Risk factors for early sepsis in very low birth weight neonates with respiratory distress syndrome.

AIM: To identify maternal and neonatal factors that increase suspicion of early sepsis in Very Low Birth Weight neonates with respiratory distress syndrome. METHODS: The cohort included 282 neonates born at Soroka Medical Centre 1996-2000. Definitions of 'high' and 'low'-suspicion groups for early sepsis were based on comparison between neonates with early sepsis and the remaining cohort. Univariate analysis and logistic regression were used to compare between groups. RESULTS: The incidence of early sepsis in the cohort was 1.8%, and 94% received antibiotics following delivery. Comparing wit

c-Jun kinase is a critical signaling molecule in a neonatal model of group B streptococcal sepsis.

Group B streptococcus (GBS) is the major cause of sepsis in newborn infants. In vitro, inactivated GBS stimulates macrophages to produce inflammatory proteins via the TLR adapter protein MyD88. Furthermore, inflammatory cytokine release in response to GBS greatly exceeds that following stimulation with pneumococci. In this study, we attempted to unravel signaling events that are involved in GBS-, but not Streptococcus pneumoniae-stimulated phagocytes to identify molecular targets for adjunctive sepsis therapy. We found that inactivated GBS and S. pneumoniae differed in the activation of the MA

Use of once-daily netilmicin to treat infants with suspected sepsis in a neonatal intensive care unit.

Once-daily administration of aminoglycoside antibiotics has become the most acceptable dosing schedule for the majority of patients. There are few published data on the impact of post-natal age on aminoglycoside concentrations in preterm infants receiving once-daily dosage regimens. Netilmicin was administered as a once-daily dose of 4 mg/kg. In 141 episodes of suspected sepsis in 123 babies, trough netilmicin concentrations ranged from undetectable to 4.0 mg/l. Netilmicin concentrations were above a level of 2 mg/l in 10.6% of episodes. Netilmicin concentrations decreased with increasing post

Early-onset neonatal sepsis in Pakistan: a case control study of risk factors in a birth cohort.

We prospectively evaluated risk factors for early-onset neonatal (EON) sepsis in a case-control study among inborn patients at the Aga Khan University Medical Centre in Karachi between 1990-1993. A total of 38 cases with blood culture proven bacterial sepsis were identified within 72 hr of birth (prevalence 5.6 of 1000 live births) and matched with two consecutive gender matched births with no complications. The most common isolates were Staphylococcus aureus (18%), group B Streptococci (13%), and Klebsiella pneumoniae (13%). Univariate analysis of maternal risk factors revealed a significant

Diagnosis and treatment of right Bochdalek hernia associated with group B streptococcal pneumonia and sepsis in the neonate.

Neonatal sepsis due to group B beta-hemolytic Streptococcus (GBS) is reported to occur in about 1 out of 330 live births. Right-sided Bochdalek hernia (RBH) occurs in about 1 of 20,000 live births. The combination of group B streptococcal sepsis and delayed appearance of a right Bochdalek hernia is an infrequently reported phenomenon--18 patients have been previously reported in the English literature. We add four patients from our own experience to these previous reports. Since approximately 10% to 15% of the newborn population are exposed to group B Streptococcus we suspect that the inadequa…

Exosome-facilitated nanoplatform for enhanced antibiotic delivery to eradicate intracellular multidrug-resistant Escherichia coli in neonatal sepsis.

Neonatal sepsis is a pathogenic infection with an estimated global fatality rate of 18% and poses a greater threat in preterm infants, where early-onset cases often result from vertical transmission from the maternal genital tract. Conventional high-dose antibiotic therapy may lead to adverse effects and reduced therapeutic efficacy due to suboptimal intracellular delivery and interactions with extracellular components before effective cellular uptake. To overcome this limitation, exosome-based nanocarriers deliver antibiotics directly into cells, preventing systemic dissociation in the blood.…

Evaluation of modified hematological scoring system and platelet indices in diagnosing neonatal sepsis-A cross-sectional study.

The gold standard for identifying neonatal sepsis is blood culture. However, there are a few drawbacks: Getting results takes time and requires a complex procedure; also, it frequently yields false-positive or false-negative findings. Platelet indices and the modified hematological scoring system (mHSS) aid in the precise diagnosis of newborn sepsis. This study was conducted to assess the diagnostic value of platelet indices and mHSS in the diagnosis of neonatal sepsis separately and in combination. This cross-sectional study was performed on 104 neonates having clinical suspicion of sepsis. V…

Predictive Value of C-Reactive Protein/Albumin Ratio for Necrotizing Enterocolitis in Preterm Infants With Neonatal Sepsis.

BACKGROUND: Necrotizing enterocolitis (NEC), a critical neonatal digestive disease, is recognized as a contributing factor to neonatal sepsis. However, the role of the C-reactive protein/albumin ratio (CAR) in diagnosing neonatal sepsis within the context of NEC remains under-researched. AIM: Our objective was to evaluate the clinical utility of the C-reactive protein/albumin (CRP/ALB) ratio in predicting neonatal sepsis at the NEC diagnosis stage. STUDY DESIGN: In this retrospective analysis, we evaluated 148 NEC cases, utilizing data extracted from electronic medical records. The relations…

A rapid time-resolved host gene expression signature predicts responses to antibiotic treatment in neonatal bacterial sepsis.

Sepsis is a leading cause of mortality and morbidity in neonates yet remains difficult to diagnose. This leads to widespread empiric antibio … ansient elevation of antimicrobial defensive genes, suggesting an impaired bactericidal response in neonatal sepsis. These findings suggest that the host response is regulated in sepsis and offer insights into early prognostic approaches for reducing antibiotic overuse.

Interleukin-27-producing cells in gram-negative neonatal sepsis display diverse phenotypes and functions in the liver.

Neonates have increased vulnerability to life-threatening infections due to the distinct immune landscape. Interleukin (IL)-27 is a key component of this immune profile that we have previously shown to be elevated in both newborn humans and mice. IL-27 continues to increase in the serum and tissues consistent with poor outcomes during gram-negative neonatal bacterial sepsis. Presently, we dissected the IL-27 producer profile at a single-cell level using IL-27p28eGFP reporter mice in our previously established model of neonatal sepsis with luciferase-expressing K1-encapsulated Escherichia coli.…

The Novel MFG-E8-derived Oligopeptide, MOP3, Improves Outcomes in a Preclinical Murine Model of Neonatal Sepsis.

INTRODUCTION: Neonatal sepsis is a devastating inflammatory condition that remains a leading cause of morbidity and mortality. Milk fat globule-EGF-factor VIII (MFG-E8) is a glycoprotein that reduces inflammation, whereas extracellular cold-inducible RNA binding protein (eCIRP) worsens inflammation. This study aimed to determine the therapeutic potential of a novel MFG-E8-derived oligopeptide 3 (MOP3) designed to clear eCIRP and protect against inflammation, organ injury, and mortality in neonatal sepsis. METHODS: C57BL6 mouse pups were injected intraperitoneally with cecal slurry (CS) and tr…

Nucleotide receptors mediate protection against neonatal sepsis and meningitis caused by alpha-hemolysin expressing Escherichia coli K1.

Neonatal meningitis-associated Escherichia coli (NMEC) is among the leading causes of bacterial meningitis and sepsis in newborn infants. Several virulence factors have been identified as common among NMEC, and have been shown to play an important role in the development of bacteremia and/or meningitis. However, there is significant variability in virulence factor expression between NMEC isolates, and relatively little research has been done to assess the impact of variable virulence factor expression on immune cell activation and the outcome of infection. Here, we investigated the role of NME
