[Clinical study on 16 cases of testicular torsion].

Sixteen cases of testicular torsion were experienced between 1983 and 1988. Diagnosis was confirmed by surgical exploration in all cases. Th … Body temperature was more than 37 degrees C in only 1 case. Urinary tract infection was not noted. Orchiopexy was performed in 12 cases. 9 cases (75%) were treated within 24 hours after onset of the symptom. Two cases (13%) were operated more than 72 hours after onset, but repaired testis was not atrophic one year after surgery.

Treatment of cryptorchidism with low doses of buserelin over a 6-months period.

In a collaborative study, 48 prepubertal boys with undescended testes ranging in age from 15 months to 11 years were treated with low-dose intranasal buserelin following an every-other-day programme for a period of 6 months. Urinary LH, FSH, and testosterone were not altered during the treatment period. Boys over 7 years of age experienced a slight but significant rise in testosterone at the end of treatment. Testicular descent was achieved in only 17% of boys. In the remainder, bilateral testicular biopsies were obtained during orchiopexy. Grouped analysis showed a significant increase in the…

The importance of door to detorsion time in children diagnosed with testicular torsion - Every minute counts!

INTRODUCTION/BACKGROUND: Testicular torsion (TT) represents an emergent pediatric condition that requires timely surgical intervention to mi … th a viable testes and 166 min with a nonviable testes. A total of 195 (68 %) patients underwent an orchiopexy compared to 91 (32 %) patients who had an orchiectomy. A multivariate analysis demonstrated that each 10-min increase in DTD was associated with a 6 % increase in odds of undergoing an orchiectomy. Each additional hour of DTD was associated with a 3 …

Gubernacular discontinuity and abnormal distal fixation in cryptorchidism: Challenging the classical concept.

BACKGROUND: The gubernaculum is essential for testicular descent, but its detailed surgical anatomy remains poorly understood. We have previously identified an unrecognized anatomy of the round ligament in female patients with sliding inguinal hernias. OBJECTIVE: This study investigated whether comparable anatomical features exist in the gubernaculum of male cryptorchidism patients, as compared to those identified in female sliding hernias. MATERIALS AND METHODS: We retrospectively analyzed undescended testes located in the inguinal canal that underwent open inguinal orchidopexy between 2016

Orchidopexy for undescended testis-rate and predictors of re-ascent.

PURPOSE: This study aimed to investigate the rate of re-ascent requiring re-operation after primary orchidopexy and to investigate eventual differences between the inguinal and scrotal approach as well as other potential predictors for re-ascent. METHODS: A retrospective cohort study of children treated for undescended testis (UDT) with orchidopexy between 2018 and 2022 was conducted. The primary outcome was re-ascent requiring re-operation, and the secondary outcome was atrophy rate. Independent variables were age, underlying conditions, side, surgical approach, operation time, bilaterality,

Recurrent testicular torsion post orchidopexy - an occult emergency: a systematic review.

BACKGROUND: Recurrent Testicular Torsion (RTT) is a rarely reported event after previous testicular torsion (TT) repair. Both conditions have similar signs and symptoms. Various techniques have been attempted to reduce the incidence of retorsion. This review assesses the presentation, diagnosis, risk factors, management and outcomes associated with RTT. METHODS: After PROSPERO Registration (CRD42021258997), a systematic search of PubMed, Google Scholar, Embase, Scopus, Web of Science, Cochrane Database of Systematic Reviews, Global Index Medicus and Cumulative Index to Nursing and Allied Heal

Assessment of referral patterns for boys with suspected undescended testes and identification of risk factors to aid diagnosis.

INTRODUCTION: Undescended testis (UDT) is a clinical diagnosis and a common reason for referral to paediatric urology outpatients. Our aim was to assess current referral patterns at our unit and to identify predictive factors that may better aid primary care physicians (PCP) in diagnosing UDT based on history and physical exam. METHODS: A retrospective analysis of referrals to outpatients from 2014 to 2016 was performed to assess current referral patterns including referral source, age, reason for referral and outcome following assessment by a single consultant paediatric urologist.A prospect

Surgical findings in cryptorchidism in children with Zika-related microcephaly: a case series.

BACKGROUND: Complications in the urinary tract related to congenital Zika syndrome have recently been reported. One complication, cryptorchidism, has been reported by the Microcephaly Epidemic Research Group/MERG, in Pernambuco/Brazil. The present article describes for the first time the surgical findings in a case series of boys with Zika-related microcephaly and cryptorchidism, who underwent surgical testicular exploration as a contribution to better understand the possible mechanisms involved in gonads formation and descent. METHODS: A total of 7 children (11 testicular units), aged 3 to 4

Blind ending vessels on diagnostic laparoscopy for nonpalpable testis: Is a nubbin present?

INTRODUCTION: The traditional management paradigm for nonpalpable testis (NPT) has been that inguinal or scrotal exploration for a nubbin may be omitted when blind ending vessels are observed during diagnostic laparoscopy. Our aim was to examine whether blind ending vessels excluded the presence of a nubbin in a series of boys who underwent exploration in this setting. MATERIALS AND METHODS: Using a surgical database and chart review, pre-pubertal boys (≤12 years) with the diagnosis of undescended or atrophic testis who underwent a diagnostic laparoscopy for unilateral NPT between 2000 and 20

Abdominoplasty in Prune Belly Syndrome.

INTRODUCTION: Many patients with Prune Belly Syndrome (PBS) require abdominoplasty alone or in combination with correction of any urogenital abnormalities. This video presents a simplified technique with which to treat the abdominal flaccidity in PBS. METHODS: A longitudinal xypho-pubic fusiform figure is drawn on the abdomen, based on the area of skin and subcutaneous tissue to be removed. This is performed with preservation of the musculo-fascial layer and the umbilicus. A lateral elliptical single xypho-pubic line is drawn in the most lax side of the fascia, which is incised along this lin

Achieved pregnancy with the delivery of a healthy child by TESE-ICSI 7 years after bilateral adult orchidopexy: a case report.

We report pregnancy with the delivery of a healthy child by TESE-ICSI 7 years after bilateral adult orchidopexy. A 29-year-old patient presented with infertility and previous bilateral cryptorchidism, but no surgical treatment had ever been performed. His partner had been assessed by a gynaecologist, and no contributing female factors were detected. Orchidopexy and conventional testicular sperm extraction (TESE) were performed and microdissection TESE 10 months after orchidopexy. The second microdissection TESE with intracytoplasmic sperm injection (ICSI) was performed 7 years after orchidopex

A critical appraisal of the evidence for improved fertility indices in undescended testes after gonadotrophin-releasing hormone therapy and orchidopexy.

Male fertility depends on the transformation of gonocytes into dark adult spermatogonia, during the first 3 months of postnatal life, and this is an androgen-dependent process. This essential developmental step appears to be defective in undescended testes, and in many patients orchidopexy alone (at the age it is currently performed) does not improve fertility indices, either because it does not address the underlying pathophysiology or the surgery is performed too late. Hormone therapy with gonadotrophin-releasing hormone creates a rise in testosterone levels, copying the postnatal gonadotrop

Natural history and long-term testicular growth of acquired undescended testis after spontaneous descent or pubertal orchidopexy.

OBJECTIVE: To assess prospectively the natural history and long-term testicular growth of acquired undescended testis (UDT) after spontaneous descent or pubertal orchidopexy in case of nondescent. PATIENTS AND METHODS: From 1996 until 2008, 391 boys with 464 acquired UDT were included in the study. In accordance with Dutch consensus on non-scrotal testes, spontaneous descent at puberty was awaited; if this did not take place, orchidopexy was performed at puberty. Acquired UDT was defined as a testis previously residing in the scrotum that can no longer be manipulated into a stable scrotal (hi

Histopathological Findings of Testis Torsion According to Parenchymal Regions: New Recommendations for Damage Detection and Future Directions.

Testis torsion is a scrotal emergency in which parenchymal damage cannot be accurately predicted. This situation leads to atrophy in the follow-up of patients who underwent orchidopexy, and potentially reversible injury can be detected in the orchiectomy series. This study aimed to find answers to questions about the accurate detection of testes with potentially reversible injury. The testes of patients who underwent orchiectomy due to testis torsion under the age of 35 (n = 54) were re-examined. Considering the vascular anatomy of the testis, necrotic changes in the epididymal, anti-epididyma

Efficacy of the transscrotal approach in cord lengthening for palpable undescended testes in children.

BACKGROUND: The transscrotal approach has been considered a promising alternative for the management of palpable undescended testes in children; however, it has not been widely adopted. One major concern is whether this approach provides sufficient cord length to allow tension-free descent of the testis into the scrotum. This study aimed to evaluate the cord-lengthening efficacy of the transscrotal approach by comparing the pubo-testicular distance after high ligation of the patent processus vaginalis (PTd1) with the pubo-scrotal distance (PSd) in children with low palpable undescended testes.

Factors which delay surgery for undescended testis in Ontario: A retrospective population based cohort study on timing of orchidopexy between 2006 and 2012.

BACKGROUND: Delays in performing orchidopexy (OP) for undescended testis (UDT) result in loss of germ cells. Despite practice guidelines recommending OP by 18 months of age, significant delays in OP occur. This delay may impact fertility and increase the risk of testicular malignancy in boys with UDT. OBJECTIVE: The objective of this study is to identify factors associated with delayed OP with the goal of identifying modifiable risk factors. STUDY DESIGN: A population-based, retrospective cohort study was conducted using linked databases held at ICES to evaluate factors associated with timin

[German guideline on undescended testis-what is relevant in daily routine?].

With an incidence of 0.7-3% in male infants, undescended testicles is one of the most common congenital anomalies. In the first 6 months of life, the testicles may spontaneously descend in up to 70% of individuals. If the testicle is not in a scrotal position afterwards, fertility can gradually be reduced and the risk of a testicular tumor increases. Therefore, the current German guideline for undescended testis recommends that therapy should be take place between 6 and 12 months of life. After extensive information on the advantages and disadvantages, hormone therapy with the aim of a descens

Changing Trends in the Referral and Timing of Treatment for Congenital Cryptorchidism: A Single-Center Experience from Bosnia and Herzegovina.

BACKGROUND: Cryptorchidism is the most common male urogenital tract disorder identified at birth. Treatment delays of cryptorchidism may be associated with significant complications such as subfertility and testicular cancer. The currently recommended age for performing orchidopexy is between 6 and 12 months of age and no later than 18 months. The aim of this study was to investigate the trends in the pattern of referral and age of boys at the time of operative treatment of congenital cryptorchidism at the largest tertiary care center in Bosnia and Herzegovina. METHODS: The study included all

Transverse testicular ectopia with persistent mullerian duct syndrome.

Persistent mullerian duct syndrome (PMDS) is a rare form of male pseudohermaphroditism caused by defects in synthesis or actions of mullerian inhibiting factor characterised by persistence of mullerian duct structures in a normal karyotype male. Transverse testicular ectopia (TTE) is a rare disease in which both testes are located in the same inguinal canal. We report a case of PMDS with TTE in a 1-year-old child who presented with non-palpable testis on right side with hernia on left side. Left herniotomy with bilateral trans-septal orchidopexy was done in this patient.

Beware the Looping Vas Deferens in Orchidopexy.

A looping vas deferens may be present in up to 20% of undescended testes located within or proximal to the inguinal canal. This associated abnormality is vulnerable to transection during orchidopexy. We present an example of a very long looping vas to emphasize the importance of inspection for this anomaly, and demonstrate the extreme extent that a looping vas may extend. Identification of the vas among cord structures may provide false reassurance of normal ductal anatomy. Examination for a looping vas by inspecting structures caudal to the testis should occur at an early opportunity during o
