Clinical assessment of infant colour at delivery.

OBJECTIVE: Use of video recordings of newborn infants to determine: (1) if clinicians agreed whether infants were pink; and (2) the pulse oximeter oxygen saturation (Spo(2)) at which infants first looked pink. METHODS: Selected clips from video recordings of infants taken immediately after delivery were shown to medical and nursing staff. The infants received varying degrees of resuscitation (including none) and were monitored with pulse oximetry. The oximeter readings were obscured to observers but known to the investigators. A timer was visible and the sound was inaudible. The observers wer

The young child with sickle cell disease.

Approximately 2,000 infants with sickle cell disease are born each year in the United States. Sickle cell disease is an inherited disorder of red blood cell hemoglobin. Sickle cells increase adhesion and cause blockage in the small blood vessels, resulting in tissue damage. The cells' production of hemoglobin S results in two major pathophysiologic features of sickle cell disorders: chronic hemolytic anemia and vaso-occlusion. These disorders cause ischemic tissue damage and acute and chronic organ failure. Potential complications for children with sickle cell disease include vaso-occlusive ev

Successful newborn sickle cell trait counseling program using health department nurses.

Every year more than 50,000 infants with sickle cell trait are identified through newborn screening programs. An infant with trait provides a "genetic window" into a family that may be at risk for having a child with sickle cell disease. No national standards have been established for newborn trait follow-up and family counseling. In the United States, counseling to families having an infant with trait is usually very limited in scope and, in many cases, nonexistent. In West Tennessee, the Mid-South Sickle Cell Center (MSSCC) and the Department of Health have jointly developed an effective, pr

Gene therapy techniques in the treatment of adenosine deaminase--deficiency severe combined immune deficiency syndrome.

Gene therapy is an innovative technique utilized in the treatment of a primary immunodeficiency known as severe combined immune deficiency syndrome (SCIDS). The first human trials of gene therapy for SCIDS were conducted in 1990, by attempting to insert the gene for adenosine deaminase into peripheral white blood cells to treat children with adenosine deaminase (ADA)-deficiency SCIDS. Recently, three infants with ADA-deficiency SCIDS received gene therapy with genetically manipulated hematopoietic stem cells. This clinical update reviews basic immunology function and malfunction, describes gen

Education Research: Feasibility and Effect of Multinational Virtual Educational Series in Neonatal Neurology Across Latin America: The NeoNERd LatAm Initiative.

BACKGROUND AND OBJECTIVES: Neurologic disorders in the neonatal period are a leading cause of death and disability in low- and middle-income countries, including Latin America. Access to specialized neonatal neurocritical care remains limited, and educational opportunities are scarce. We aimed to evaluate the feasibility, reach, knowledge acquisition, and perceived value of a virtual educational series in neonatal neurology targeting health care professionals across Latin America. METHODS: This prospective educational intervention included 6 case-based webinars delivered between October 2023

Closing the Educational Gap: Engaging Partners in Maternal Sepsis Recognition After Unscheduled Cesarean Birth.

Maternal sepsis accounts for 5% of obstetric intensive care unit admissions and 23% of in-hospital maternal deaths in the United States, 63% of which are preventable. Individuals who undergo unscheduled cesarean birth face elevated postoperative infection risk, yet partners and support persons are rarely included in discharge sepsis education. During recovery, the birthing person simultaneously manages surgical pain, sedation effects, and neonatal care demands, all of which can delay early sepsis recognition. This article presents an evidence-based framework for nurses to deliver partner-focus

Listeria monocytogenes--which of your patients is not at risk?

Listeria monocytogenes is an environmental bacterium that is capable of causing infection (known as listeriosis) in vulnerable people who generally acquire the organism following the ingestion of contaminated foods. Pregnant women are a particular risk group, as listeriosis in pregnancy is associated with an increased risk of spontaneous abortion, stillbirth, neonatal septicaemia and meningitis. The immunocompromised, and people over the age of 65 to 70 are also at increased risk of listeriosis. Listeriosis can present in a number of ways and may lead to significant morbidity. The mortality ra

One-stage neonatal reconstruction without stoma for Hirschsprung's disease.

Interest in primary one-stage reconstruction for Hirschsprung's disease has been increasing steadily because of the obvious clinical quality issues and resource benefits. This report describes one surgeon's 13-year experience with 52 children who had proven Hirschsprung's disease. The patients were managed in a neonatal surgical unit by a one-stage neonatal reconstruction without stoma. Results indicate that the procedure is not age- or weight-dependent; it should be considered only when parameters for safe neonatal anesthesia, nursing, surgery, and expert pathology are available. Complication

Construction of a sensitive indicator system for the quality of neonatal jaundice care.

BACKGROUND: Neonatal jaundice is a prevalent neonatal condition, with improper management potentially leading to severe neurological damage. Significant disparities in care quality exist globally, highlighting the urgent need for a standardized, disease-specific nursing quality evaluation system. AIMS AND OBJECTIVES: This study aimed to construct a scientifically rigorous and reliable sensitive indicator system for evaluating the quality of neonatal jaundice care, based on Donabedian's structure-process-outcome model. METHODS: A methodological study design was employed. An initial draft was

Exploratory study: knowledge about the benefits of breastfeeding and barriers for initiation in mothers of children with spina bifida.

The aim of the study is to identify the level of knowledge on breastfeeding by mothers of infants with spina bifida and the barriers encountered in initiating breastfeeding. A non-probabilisitic sample (n=30) of mothers was used in this study. The sample was obtained in two institutions specializing in care for infants with congenital anomalies in Puerto Rico. A self-administered questionnaire was used consisting of six sections. Descriptive statistics were used for data analysis. Among participants, 56.7% had adequate knowledge on the benefits of breastfeeding. The most frequently encountered

Effectiveness of a risk perception-based nursing model for high-risk pregnant women with gestational diabetes: A retrospective cohort study.

Gestational diabetes mellitus (GDM) is a common and high-risk pregnancy complication, increasing the risk of maternal hypertension, preterm birth, infections, and other issues, as well as long-term effects on the newborn. This study aimed to assess the effectiveness of a risk perception-based nursing model for high-risk GDM pregnant women, focusing on blood glucose control, self-management, psychological adjustment, and pregnancy outcomes. A retrospective cohort design was used, with clinical data collected from 106 high-risk GDM pregnant women between June 2023 and December 2024. The observat

Horizontal hostility and verbal violence between nurses in the perinatal arena of health care.

The goal of this US study was to determine the frequency of horizontal violence in a perinatal service and its effect on patient outcomes. A 24-question survey instrument was completed online by 63 nurses. The results indicated that labour and delivery wards experience a higher frequency of horizontal violence than other units in the perinatal service. They also showed that the mother and baby unit demonstrates a higher frequency of recipient or victim behaviours. A relationship between horizontal violence and ineffective communication, as well as a relationship between horizontal violence and

Active versus expectant management of the third stage of labor and implementation of a protocol.

Although the focus at delivery may naturally shift to infant transition, continued maternal vigilance during stage 3 is imperative to accomplish a safe outcome for the mother and her newborn. The third stage of labor is a normal physiological progression of birth that may be compounded by serious complications. The most common complication is postpartum hemorrhage due to uterine atony. Clinicians choose either active management or expectant management for stage 3 to prevent excessive maternal blood loss. Rapid identification and response to a postpartum hemorrhage are critical. A wireless physiological parameter monitoring system with a treatment feedback function during neonatal phototherapy.

Objective. Neonatal jaundice is a common condition in the early stages of newborns, and phototherapy is a fast, safe and effective method that is used to treat it. However, recent studies have shown that phototherapy may elicit side effects in infants, such as hypothermia, hyperthermia and dehydration. To improve the quality of phototherapy and the prognosis of patients, the changes in neonatal physiological parameters during phototherapy should be monitored to give better feedback to pediatricians or the phototherapy system. However, the current standard of clinical care during neonatal photo

[Video recording of newborn resuscitation in the delivery room: technique and advantages].

An extreme attentiveness is mandatory when caring about extremely-low-gestational-age neonates at birth because of their innate vulnerability. The interventions performed during resuscitation of these infants may have direct influence on the immediate survival and also on long-term morbidity. Although stressfull, each resuscitation step is crucial and needs to be precise, fast and harmless. In order to determine our compliance to the international guidelines and to assess our neonatal performances in delivery room, we used a Mobotix camera to record all resuscitations of extremely-low-gestatio
