BACKGROUND: Narcolepsy is a chronic hypersomnia of central origin, linked with dysfunction of hypocretin-containing neurons, localized in the lateral hypothalamus. Main symptoms of narcolepsy include excessive daytime sleepiness, cataplexy, hypnagogic hallucinations and sleep paralysis. Establishing the proper diagnosis is very important, because of the negative impact of narcolepsy on patients' functioning in social-life and the possibility of improvement of quality of life with adequate treatment.
AIM OF THE STUDY: Present review describes clinical characteristics of narcolepsy, usefulness

Even though pendular seesaw nystagmus (PSN) or periodic alternating nystagmus (PAN) has been reported in blind persons, the co-occurrence of PSN and PAN has not been described. A 69-year-old woman with progressive visual loss to only light perception in both eyes showed PSN with a conjugate torsional component and the vertical components in the opposite directions in both eyes. In complete darkness, she also exhibited PAN in addition to PSN. PSN remained unchanged in darkness. The rest of the neurological examination, testing for mitochondrial mutations of Leber's hereditary optic neuropathy,

Understanding three peripheral vestibular disorders--vestibular neuritis, benign paroxysmal positional vertigo, and Meniere's disease--is the key to the evaluation and management of vertigo and dizziness presentations in the emergency department. Each of these benign disorders is a common cause of a broad category of dizziness presentation. In addition, each of these disorders has characteristic features that allow for a bedside diagnosis. An effective strategy for "ruling-out" a serious disorder, such as stroke, is "ruling-in" a peripheral vestibular disorder. In this article a focus is on th

Dementia is a subject of interest for both neurologists and psychiatrists. The most common causes of dementia are neurodegenerative diseases of the CNS. Alzheimer's disease is the most frequent of them, much less common are Lewy's body disease, Pick's disease, Parkinson's disease or Huntington's disease. Huntington's disease is an autosomally dominant terminal illness, that occurs in approximately 5 - 7 persons in 100 000. In 90% of the cases it begins after the age of 35, the remaining 10% is the juvenile and early form, which varies from that seen in adults. Rigidity, oral motor dysfunction,

Physical therapists commonly use screening tests to identify upper motoneuron lesions such as cord compressive myelopathy (CCM), the presence of which necessitates appropriate medical referral. Signs and symptoms of CCM include sensory and ataxic changes of the lower extremities, poorly coordinated gait, weakness, tetraspasticity, clumsiness, spasticity, hyperreflexia, and primitive reflexes. Clinical tests and measures such as Hoffmann sign, clonus, Lhermitte sign, the grip and release test, the finger escape sign, the Babinski test, and the inverted supinator sign have historically been used

OBJECTIVE: To study characteristics of peripheral neuropathy in a group of Thai HIV-infected patients by clinical and electrophysiologic evaluation.
MATERIAL AND METHOD: Patients with HIV infection from HIV and neurology clinic were recruited during June and October 2005. Neurological examination and nerve conduction study were done in all patients to establish the presence and pattern of peripheral neuropathy. Clinical data were compared between the groups with and without HIV-related neuropathy.
RESULTS: Forty-eight HIV-infected patients were recruited but complete data were obtained in 34

Among movement disorders, dystonia is a particularly complex phenomenon and difficult to describe. For this reason, cinematographic documents were particularly important to the establishment of this disorder within the neurological nosology. The seminal 1944 article on dystonia by E. Herz anchored its arguments in moving film documentation, published with frame-by-frame demonstrations of dystonic patients. Although the original films that comprised the basis of this article have not been located, two related contemporaneous films, one by Herz in association with T.J. Putnam, and one by S.P. Go

Rod photoreceptor degeneration causes deterioration in the morphology and physiology of cone photoreceptors along with changes in retinal circuits. These changes could diminish visual signaling at cone-mediated light levels, thereby limiting the efficacy of treatments such as gene therapy for rescuing normal, cone-mediated vision. However, the impact of progressive rod death on cone-mediated signaling remains unclear. To investigate the fidelity of retinal ganglion cell (RGC) signaling throughout disease progression, we used a mouse model of rod degeneration (Cngb1neo/neo). Despite clear deter

This study was done to assess the percentage of abnormality in additional nerve conduction techniques after normal median distal latency (routine) in mild carpal tunnel syndrome (CTS). Bilateral nerve conduction studies were carried out in 116 consecutive symptomatic CTS patients (153 hands). Mild cases were based on normal routine (< 3.7 ms, peak-measured, 14 cm) and at least one technique abnormal of the following: sensory median-radial difference (MR); sensory median-ulnar difference (MU4); mixed palm median-ulnar difference (MUP); median palm latency (PW); and motor median distal latency (

With the aim of analyzing which complementary tests are relevant in the diagnostic evaluation of individuals with pervasive developmental disorders, a protocol of clinical and laboratory evaluation was applied in 103 outpatients. The protocol included chromosomal analysis, screening for inborn errors of metabolism, cytogenetic and molecular study of the FRAXA, FRAXE, and FRAXF mutations, EEG, SPECT, and magnetic resonance imaging study. Eighty-four subjects concluded the complementary tests and were classified either as having autism, atypical autism or Asperger syndrome according to the DSM-I

We present a case of isolated abducens nerve palsy with negative magnetic resonance imaging (MRI) findings and unusual cerebrospinal fluid results. The patient was treated for a central nervous system infection initially. After the second cerebrospinal fluid examination, a diagnosis of central nervous system leukemia was made. Isolated abducens nerve palsy is not always a stable or benign illness, even with a normal brain MRI study. Abnormal cerebrospinal fluid results should suggest the presence of a central nervous system malignancy.

Ischemia, hemorrhages, and other vascular disorders can result in various central or peripheral vestibular syndromes with vertigo, oculomotor/balance disturbances, and nausea. The vascular vertigo syndromes listed in Table 1 can however be brought about by other causes such as demyelitizing focuses in multiple sclerosis or space-occupying lesions, so that not only localization of the damaged structure but also the various etiologies are decisive for the choice of therapy. Occasionally, combined functional disturbances of the peripheral and central vestibular system appear, such as an infarction

Congenital hydrocephalus results from a variety of causes, some of the most common include spina bifida (myelomeningocele), aqueductal stenosis, and Dandy-Walker malformation. In addition, a number of cases result from genetic causes, other malformations, postinfectious, or neoplastic conditions. Outcome varies with cause but can be favorable. Most cases still are managed with shunting, although endoscopic modalities also can be considered.

Amyotrophic lateral sclerosis and other motor neuron disorders : official publication of the World Federation of Neurology, Research Group on Motor Neuron Diseases2000eng
The 2nd Consensus Conference (Versailles) recommended that an ALS knowledge-base for initial healthcare providers, diagnosing neurologists and confirming neurologists should be defined to include a simplified version of diagnostic criteria less formal than the World Federation of Neurology El Escorial Revisted Criteria ('ALS diagnosis - An algorithm'), a set of rules concerning red flags which should increase the suspicion of ALS as the diagnosis and minimize the time between suspicion and referral for confirmation of diagnosis ('ALS axioms of referral'), as well as a site of symptom onset-spe
