Cervical insufficiency, amniotic fluid sludge, intra-amniotic infection, and maternal bacteremia: the need for a point-of-care test to assess inflammation and bacteria in amniotic fluid.

Acute cervical insufficiency is frequently associated with subclinical intra-amniotic inflammation and intra-amniotic infection. Amniotic fluid analysis has been recommended prior to the placement of a cervical cerclage given that preexisting infection is associated with adverse pregnancy outcome. We report a case for which commonly available laboratory tests-amniotic fluid Gram stain, white blood cell count, and glucose concentration-did not detect either intra-amniotic inflammation, diagnosed by elevated amniotic fluid interleukin-6, or intra-amniotic infection, diagnosed by cultivation. Assessing the Need for Preprocedural Laboratory Tests and Stopping Non-steroidal Anti-inflammatory Drugs/Aspirin in Patients Undergoing Percutaneous Bone and Soft Tissue Biopsies.

PURPOSE: Although image-guided biopsies of bone and soft tissue lesions have a low complication rate, there is limited data evaluating use of preprocedural laboratory tests. To address this issue, patients were not required to stop non-steroidal antiinflammatory drugs (NSAIDs) and aspirin or to obtain preprocedural laboratory tests [complete blood count (CBC) and international normalized ratio (INR)], except in special circumstances. The bleeding complication rate, rate of same day biopsies, and the time from when the biopsy was ordered to when it was performed were obtained. MATERIALS AND ME

Evaluation of a validated questionnaire to assess the need for prevention or rehabilitation by preventive health examinations: a cross-sectional study of German employees aged 45 to 59 years (Ü45-check).

OBJECTIVE: A longer life expectancy can lead to a longer working life and is therefore important for the individual and the healthcare system. Therefore, a study group established a Risk-Index Disability-Pension (RI-DP), which assesses the risk of work disability. However, a standardized and well-founded preventive health examination does not yet exist in Germany. Hence, we developed a preventive health examination conducted by physicians and compared it with a questionnaire survey to examine its differences and results in relation to the need for prevention and rehabilitation, taking into con

Do We Need Referral Centers for Benign Prostatic Hyperplasia that Offer Expertise in More Options? Yes, with a SMART (Specialized Medical Assessment for Right Treatment) Approach.

Referral centers for lower urinary tract symptoms due to benign prostatic hyperplasia (BPH) play a crucial role in providing personalized and effective solutions for patients. In specialized centers, urologists can effectively select the most appropriate tailored intervention that balances efficacy, morbidity, speed of onset, patient preferences, and disease progression, and can also manage challenging cases. This patient-centered approach should become standard practice for BPH management.

Do We Still Need to Assess Post-procedural Radial Artery Compression Time and Radial Artery Occlusion in Patients Who Undergo Transradial Coronary Intervention?

Background and objective Transradial access (TRA) for interventional coronary procedures has now been widely accepted as the preferred vascular site approach. The duration of post-procedure compression has been shown to be a crucial factor and different hemostatic devices used in this regard have been compared. In this study, we aimed to compare the post-procedure compression time, radial artery occlusion (RAO), hematoma, and bleeding between the transradial (TR) band and AIR band for radial artery patency among patients presenting at a tertiary care hospital. Methodology This observational st

Assessing the Need for Multiplex and Multifunctional Tick-Borne Disease Test in Routine Clinical Laboratory Samples from Lyme Disease and Febrile Patients with a History of a Tick Bite.

Human polymicrobial infections in tick-borne disease (TBD) patients is an emerging public health theme. However, the requirement for holistic TBD tests in routine clinical laboratories is ambiguous. TICKPLEX® PLUS is a holistic TBD test utilized herein to assess the need for multiplex and multifunctional diagnostic tools in a routine clinical laboratory. The study involved 150 specimens categorized into Lyme disease (LD)-positive (n = 48), LD-negative (n = 30), and febrile patients from whom borrelia serology was requested (n = 72, later "febrile patients") based on reference test results from

Detecting falsified oral contraceptives by visual assessment and diffuse reflectance spectroscopy (350-2500 nm): the need for supplementing traditional pharmacopeia techniques and the public health implications.

OBJECTIVES: Substandard and falsified pharmaceuticals can present a major health risk, particularly for low- and middle-income countries. In a Sub-Saharan African market, United States Agency for International Development (USAID) staff found an oral contraceptive product (0.15 mg levonorgestrel/0.03 mg ethinyl estradiol) labeled with a logo (and brand name) visually matching one historically used by USAID but purportedly manufactured by an unauthorized source. Additional assessment was conducted to determine if more evidence of falsification was present to better understand the public health i

Complementary therapies for reducing the risk of osteoporosis in patients receiving luteinizing hormone-releasing hormone treatment/orchiectomy for prostate cancer: a review and assessment of the need for more research.

Osteoporosis in women has received a substantial amount of attention, but its impact in men is also significant and noteworthy. Those men who benefit from treatment for prostate cancer with androgen deprivation therapy (ADT) may also be at a higher risk for osteoporosis. Pharmacologic approaches to reduce this risk have received some attention. For example, agents such as bisphosphonates, estrogen receptor-binding drugs (diethylstilbestrol, tamoxifen, and raloxifene), calcitonin, and fluoride are some of the more promising interventions that have been previously outlined. In addition, statin d

What Should Be Assessed Next After a Positive Screening Test for the Long-Term Care Needs?: A Comparative Analysis of the Clinical Frailty Scale, the Kihon Checklist, and the Dementia Assessment Sheet for Community-Based Integrated Care System-8 Items.

AIM: The Kihon Checklist (KCL), Clinical Frailty Scale (CFS), and Dementia Assessment Sheet for Community-Based Integrated Care System-8 items (DASC-8) predict incident long-term care needs (LTCN) certification. We aimed to identify additional factors associated with incident LTCN certification among older adults who screened positive on any tool. METHODS: In this retrospective observational cohort study, we included patients aged ≥ 65 years attending a frailty outpatient clinic and had no LTCN certification at baseline. Positive screening was defined as CFS, KCL, and DASC-8 scores ≥ 4, 8, an

Assessment of needs, challenges, and re-design considerations for culturally sensitive provision and delivery of palliative care supports and services for older adults who prefer to age and die in place in diverse India.

As the global older adult population continues to grow, challenges related to managing multiple chronic conditions (MCCs) or multimorbidity underscore the growing need for palliative care. Palliative care preferences and needs vary significantly based on context, location, and culture. As a result, there is a need for more clarity on what constitutes palliative care in diverse settings. Our objective was to present an international perspective on palliative care in India, a culturally diverse and large ancient Eastern middle-income country. In this narrative review article, we considered three
