Expression and localization of Nell-1 during murine molar development.

Nel-like molecule-1 (Nell-1) is a recently discovered secreted protein that plays an important role in osteoblast differentiation, bone formation, and bone regeneration. However, its expression and distribution during tooth development are largely unknown. The aim of this study was to investigate the expression patterns of Nell-1 during murine molar development by immunohistochemistry. Nell-1 protein was expressed during molar development in embryonic and postnatal Kunming mice, but its expression levels and patterns at various developmental stages differed. At embryonic day 13.5 (E13.5) and E…

Morphogenesis and bone integration of the mouse mandibular third molar.

The mouse third molar (M3) develops postnatally and is thus a unique model for studying the integration of a non-mineralized tooth with mineralized bone. This study assessed the morphogenesis of the mouse M3, related to the alveolar bone, comparing M3 development with that of the first molar (M1), the most common model in odontogenesis. The mandibular M3 was evaluated from initiation to eruption by morphology and by assessing patterns of proliferation, apoptosis, osteoclast distribution, and gene expression. Three-dimensional reconstruction and explant cultures were also used. Initiation of M3…

Chronological course of third molar eruption in a Portuguese population.

Forensic age determination has become increasingly important over the past few years. Dental age estimation is frequently used as a part of this process due to the technique simplicity and reliability. This process is done taking into account pre-established values that, often, concern the population of the undocumented person. In this manner, population parameters are needed. In this paper, the authors present data concerning the third molar eruption in a Portuguese population: The minimum age for alveolar emergence of third molars ranged from 6.6 to 11.3 in females and 7.4 to 8.3 in males, f…

Bilateral lower second molar impaction in teenagers: an emergent problem?

The frequency of simultaneously impacted second and third molars in teenagers is increasing and becoming a common occurrence in adolescent oral surgery practice. The traditional treatment is the removal of the third molar by conventional access but repositioning of the surgical flap to the distal face of the first molar can predispose to complications such as pericoronitis and delayed healing of the attached gingiva. We present a case in which we use the germectomy approach to remove the impacted third molar for the eruption of the second molar through a vestibular incision. This incision offe…

Molar pregnancy with false negative urine hCG: the hook effect.

Urine pregnancy tests are widely used in emergency departments as the first screening test for patients of reproductive age presenting with gynaecological problems in order to rule out pregnancy. Conditions such as complete molar pregnancy, which produces large amounts of beta human chorionic gonadotropin (beta-hCG), may cause a false negative result due to an oversaturation of the assay system, known as the "hook effect". We report a case where the exclusion of pregnancy by urine testing led to the initial misdiagnosis of a molar pregnancy as a degenerative fibroid. Physicians need to be remi…

Temporomandibular joint disorders in patients referred for third molar extraction.

BACKGROUND: Third molar removal has been implicated as a precipitating event for temporomandibular joint disorders. The aim of this study was to determine what proportion of patients had pre-existing pain and dysfunction that could be attributed to the temporomandibular joints. METHODS: Sixty patients referred for third molar removal were clinically examined and a history of their presenting complaint recorded at the initial consultation visit. Patients were then diagnosed and categorized. RESULTS: Of the total number of patients examined, 13.3 per cent showed signs and symptoms of temporoma…

Influence of first premolar extraction on mandibular third molar angulation.

OBJECTIVE: To compare the angular changes in the developing mandibular third molars in both first premolar extraction and nonextraction cases and to determine whether premolar extraction results in a more mesial movement of the mandibular buccal segment and causes favorable rotational changes in the mandibular third molar tilt, which can enhance later eruption of the third molars. MATERIALS AND METHODS: Pretreatment (T1) and posttreatment (T2) panoramic radiographs were taken of 25 subjects who had been treated by the extraction of all the first premolars and 25 subjects who had been treated …

Management of third molar-related nerve injuries: observe or treat?

Injury to branches of the trigeminal nerve is one of the possible complications associated with the removal of third molars. Fifty-two percent of patients referred to a university-based practice with trigeminal neurosensory complaints associated their nerve injury with third molar odontectomy. The inferior alveolar nerve (IAN) was the most commonly injured nerve (61.1%), followed by the lingual nerve (LN; 38.8%). There was a 1.5-times greater incidence in females, the mean age of the patients was 34.5 years, and the mean time to consultation from the third molar extraction date was 6.9 months.…

Management of a partial molar pregnancy: a case study report.

Partial molar pregnancy with coexisting fetus is a rare complication of pregnancy and carries significant risks to both the mother and the fetus. Maternal risks include abnormal bleeding and the development of preeclampsia. The fetus frequently develops abnormally, often due to abnormal karyotype. This case presents a woman with a partial molar pregnancy with coexisting fetus, including diagnosis, plan of care, and delivery information.

Unusual complications associated with third molar surgery: a systematic review.

OBJECTIVE: Third molar removal is a frequent surgical procedure. Common complications associated with third molar surgery are well-recognized and frequently explained to patients during the process of informed consent. The general dental practitioner, as well as the oral and maxillofacial surgeon, must be familiar with all possible complications. This systematic review serves as a reminder of the unusual complications of this routine procedure. METHOD AND MATERIALS: Studies were located using systematic searches in Medline and the Cochrane Library electronic databases, as well as hand searchi…

Effects of early unilateral first molar extraction on skeletal asymmetry.

INTRODUCTION: The purpose of this study was to investigate dental and skeletal asymmetry in patients who had unilateral first molar extractions. METHODS: Two study groups were formed according to the location of the extracted first molars. Group I included 25 subjects who had a maxillary permanent first molar extracted (mean age, 18.25 years). Group II included 26 subjects who had a mandibular permanent first molar extracted (mean age, 17.75 years). The control group included 30 subjects with no missing teeth and normal occlusion (mean age, 18.50 years). Dental and skeletal asymmetry values w…

A case of intrauterine molar pregnancy with coexistent ectopic pregnancy.

We describe a woman who presented to the Emergency Department (ED) with vaginal bleeding and abdominal pain. She was initially diagnosed by the emergency physician with a molar pregnancy by transvaginal ultrasound, which was confirmed and treated by the consulting obstetrical service with a dilatation and curettage the following day. The patient was discharged home later that same day and subsequently returned to the ED after several hours complaining of an acute worsening of her abdominal pain with associated fatigue and lightheadedness. Transabdominal ultrasound performed by the emergency ph…

Gestational trophoblastic neoplasia following molar ectopic pregnancy: a case report.

BACKGROUND: Ectopic molar pregnancy is a gestational trophoblastic disease (GTD) of rare occurrence and therefore not always remembered as a diagnostic possibility. CASE: We describe a case of molar ectopic pregnancy in a primiparous woman who developed gestational trophoblastic neoplasia and required chemotherapy to achieve remission. CONCLUSION: This case stresses the important role of histopathologic examination in establishing a diagnosis of ectopic molar pregnancy. Moreover, close follow-up of human chorionic gonadotropin levels is required when a GTD is suspected.

Supernumerary tooth in the primary molar region: a case report.

Supernumerary teeth are among the most common dental anomalies affecting the primary and permanent dentition. They are usually found in the anterior maxilla and occur infrequently in the primary dentition. The purpose of this paper was to report a case diagnosed with primary supernumerary tooth in the primary second molar region. The crown and root shape of the primary supernumerary tooth resembled that of the primary first molar. On radiographic examination, the primary supernumerary tooth was followed by a permanent supernumerary successor with an unusually big crown. The primary supernumera…

External invasive resorption in a three-rooted lower first molar.

Several pathoses can be present concurrently in one tooth, and if that tooth has an unusual anatomical variation, the diagnosis and treatment can be further complicated. This case stresses the importance of accurate assessment and diagnosis prior to intervention and their role in identifying cases for referral to a specialist in today's increasingly litigious environment.

[Recurrent hydatidiform mole. Case report of 9 successive molar pregnancies].

The recurrent hydatiform mole is rare, its frequency is lower to 1%. We report the observation of a 28 years old women. without antecedents of molar pregnancy in the family and without considerable pathological antecedents. She had 9 successive molar pregnancies, without inset normal pregnancies and without living children. The delay of at least 1 year between the gestations has been respected in 6 pregnancies. The etiologic balance in particular the caryotypes of parents is normal. The authors debate the étiopathogénic and prognostic factors of this pathology.

[Immunocytochemical and molecular studies with primary cultures of molar tissue].

INTRODUCTION: Gestational trophoblastic disease includes a group of pathologies characterized by abnormal trophoblast growth and invasion. The molecular bases of the disease are largely unknown, due in part to the lack of appropriate biological models. The insulin-like growth factor (IGF) system plays a fundamental role in the growth and development of many tissues and is involved in the progression of several diseases. OBJECTIVES: Primary cell cultures derived from first trimester placenta were characterized from patients with complete hydatidiform mole and spontaneous non molar abortion by …

Autotransplantation of an impacted third molar: an orthodontic case report.

This case report describes the use of autotransplantation as part of an orthodontic treatment plan where there was early loss of an upper molar tooth secondary to irreversible pulpitis. An impacted lower third molar, which had symptoms of pericoronitis, was transplanted into the upper left second molar socket. Fixed orthodontic treatment, to correct the patient's initial complaint of crowding, was completed in 22 months. The transplanted tooth remained vital and functional throughout.

Asymmetry in the condylar long axis and first molar rotation.

Asymmetric growth occurs frequently in the mandibulofacial region, but little attention has been given to asymmetry in the temporomandibular joint. The purpose of this study was to clarify the feature of asymmetry in the condylar long axis and its relation to upper first molar rotation. Records of 148 pre-orthodontic patients were used. The angle of the condylar long axis and that of the molar rotation were both larger on the left side than on the right side. Positive correlations were found between the corresponding bilateral measurements of condylar long axes and also between those of molar …
