[Atypical lentiginous nevus: a clinical and histopathologic study of 14 cases].

BACKGROUND: Atypical lentiginous nevus (of the elderly) is a peculiar form of dysplastic nevus. Clinically, this condition can resemble malignant melanoma and histologically, it has a lentiginous pattern with variable degrees of atypia and an absence of dermal nests. These features may lead to an erroneous diagnosis of lentigo maligna melanoma or lentiginous melanoma. MATERIAL AND METHODS: We reviewed 14 cases of atypical lentiginous nevus diagnosed at the dermatology department of Hospital General de Valencia in Valencia, Spain between December 2007 and March 2009. We studied the clinical an…

Resolution and recurrence of vitiligo following excision of congenital melanocytic nevus.

Vitiligo associated with halo congenital melanocytic nevus (CMN) is rare. There are limited reports in the literature, especially with regard to CMN excision. We present the case of a 5-year-old girl who presented with vitiligo of the periorbital and axillary regions and halo formation around CMN of the buttock. The lesion was excised, and all areas of vitiligo improved, but 18 months postoperatively, a halo of depigmentation appeared around the excision scar and later in the periorbital and axillary regions. In review of literature, there is only one report of excision of halo CMN and resulta…

An inflammatory verrucous epidermal nevus concomitant with psoriasis: treatment with adalimumab.

A 42-year-old-man presented with generalized inflammatory linear verrucous epidermal nevus with concomitant psoriasis vulgaris. Because of unresponsiveness of the conventional treatment, we considered treatment with adalimumab. Adalimumab was well tolerated and no side effects were observed. Adalimumab has a limited effect in treating ILVEN lesions. However, it was effective in treating the psoriasis and pruritus related to ILVEN. For these reasons adalimumab can be used in treatment of ILVEN concomitant with psoriasis.

Mucinous nevus with fat: an unusual case report and literature review.

Mucinous nevus is a rare entity classified as either cutaneous mucinosis or connective tissue nevi. Clinically, multiple papules or plaques develop at birth or in early adulthood and grow to form verrucous or nevoid feature with a unilateral or often zosteriform distribution on the trunk. Histopathologically, it is characterized by mucin deposits localized in the papillary dermis, clearly distinguishing this entity from other types of cutaneous mucinosis. A 18-year-old male presented with multiple, skin-colored, pinhead to pea-sized, grouped, flat tapped, soft papules on the back. This skin le…

Bilateral renal artery stenosis and epidermal nevus syndrome in a child.

Epidermal nevus syndrome is a rare congenital sporadic neuro-ectodermic disorder, characterized by the presence of epidermal nevi in association with various developmental abnormalities of the skin, eyes, nervous, skeletal, cardiovascular and urogenital systems. We describe a 5-year-old boy with conjunctival lipodermoid, cervical and facial sebaceous nevi who presented at 3 years of age with hypertension due to bilateral renal artery stenosis together with multiple vascular anomalies (aorta, celiac trunk, superior mesenteric artery) as shown by magnetic resonance angiography. Systemic arterial…

Spitz nevus in a Hispanic population: a clinicopathological study of 130 cases.

Spitz nevus is an uncommon melanocytic nevus distinctive by its epithelioid and spindled melanocytes. Many studies have attempted to characterize Spitz nevus, but none of them in a Hispanic population. Our aim is to characterize the clinical and histopathological presentation of the Spitz nevus in a Hispanic population. A retrospective study was carried out from our files that included those cases histopathologically diagnosed as Spitz nevus. A blinded examination was performed to evaluate the histopathological characteristics of 130 lesions. The demographics of the patients, the anatomic loca…

Clinical, dermoscopic and histopathological evaluation of the Meyerson nevus: case report.

Meyerson nevi occur whenever a rare focal and transitory eczematous eruption arises around melanocytic lesions. The same phenomenon has also been observed in non-melanocytic lesions as well. Herein we report the case of a 25 year old, male patient, who had noticed, two months before, the arising of a pruriginous and erithematous halo around two nevi localized on his abdomen. The lesions were found to be atypical on dermoscopic examination and he was submitted to surgical excision of both nevi. Histopathological examination revealed displastic compound melanocytic nevi, surrounded by intraepide

Cellular blue nevus of the uterus: a case report and review of the literature.

Cellular blue nevus is extremely rare in the gynecologic tract with only 2 cases reported thus far, one in the myometrium and the other in the cervix/vagina/hymenal ring. In this report, we present the clinicopathologic features of the second case of cellular blue nevus arising in the myometrium. The patient was a 48-year-old woman who underwent a hysterectomy with bilateral salpingo-oophorectomy for uterine leiomyomas. On gross examination, the uterus showed a subserosal, 1.0×0.5×0.3 cm, dark red nodule in the anterior uterine wall in addition to multiple leiomyomas. Microscopically, the subs…

Nevus simplex: a reconsideration of nomenclature, sites of involvement, and disease associations.

BACKGROUND: Nevus simplex (NS) is a common birthmark on the forehead, glabella, upper eyelids, and nape. More widespread involvement can be confused with port-wine stains (nevus flammeus) and other vascular birthmarks. OBJECTIVES: To further categorize the anatomic locations in infants with extensive NS and evaluate for any possible disease associations. METHODS: We conducted a retrospective review of patients with extensive NS seen at two tertiary care centers. RESULTS: Twenty-seven patients with extensive NS were identified. All had at least one typical site of involvement: glabella (77.8…

Becker nevus on the lower limb: case report and review of the literature.

BACKGROUND: Becker nevus is a hamartoma with brown hyperpigmentation and hypertrichosis that usually affects young males. It has a predilection for the upper half of the trunk and proximal upper extremity. OBJECTIVE: This is a case report of a 38-year-old male with a Becker nevus without hypertrichosis on the lower limb and a review of the literature. RESULTS: Including our case report, 12 cases of Becker nevus on the lower limb were identified in the English literature (8 males and 4 females). It has never been reported below the knee. In males, the lesion was not associated with other anom…

Newborn with sebaceous nevus of jadassohn presenting as exophytic scalp lesion.

It is essential that various factors be considered when determining the differential diagnosis of congenital scalp lesions, including lesion size, appearance, intracranial extension, underlying medical condition and the embryological germ layer involved. We present the case of a newborn diagnosed as having a sebaceous nevus of Jadassohn scalp lesion. While a common congenital lesion, we describe the unusual presentation at birth as an exophytic nodular lesion. To our knowledge only one other case report of an exophytic congenital lesion has been published.

Linear nevus sebaceous syndrome: case reports and review of the literature.

Linear nevus sebaceous syndrome is reported to occur in as many as 1 in 1000 live births, and is hypothesized to result from genetic mosaicism involving a lethal autosomal-dominant gene. The terms "epidermal nevus syndrome" and "linear nevus sebaceous syndrome" are often used interchangeably, although linear nevus sebaceous syndrome more strictly applies to patients with a typical midline nevus, and epidermal nevus syndrome is a more inclusive term that includes all varieties of epidermal nevi. Linear nevus sebaceous syndrome encompasses a broad spectrum of abnormalities that may affect every …

Diagnosis and treatment of blue rubber bleb nevus syndrome in children.

BACKGROUND: Blue rubber bleb nevus syndrome (BRBNS) is characterized by distinctive vascular malformations of skin and the gastrointestinal tract, often leading to chronic anemia and intestinal bleeding. It usually presents right after birth or during early infancy. Though the disease is inherent, its occurrence is sporadic. Thus it is usually not timely diagnosed. We analyzed the clinical characteristics and treatment of this disorder in order to improve the diagnosis and treatment. METHODS: Three patients with BRBNS treated at our hospital during 2002-2003 and 39 patients from the literatur…

A rare conjunctival Spitz nevus: a case report and literature review.

A conjunctival Spitz nevus is a very rare, benign melanocytic lesion, which can be mistaken for a malignant melanoma. We present a case of a 28-year old man, who suffered from a rapidly growing, non-pigmented mass in the left caruncular area, extending to the nasal conjunctiva. The lesion was excised and pathologic examination showed nests of large, polygonal, non-pigmented epithelioid cells, located in the stroma. The overlying epithelium showed focal erosions. At the base, there was a lymphocytic infiltrate. Immunohistochemical techniques, with stainings for S-100 protein, HMB-45 and MIB-1, …

Segmental neurofibromatosis in association with a large congenital nevus and malignant melanoma.

We describe a patient with segmental neurofibromatosis and a large congenital nevus who developed malignant melanoma in the same anatomic distribution. The overlapping presentation suggests a common etiologic link between neurofibromatosis and large congenital nevi and the possibility that neurofibromatosis increases the risk of melanoma with large congenital nevi.
