Risk factors for spinal cord injury during surgery for spinal deformity.

BACKGROUND: Spinal cord monitoring is now considered standard care during surgery for spinal deformity. Combined somatosensory and motor evoked potential monitoring allows the detection of early spinal cord dysfunction in most patients. The purpose of the current study was to identify clinical factors that increase the risk of intraoperative electrophysical changes and to provide management recommendations. METHODS: The records of 162 consecutive patients who underwent surgery for the treatment of spinal deformity at a tertiary referral center were reviewed. Electrophysical monitoring of thes

Maternal-Fetal Administration of Risdiplam Partially Rescues the SMNΔ7 Mouse Model of Spinal Muscular Atrophy.

OBJECTIVE: Spinal muscular atrophy (SMA) is caused by deletions or mutations in the survival motor neuron 1 (SMN1) gene and subsequent reduction in the expression of survival motor neuron (SMN) protein. The disease is characterized by degeneration of α motor neurons and subsequent muscle atrophy. Postnatal disease modifying therapies improve outcomes yet fall short of a cure. We have investigated a prenatal therapeutic strategy using the SMNΔ7 mouse model of SMA. METHODS: A risdiplam-like compound was administrated by oral gavage to pregnant mice with treatment initiated mid-gestation and hal

Assessing Cognitive Workload in Motor Decision-Making through Functional Connectivity Analysis: Towards Early Detection and Monitoring of Neurodegenerative Diseases.

Neurodegenerative diseases (NDs), such as Alzheimer's, Parkinson's, amyotrophic lateral sclerosis, and frontotemporal dementia, among others, are increasingly prevalent in the global population. The clinical diagnosis of these NDs is based on the detection and characterization of motor and non-motor symptoms. However, when these diagnoses are made, the subjects are often in advanced stages where neuromuscular alterations are frequently irreversible. In this context, we propose a methodology to evaluate the cognitive workload (CWL) of motor tasks involving decision-making processes. CWL is a co

Hypokalemic paralysis following intramuscular betamethasone injection: A case report and review of literature.

Acute neuromuscular paralysis is a relatively common condition in emergency rooms (ERs). They can be caused by several reasons, including adverse drug reactions. Betamethasone is a glucocorticoid commonly used for various conditions, such as allergic conditions. One of the rare but known side effects of glucocorticoids is hypokalemia. Rare cases of hypokalemia following high- and low-dose glucocorticoid injections have been reported. This study presents the history of a young, healthy male without significant past medical history who presented with an inability to stand and walk due to four-li

Lambert-Eaton Myasthenic Syndrome, Botulism, and Immune Checkpoint Inhibitor-Related Myasthenia Gravis.

PURPOSE OF REVIEW: This article reviews the pathophysiology, epidemiology, clinical presentation, diagnosis, and treatment of Lambert-Eaton myasthenic syndrome (LEMS) and of botulism, and immune-related myasthenia gravis (MG) occurring in the context of immune checkpoint inhibitor therapy for cancer. RECENT FINDINGS: The suspicion that LEMS is rare but also likely underdiagnosed is supported by recent epidemiologic data. A validated, LEMS-specific scale now exists to assess and monitor disease, and symptomatic and immunomodulatory treatments are available. As presynaptic disorders of neuromus

Functional assessment tools in children with Pompe disease: A pilot comparative study to identify suitable outcome measures for the standard of care.

BACKGROUND: Pompe disease (PD) is a rare condition caused by mutations in gene encoding for the enzyme alpha-glucosidase, resulting in an abnormal intracellular accumulation of glycogen. The disease clinical spectrum ranges from severe infantile forms to adult-onset forms with minor limitations. Since 2000 enzyme replacement therapy (ERT) is available and disease natural history has changed, with prolonged survival and evidence of myopathic features. METHODS: In this study, we monitored disease progression up to three years in eight young patients with PD. Based on the literature data and the

Increased levels of UCHL1 are a compensatory response to disrupted ubiquitin homeostasis in spinal muscular atrophy and do not represent a viable therapeutic target.

AIM: Levels of ubiquitin carboxyl-terminal hydrolase L1 (UCHL1) are robustly increased in spinal muscular atrophy (SMA) patient fibroblasts and mouse models. We therefore wanted to establish whether changes in UCHL1 contribute directly to disease pathogenesis, and to assess whether pharmacological inhibition of UCHL1 represents a viable therapeutic option for SMA. METHODS: SMA mice and control littermates received a pharmacological UCHL1 inhibitor (LDN-57444) or DMSO vehicle. Survival and weight were monitored daily, a righting test of motor performance was performed, and motor neurone loss,

Comparing the local dynamic stability of trunk movements between varsity athletes with and without non-specific low back pain.

The local dynamic stability of trunk movements, quantified using the maximum Lyapunov exponent (λmax), can provide important information on the neuromuscular control of spine stability during movement tasks. Although previous research has displayed the promise of this technique, all studies were completed with healthy participants. Therefore the goal of this study was to compare the dynamic stability of spine kinematics and trunk muscle activations, as well as antagonistic muscle co-contraction, between athletes with and without low back pain (LBP). Twenty interuniversity varsity athletes (10

Evoked potential testing.

Electrophysiologic tests of the sacral neuromuscular system and its suprasegmental control may be divided into EMG and methods involving stimulation (i.e., evoked potential and sacral reflex testing). The latter group of methods tests the function of defined parts of the motor or sensory nervous system, or reflex arcs. There already is ample experience with testing the somatic sensory pathways (pudendal SEP) and the (somatic) sacral reflex arc, whereas other methods (testing the motor system and tests involving visceral afferents and sympathetic efferents) need further study to establish their

Rocuronium (Org 9426) for caesarean section.

This was a prospective, non-randomized, multicentre study of rocuronium (Org 9426) in 40 elective Caesarean section patients at full term without fetal distress. Anaesthesia was induced with thiopentone 4-6 mg kg-1 i.v. and rocuronium 0.6 mg kg-1 and maintained with isoflurane and nitrous oxide in oxygen. Monitors included ECG, arterial pressure, pulse oximeter and train-of-four (TOF) produced by ulnar nerve stimulation. In all patients, full neuromuscular block at the hand indicating the maximum effect of rocuronium (T1 = 0) occurred at a mean time of 98.1 (SE 9.4) s. However, after 79.3 (2.9

Restoring brain wiring, mobility, sleep, and lifespan with a novel repurposed modulator of voltage-gated K+ channels: an emerging perspective for channelopathies.

BACKGROUND: Voltage-gated K+ channels (Kv) regulate various essential brain functions. Cognitive impairment, intellectual disability, and sleep disruption are prevalent in Kv1.1- and Kv1.2-related channelopathies, yet the mechanistic basis is unresolved and no symptom-specific repurposing candidates for pharmacotherapy have been identified. Furthermore, the role Kv channels play in establishing synaptic wiring—the intricate network of neuron-to-neuron connections that underpins communication within the brain that is fundamental to cognitive functions—remains poorly understood at ultrastructura

Measurement of temporal and spatial parameters of ice hockey skating using a wearable system.

Ice hockey is a dynamic and competitive sport that requires a high level of neuromuscular and cardiovascular function. An objective assessment of skating helps coaches monitor athletes' performance during training sessions and matches. This study aimed to estimate the temporal and spatial parameters of skating by proposing an optimized configuration of wearable inertial measurement units (IMUs) and validating the system compared to in-lab reference systems. Ten participants were recruited to skate on a 14 m synthetic ice surface built in a motion-capture lab. Eight original event detection met

Functional testing.

Functional testing is useful to monitor the natural history of several neuromuscular disorders, and to measure the efficacy of therapeutic agents in clinical trials. A major limitation of functional testing is that a single test is often not appropriate throughout all stages of disease. The same limitation holds true for other measurements of disease progression. MMT and quantitative myometry become technically difficult to perform and lack sensitivity to disease progression at some stages in the course of DMD, ALS, and spinal muscular atrophy. Other limitations of functional testing are its l

Surveillance of Acute Flaccid Paralysis (AFP) in Greece: 2008-2024.

As part of the WHO's Global Polio Eradication Initiative, acute flaccid paralysis (AFP) surveillance in children under 15 years old is crucial for monitoring the emergence of polioviruses and tracking Non-Polio Enteroviruses (NPEVs). This study outlines the past 17 years of AFP surveillance in Greece from 2008 to 2024, during which a total of 256 AFP cases were recorded. Stool samples from these cases were analyzed using virus isolation in cell cultures (RD/L20B) and sequencing of NPEV-positive samples. The Attica region reported the highest number of cases with 81 (31%), followed by Central M

Acute hypomagnesaemia causing intra-operative lower limb movements in a paraplegic patient, despite full neuromuscular blockade.

Alterations in electrolyte homeostasis associated with major surgery, intravenous fluid therapy, malnutrition, and critical illness are common but usually non-specific. This case report describes the sudden onset of forceful rhythmic flexion movements in both lower limbs during scoliosis surgery in a 12-year-old paraplegic boy with spina bifida. These movements arose despite adequate depth of anaesthesia, complete neuromuscular blockade, and treatment of hypocalcaemia. They persisted post-operatively after reversal of neuromuscular blockade and emergence from anaesthesia. Hypomagnesaemia was i

Endobronchial blocker dislodgement leading to pulseless electrical activity.

This is a report of a case in which an endobronchial blocker was dislodged, leading to severe air trapping and a brief episode of pulseless electrical activity. Bronchial blockade for lung deflation was successfully instituted during emergency repair of a ruptured descending aortic aneurysm. During a period not involving manipulation of aortic cross-clamps, end-tidal CO2 decreased precipitously to zero and airway pressures increased markedly, followed by equalization of intracardiac pressures. Prompt deflation of the endobronchial blocker balloon reversed the problem. We hypothesize that when

Awareness under general anesthesia.

General anesthesia aims to eliminate patients' awareness of excruciating pain during surgery. Nevertheless, rare occurrences of patient awareness continue because the problem is not yet completely preventable. One study puts the incidence of awareness at 0.18% for patients receiving muscle relaxants and at 0.10% for patients not given relaxant drugs. Awareness experiences frighten patients and impact their implicit and explicit memories in ways that can leave a lifetime of residual emotional and psychological problems ranging from sleep disturbances, nightmares, and daytime anxiety that may su
