The 1990s and beyond: determining the need for community health and primary care nurses for rural populations.

Increased numbers of primary care and advanced practice nurses with unique generalist skills will be required to meet the accelerating physiologic and sociocultural health care needs of rural populations. Several factors have been identified that will influence the demands and position of community-based nurses in rural practice settings during the next decade. A back-to-basics type of health care offered out of a growing elderly population; technological breakthroughs that make it possible for more chronically ill patients to live at home; serious substance abuse and other adolescent problems

Acceptability of point-of-care viral load testing to facilitate differentiated care: a qualitative assessment of people living with HIV and nurses in South Africa.

BACKGROUND: Providing viral load (VL) results to people living with HIV (PLHIV) on antiretroviral therapy (ART) remains a challenge in low and middle-income countries. Point-of-care (POC) VL testing could improve ART monitoring and the quality and efficiency of differentiated models of HIV care. We assessed the acceptability of POC VL testing within a differentiated care model that involved task-shifting from professional nurses to less highly-trained enrolled nurses, and an option of collecting treatment from a community-based ART delivery programme. METHODS: We undertook a qualitative sub-s

Nurse-led intensive care follow-up interventions in low- and middle-income countries: A scoping review.

INTRODUCTION: Post-intensive care syndrome affects more than half of intensive care unit (ICU) survivors, resulting in long-term physical, psychological, and cognitive impairments. Structured post-ICU recovery programs, including nurse-led follow-up, have improved patient outcomes in high-income countries. However, little is known about such models in low- and middle-income countries (LMICs), where post-ICU care is often lacking. OBJECTIVE: The aim of this scoping review was to identify and map the existing nurse-led follow-up care interventions for ICU survivors in LMICs to inform future dev

Geographic Rounding on Young Adults With Childhood-Onset Chronic Conditions: A Mixed Methods Retrospective Observational Cohort Study.

Background Geographic rounding has been associated with improved efficiency and quality of care on inpatient general medicine services. Young adults with childhood-onset chronic conditions (COCC) often experience longer lengths of stay and higher readmission rates than other general medicine populations. To evaluate whether geographic rounding could improve outcomes for this group, these patients were transitioned to a geographic team. Methods We conducted a retrospective observational cohort study comparing length of stay (LOS) and 30-day readmission rates for hospitalized young adults with C

Are psychotropic medications equivalent in fall risk among nursing home residents? A meta-analysis of pharmacologic subtypes and polypharmacy effects.

BACKGROUND AND OBJECTIVE: Falls are a major concern for nursing home residents, and psychotropic medications are key risk factors. However, the differential fall risk among pharmacologic subtypes remains unclear, as exemplified by the comparison of typical and atypical antipsychotics, which impedes precise medication safety management. The aim of this study is to compare the fall risk across specific pharmacologic subgroups of psychotropic medications in nursing home residents through a systematic review and meta-analysis. METHODS: This systematic review and meta-analysis involved comprehensi

Using novel natural language processing approaches to examine age-friendly communication about nursing Nome residents with dementia.

BACKGROUND AND OBJECTIVES: The Age-Friendly Health Systems model has emerged as a geriatric care model focusing on the 4Ms: what matters, mobility, mentation, and medications. Representation of the 4Ms in interdisciplinary communication could have implications for outcomes including avoidable nursing home-to-hospital transfers. The objective of this article was to explore the association between the 4Ms found in text messages and avoidable transfer of residents with and without Alzheimer's Disease and Related Dementias (ADRD). RESEARCH DESIGN AND METHODS: The researchers merged data from two
