In Germany, the prevalence is more than 1 in 500 births. The treatment is complex; multistep interdisciplinary treatment is needed for full recovery. In this S3-level clinical practice guideline, we provide evidence- and consensus-based recommendations for the treatment of patients with CLPD. METHODS: A systematic literature search based on key questions was carried out. All recommendations and statements were formulated by specialists from multiple disciplines and were adop

Office-based endoscopic procedure for diagnosis in conductive hearing loss cases using OtoScan Laser-Assisted Myringotomy.

OBJECTIVES/HYPOTHESIS: In conductive hearing loss (HL), even though preoperative examinations including audiometry, tympanometry, and computed tomography (CT) scan are applied, exploratory tympanotomy is necessary to make an exact diagnosis of ossicular interruption and stapes fixation. Precise preoperative diagnosis would be useful for patient counseling, surgical indications, and planning. The advantages of office-based OtoScan laser-assisted myringotomy (LAM), which makes a circular and non-hemorrhagic perforation, prompted us to investigate other uses of LAM in addition to ventilation and

Reimagining grommet follow-up care: audiology-led outcomes and cost comparison with ENT (Ear, Nose and Throat) and nurse-led models.

BACKGROUND: Grommet insertion is one of the most common surgeries in childhood. While post-operative care has been traditionally provided by Ear, Nose and Throat (ENT) specialists, workforce capacity challenges have led to the delegation of care to alternate workforces, including audiology. This study aims to evaluate safety and effectiveness of audiology-led post-grommet services and to compare costs of audiology-led models with traditional medical and simulated nurse-led models. METHODS: A retrospective cohort study was conducted in a regional New Zealand public hospital including all 247 ch
