We report, on two, school-age girls with clinical and electroencephalographic features of early onset childhood epilepsy with occipital paroxysms (CEOP) of the "Panayiotopoulos type" that showed atypical evolution. Neurological examination and brain imaging were normal in both. One child presented at age 2.5 years episodes of oculocephalic deviation, and ictal vomiting during nocturnal sleep. The EEG showed left occipital spikes during wakefulness and sleep. One year later, frequent inhibitory seizures appeared in the lower limbs causing, "pseudoataxic gait". At the same time she presented wit

BACKGROUND/AIMS: Nitric oxide (NO) has recently been shown to be a neurotransmitter in non-adrenergic non-cholinergic (NANC) inhibitory nerves in the gastrointestinal tract. To clarify the role of NO in the human pyloric sphincter, enteric nerve responses in pyloric tissue specimens obtained from patients with gastric cancer were investigated.
METHODOLOGY: Fresh specimens of normal pylorus obtained from 18 patients with gastric cancer were used. The subjects consisted of 12 men and 6 women, aged from 45-74 years (average: 60.1 years). A mechanograph was used to evaluate in vitro pyloric sphin

The brainstem is said to be the generator of pathological startle responses due to reticular reflex myoclonus or hyperekplexia. A patient with facial weakness, nystagmus, and pyramidal tract signs had generalised reflex spasms in response to auditory, visual and tactile stimuli which clinically and neurophysiologically resembled hyperekplexia. The case is unusual because as well as hyperekplexia, the patient's initial presentation was with an equally rare manifestation of brainstem pathology-brainstem mediated trismus. The causes of brainstem trismus and exaggerated startle responses are discu

It has been observed that knee extensor muscles cannot be fully activated during voluntary contractions following knee injuries. This muscle inhibition has an unknown origin and appears to hinder full rehabilitation of the affected joint. We have investigated muscle inhibition during and following knee injuries in non-athletic and athletic patients and compared their results to non-athletic, unaffected volunteer subjects. There appears to be a small amount of muscle inhibition in the knee extensors of normal subjects; this inhibition increases dramatically following knee injury, and appears to

In this study, we have used paired transcranial stimulation of the motor cortex to test the hypothesis that cortical inhibition is decreased in juvenile myoclonic epilepsy (JME). The double shock technique was adopted here because it offers a means for highlighting abnormal inhibitory mechanisms. From previous experiments performed on healthy subjects, it is known that a magnetic conditioning stimulus, of subthreshold intensity, suppresses the MEP in response to a subsequent suprathreshold stimulus delivered after 1-4 msec. JME patients were selected as a potential contrast with other forms of

1. We recently described a capsaicin-sensitive vagal pathway mediating non-adrenergic, non-cholinergic (NANC) relaxations of an isolated, innervated rostral guinea-pig tracheal preparation. These afferent fibres are carried by the superior laryngeal nerves and relaxations elicited by their activation are insensitive to autonomic ganglion blockers such as hexamethonium. In the present study this vagal relaxant pathway was further characterized. 2. Relaxations of the trachealis elicited by electrical stimulation of capsaicin-sensitive vagal afferents were mimicked by bath application of capsaici

Both rapid eye movement sleep and cataplexy in the narcoleptic canine have been shown to increase after both systemic and local administration of cholinergic agonists in the pontine reticular formation. Furthermore, binding studies indicate an increase in the number of M2 muscarinic receptors in the pontine reticular formation of narcoleptic canines. In the present study we have investigated the receptor subtypes involved in mediating the cholinergic stimulation of cataplexy, as defined by brief periods of hypotonia induced by emotions, within the pontine reticular formation of narcoleptic can

We determined whether electrical stimulation of the abdominal vagus nerve causes secretion of vasopressin in the rabbit and whether inhibition of neuronal function in the A1 region of the medulla oblongata impairs this secretion. In urethan-anesthetized rabbits, electrical stimulation of the abdominal vagus (5-min train of cathodal pulses, 0.5 ms duration, 20 Hz, 0.5-1 mA) increased plasma vasopressin from 37 +/- 8 to 133 +/- 19 pg/ml (P < 0.01, n = 11). Prior section of the cervical vagus completely prevented the increase seen with stimulation of the abdominal vagus. Injecting the inhibitory

Thalamocortical oscillations mediate both physiological and pathophysiological behaviors including sleep and generalized absence epilepsy (GA). Reciprocal intrathalamic circuitry and robust burst firing, dependent on underlying transient Ca current (IT) in thalamic neurons, support generation of such rhythms. In order to study the regulation of intrathalamic rhythm generation and the effects of GA anticonvulsants previously shown to reduce IT in acutely isolated thalamic neurons, we developed an in vitro rat thalamic slice preparation that retains sufficient intrathalamic circuitry to support

We measured the effect of vasopressin (8 mU X kg-1 X min-1) on reflex inhibition of renal sympathetic nerve activity induced by volume expansion in 13 sinoaortic-denervated anesthetized rabbits. Volume expansion increased left ventricular end-diastolic pressure (LVEDP) from 5.1 +/- 0.7 to 14.1 +/- 1.4 mmHg and decreased renal nerve activity (RNA) from 57.4 +/- 6.9 to 30.2 +/- 5.6 impulses/s. Infusion of vasopressin elevated LVEDP from 6.0 +/- 1.0 to 7.3 +/- 1.1 mmHg and decreased RNA from 61.8 +/- 7.2 to 47.1 +/- 6.3 impulses/s. Heart rate fell from 243 +/- 7 to 231 +/- 9 beats/min; no other s

The reflex responses evoked by direct electrical stimulation of the intracranial portion of the trigeminal nerve have been studied in 16 subjects undergoing percutaneous retrogasserian thermocoagulation for the treatment of trigeminal neuralgia affecting the second or third division. In the obicularis oculi muscle, early and late responses similar to the R1 and R2 components of the blink reflex were recorded. The former could be evoked only by stimulation of the second division and its latency was consistent with intermediately fast afferents. A late reflex (50-70 ms) was occasionally recorded

The hypothesis that acutely administered ethanol could interfere with neocortical recurrent inhibition (RI) was supported. The large surface negative wave in response to antidromic stimulation of the cerebral peduncle represents a summation of inhibitory postsynaptic potentials, a measure of RI. In acute experiments on adult rats, blood alcohol levels of less than about 120 mg/100 ml slightly facilitated the surface negative wave. Higher blood alcohol levels always blocked the surface negative response. Stimulation of the somatosensory thalamic relay nuclei produced a cortical response on whic

Partial complex seizures are known to arise from abnormal firing of neurons in cortex that has histologic abnormalities associated with tumors, infarcts, or neuron loss. The latter pathology of sclerosis is most frequently found in the hippocampus, and partial seizures from this region are focalized by direct electrical recordings and treated by anterior temporal lobectomy. Although we can link this hippocampal sclerosis to nearby hyperexcitability, the synaptic mechanisms involved in hippocampal seizure genesis are not yet known. We have used in vivo microelectrode recordings from hippocampal

The effects of propranolol and of the selective beta 1- and beta 2-adrenoreceptor blocking drugs atenolol and ICI 118,551 were determined on the inhibitory responses of isolated segments of rabbit ileum to noradrenaline, isoprenaline and salbutamol and to periarterial sympathetic nerve stimulation. Responses to isoprenaline (0.04-10.24 microM) and salbutamol (1.4-89.6 microM) were blocked by propranolol in concentrations up to 5.0 and 12.8 microM, respectively. Responses to sympathetic nerve stimulation were reduced but responses to noradrenaline (0.03-1.92 microM) were unaffected by propranol

Fetal alcohol syndrome (FAS) is the most prevalent known preventable health hazard to the human fetus by a noxious agent. It is associated with impairments of the central nervous system that are expressed in the forms of mental retardation of varying severity, learning disabilities, attentional deficits and an increased vulnerability to stress. Results of psychophysiological studies of the effects of ethanol on the central nervous system are reviewed, with the aim of exploring how conclusions derived from them can serve as testable hypotheses in FAS research. The experimental methods used in s

The responses of 302 neurons in the medial medullary reticular formation (MRF) to a variety of noxious and innocuous somatic stimuli were studied in anesthetized and awake rats. In addition, the effects of analgesic electrical stimulation in the mesencephalon (MES) on unit responses were examined. Tail shock was the most effective stimulus, exciting more than 80% of all units recorded. This stimulus was considered separately during data analysis, since it could not be classified as noxious or innocuous. Noxious somatic stimuli (including pinch, firm pressure, pin prick, and radiant heating of

Unitary discharges in the efferent ciliary nerves were studied in cervical sympathectomized rabbits. In all, 279 units were recorded and categorized, except 7 units, into the following 3 types from their responses to photic and other stimuli, i.e. type A (170 units), type B (42 units) and type C (60 units). Type A units were characterized by exhibiting excitatory response to ipsilateral eye illumination and the response was closely related to the luminance level. The type A was further classified into AI (excitation), AII (no change) and AIII (inhibition) on the basis of the response to contra

1. Intracarotid (I.C.) injection of either acetylcholine (ACh) or dopamine inhibited spontaneous chemosensory activity recorded from the peripheral cut end of the sinus nerve in the anaesthetized rabbit. 2. High doses of ACh (greater than or equal to 50 micrograms I.C.) evoked a slight increase in discharge which preceded the inhibition. This excitation was attributable to a nicotinic action of the drug since it was abolished by mecamylamine. 3. The muscarinic agonist bethanechol inhibited chemoreceptor activity, an effect which was blocked by high doses of atropine, as was the inhibition caus

The influence of the cerebellum on activity of the septal region, hippocampus, and amygdala of cats and rats was determined by obtaining unit recordings from those supratentorial sites with cerebellar stimulation. With stimulation of the postral vermis, fastigial nucleus, and intervening midline folia of the cerebellum, units in the septal region were facilitated, whereas those in the hippocampus were inhibited. Mixed results were obtained in the amygdala, some units being facilitated and others inhibited. Stimulation over the lateral cerebellar hemispheres and dentate nucleus, on the other ha
