[Fungal and bacterial nail infections].

Fungal and bacterial infections are very common causes of nail deformity. The majority of fungal nail infections are caused by dermatophytes. Dermatophytosis result from a human contamination. A dermatophyte is always a pathogenic agent. Very effective drugs are available to treat dermatophyte nail infections. Yeasts of the genus Candida, notably C. albicans are the second most common cause of nail infection. The infections of nails due to Candida and bacteria are related with Candida sp and bacteria which are common commensals of the gastrointestinal tract, vagina or skin. Except C. albicans …

[Pigmented lesions of the nail apparatus].

Linear pigmentation of the nail apparatus most reflects longitudinal melanonychia. It results either from simple activation of matrical melanocytes induced by various factors or from benign (lentigo or naevus) or malignant (melanoma) melanocyte hyperplasia. Hematomas and some fungal infections can falsely resemble linear pigmentation. Non-linear pigmented lesions of the nail bed are most often subungual hematoma or infections (fungal, or pyocyanic superfection of a loosened or separated nail). Opening the nail bed directs the diagnosis. A possible tumoral cause should be kept in mind, especial

Nail involvement in pemphigus vulgaris.

Nail involvement in pemphigus vulgaris is relatively rare. We describe a case of severe pemphigus involving both the skin and oral mucosa in which an acute exacerbation was preceded by the onset of nail involvement of all 4 extremities. Nail involvement occurred in the form of hemorrhagic paronychia of multiple digits. Oral, cutaneous, and nail manifestations of the disease were all well controlled by systemic therapy. A review of the literature on nail involvement in pemphigus reveals that this involvement may be manifested in multiple ways, with chronic paronychia and onychomadesis being the

Treatment of common nail disorders.

Treatment of nail diseases is not always easy; topical treatment is often ineffective because the nail plate is not penetrated by topical drugs, and the nail matrix is located under the proximal nail fold. The most frequent nail disorders are nail psoriasis and onychomycosis. In these cases, systemic treatment is often necessary to cure the nail changes. Systemic treatment with steroids is mandatory for patients with nail lichen planus because the disease may cause definitive destruction of the nail matrix. Environmental nail abnormalities are frequent and include nail brittleness, onycholysis

[Nail manifestations of systemic diseases].

General diseases-associated nail disorders may be classified into 5 types according to their clinical impact. Type I refers to nail disorders considered as peculiar features among the various symptoms of general disease such as peri-ungueal vascular dilatation, onycholysis, Beau lines, opaque nails or melanonychia. Type II refers to nail disorders with a strong clinical value such as Terry's nails, Muerhrcke's lines or koilonychia. Type III refers to nails disorders presenting as significant syndromes of several diseases such as yellow-nail syndrome of subungueal splinter haemorrhages, and typ…

Sarcoidosis presenting as nail dystrophy.

A 45-year-old woman was referred to the dermatology clinic for assessment of "refractory onychogryphosis." She had a 3-year history of lesio … f significant nail changes including dystrophy, onycholysis, and hyperkeratosis (Fig. 1). The fingernails were normal. There were no other skin abnormalities. A punch biopsy of the tip of the third toe showed granulomatous inflammation. There was evidence of hyperkeratosis, exocytosis, and a dense infiltrate composed of collections of histiocytes and a …

Allergenic ingredients in nail polishes.

It has been known since the 1940s that nail polishes contain allergenic ingredients. The aim of this study was to clarify whether the nail polishes on the market today contain significant amounts of allergens, and what the solvents are. The following ingredients were determined: toluene, toluene sulfonamide formaldehyde resins, free formaldehyde, acrylates, methacrylates and certain organic solvents. The study comprised 20 brands and 42 samples. All the nail polishes analysed contained allergenic toluene sulfonamide formaldehyde resins (TSFR), in concentrations from 0.08 to 11.0%. The concentr

Arthroscopic advancement of a tibial nail.

Rapid restoration of maximal extremity function has stimulated the aggressive treatment of tibia fractures with intramedullary nails. Rod migration is a well-described complication of flexible or unlocked intramedullary rods. Interarticular extrusion of an unlocked tibial rod occurred in a patient after a second accident. Arthroscopy was used to evaluate the chondral injury, as well as to advance the rod.

Yellow nail syndrome in infancy.

This report describes an infant with clinical features consistent with the yellow nail syndrome (YNS), a rare autosomal dominant disorder. He presented at birth with congenital lymphoedema and was referred at 6 months of age for investigation of recurrent cough and wheeze. He had clinical and radiological evidence of bilateral pleural effusions and a pericardial effusion. Following a lung biopsy and pericardial window these were shown to be manifestations of his lymphatic abnormality. He also had persisting middle ear effusions causing conductive deafness requiring hearing aids and secondary i

Nail abnormalities in rheumatoid arthritis.

Many nail abnormalities have traditionally been described in association with rheumatoid arthritis (RA), but their specificity has never been assessed in a controlled study. Our purpose was to evaluate the frequency and the specificity of nail changes associated with RA in a case-controlled study including 50 patients suffering from RA and 50 controls. For each patient, a general skin examination was performed and the 20 nails were examined. The nail features were noted and classified. A chi 2 test or a Fisher test was used to compare the two groups. The only nail abnormalities significantly a…

There is something wrong with my nail.

Nail problems are a frequent source of concern to patients and are a relatively common presenting complaint to general practitioners. While tinea unguium and psoriasis are the main causes of nail problems for the majority of patients, there are numerous other causes of nail dystrophies. This article aims to discuss some of the commoner causes of nail dystrophy. In addition it will give GPs an understanding of the anatomy and function of the nail and provide an approach to diagnosis based on morphology. Treatment will be dealt with in a subsequent article.

[Intramedullary nailing in multiple trauma].

The indication and best time for intramedullary nailing of long bones in the multiple trauma patient is still controversial. While in the Anglo-American literature primary intramedullary nailing is regarded as the method of choice, differentiated management is preferred in German-speaking countries on account of reports about the pathogenesis of trauma shock. On the one hand, it has been shown that severe thoracic injury greatly influences the course after multiple trauma and, in particular, its combination with femoral fracture represents an increased risk. On the other hand, every surgical i

[Local complications of intramedullary nailing].

In the period from January 1990 to the end of December 1995 intramedullary nailing was performed in 294 patients with 156 fractures of the femur and 162 fractures of the tibia. Early nailing within the first 24 h after trauma was realized in 70% of the femur and in 64% of the tibia fractures. A synopsis focusing on local complications after intramedullary nailing is given on the basis of our results and a thorough analysis of the literature. Iatrogenic fractures of the femoral neck (0-5%), of the proximal femur (0.8-11.2%) and of the tibial head (0-8.3%) represent important intraoperative comp

Retrograde reamed femoral nailing.

Closed, reamed, antegrade nailing remains the standard of care for femoral shaft fractures. This technique however, may be less attractive in the management of femoral shaft fractures associated with (a) ipsilateral acetabular, pelvis, or femoral neck fractures; (b) polytrauma requiring multiple simultaneous surgical procedures; and (c) pregnancy. We now report on our experience with the retrograde femoral nailing as a treatment option in these situations. Between 4/88 and 10/90, 29 retrograde femoral nailing in 24 patients were attempted. Average age was 29.3 (16-74) years. Five fractures wer

Nail biopsy. Indications and methods.

Nail biopsy is a safe and useful diagnostic procedure for many nail disorders when routine clinical and laboratory methods fail to produce a diagnosis. Prerequisites for nail biopsy are an understanding of the surgical anatomy of the nail, adequate anesthesia and hemostasis, and a nail abnormality for which histopathology can provide the diagnosis. Indications and methods for nail biopsy vary according to the site and type of pathology in the nail unit. Nail bed and perionychial biopsies can be performed easily and with minimal scarring. They are most commonly used to diagnose tumors as well a
