Effect of foot orthoses on tibialis posterior activation in persons with pes planus.

PURPOSE: To examine the influence of footwear on tibialis posterior (TP) activation in persons with pes planus. METHODS: Six asymptomatic adults with pes planus (arch index of < or =0.16) participated. Subjects performed a resisted foot adduction with plantar flexion exercise (3 sets of 30 repetitions). The exercise was performed barefoot and shod with foot orthoses. The two testing conditions were separated by a week. Magnetic resonance image signal intensity of the tibialis posterior, tibialis anterior, soleus, medial gastrocnemius, and peroneus longus was measured immediately before and af

Effects of a new spinal orthosis on posture, trunk strength, and quality of life in women with postmenopausal osteoporosis: a randomized trial.

OBJECTIVE: One fourth of women > or =50 yrs of age in the general population have one or more vertebral fractures. The orthotic treatment modality in the management of vertebral fractures caused by osteoporosis remains subjective because no objective data from clinical trials are available. The objective of this research was to evaluate the efficacy of a newly developed spinal orthosis in patients with osteoporotic vertebral fractures. DESIGN: We conducted a study that measured trunk muscle strength, angle of kyphosis, body height, body sway, and variables of quality of life such as pain, wel

Treatment of true developmental dysplasia of the hip using Pavlik's method.

Ultrasound is the most sensitive method for detecting hip pathology in the first year of life. However, it has been shown that neonatal sonography, or combined sonographic-clinical screening, can cause some over-diagnosis which, when not used properly, can lead to over-treatment. We developed the concept of 'true developmental dysplasia of the hip' on the basis of our experience. Despite the still-growing popularity of Pavlik's method in the treatment of the condition, we were unable to find a study dealing with the relationship between sex, sonographic and clinical pathology, age at the start

Treatment of stage 4 adult acquired flatfoot.

Stage 4 PTT dysfunction is a rare anatomic condition in which fixed hindfoot valgus is associated with valgus tilting of the talus within the ankle mortise. Success with nonoperative management is the exception rather than the rule. The surgical options are a tibiotalocalcaneal fusion or a pantalar fusion; however, there are few results reported in the adult acquired flatfoot population. Valgus talar tilting after triple arthrodesis may be the challenge of the future.

Surgical correction of lumbar kyphotic deformity: posterior reduction "eggshell" osteotomy.

OBJECT: Progressive kyphotic deformity of the lumbar or thoracolumbar spine may lead to back pain, cosmetic deformity, and risk of neurological compromise. The authors describe a series of patients in whom they performed a single-stage, posterior reduction ("eggshell") osteotomy procedure to improve sagittal contour by creating lordosis within a single vertebral body. METHODS: From 1995 to 1997 the authors performed 12 osteotomy procedures in 11 patients with thoracolumbar or lumbar kyphosis. Seven patients presented with iatrogenic deformity, three with deformity secondary to traumatic injur

An electromyography study of wrist extension orthoses and upper-extremity function.

OBJECTIVE: This study examined the effect of commonly used long and short styles of commercially produced wrist extension orthoses on the activity of the proximal muscles of the shoulder and elbow and on wrist flexor and extensor muscle activity. METHOD: While 17 women between 22 and 40 years of age (M = 26.6) performed a specified movement wearing each of the two styles of orthosis and without an orthosis, their motor unit recruitment of five proximal joint muscle groups, wrist extensors, and wrist flexors was measured by surface electromyography. RESULTS: Motor unit recruitment was signifi

Geste device in tardive dystonia with retrocollis and opisthotonic posturing.

Retrocollis and opisthotonic posturing are typical clinical features of tardive dystonia and may be extremely disabling. We have treated five such patients with a custom-made mechanical device that delivers constant contact to the occiput and shoulders. All patients experienced relief and their walking improved. This geste device, working like a geste antagonistique, may be useful for the management of some patients with severe axial dystonia with retrocollis and back arching.

Effectiveness of external orthotic support on passive soft tissue resistance of the chronically unstable ankle.

Functional ankle instability, orthoses, and passive resistive torque tolerated have not been researched. The purpose of the study was to evaluate the passive resistance torque exerted by a flexible and semirigid orthosis for individuals with chronic instability. Twenty-two subjects were evaluated on the passive ankle resistance unit during unbraced, flexible, and semirigid brace conditions. Data from the final three trials for each condition were analyzed using a multiple analysis of variance with repeated measures for resistive torque and inversion range of motion. The semirigid and flexible

Three-dimensional effect of the Boston brace on the thoracic spine and rib cage.

STUDY DESIGN: Three-dimensional reconstructions of the spine and rib cage were done and compared just before and 1 month after initiation of treatment with a Boston brace in a group of adolescents with idiopathic scoliosis. OBJECTIVES: To document the immediate changes in shape of the thoracic spine and rib cage induced by the original Boston brace design. SUMMARY OF BACKGROUND DATA: The effect of the Boston brace has been well documented in the frontal plane but is poorly understood in the other planes of deformity. METHODS: Three-dimensional reconstructions were obtained with and without

Traumatic focal posterior tibialis muscle denervation.

The purpose of this case presentation is to demonstrate posterior tibialis muscle (PTM) denervation as a cause of traumatic plantarflexion inversion weakness. In a 42-year-old woman, severe pain, swelling, and ecchymosis over the medial aspect of her left ankle developed after she twisted it while playing tennis. Plantarflexion inversion weakness developed (grade 3/5). The strength of all other muscle groups of the lower extremity was normal. Her pin and light touch sensation were normal in the left lower extremity. Deep tendon reflexes were equal and active at both knees and ankles. A magneti

[Linear scleroderma. Report of 2 cases with a review of the disease entity].

We report on two patients with linear scleroderma (a form of localised scleroderma) and their orthopaedic problems such as shortening of the limbs and contractions of the joints. The disease is very rare but easy to diagnose by its typical lesions of the skin (band like lesions with ivory like induration and hyper- or hypopigmentation; a peripheral erythema appears as a lilac ring). In the orthopedic management it is very important to follow the patient carefully during the period of growth for good planning of the conservative (physiotherapy and orthosis) and operative therapy, which in one o

Physeal injuries in myelomeningocele patients.

Epiphysiolysis from a series of 253 patients under regular control in the Spina Bifida Unit at our hospital has been analysed from 1967 to the present time. From that total there have been 18 physeal fractures in 9 of the myelomeningocele patients. The clinical characteristics, diagnostic difficulties and therapeutic criteria for this type of lesion are considered, and we suggest the need to use suitable orthoses whenever the patient is able to walk, and to temporarily increase the level of orthotisation in patients if the physis of the knee is affected.

Conservative treatment of idiopathic clubfoot.

Operation in the early stage of clubfoot is still a controversial issue. Twenty-five patients treated conservatively were studied at the age of 4-16 years. In 95%, the outcome was either excellent or good according to Laaveg and Ponseti's rating system. Operation appears not to be indicated when the residual varus, adductus, or intoeing is passively correctable at least to the neutral position. Residual equinus without fixed varus was correctable by serial casting in childhood. Muscle imbalances, which make maintenance of the corrected position difficult in infancy, tend to improve with growth

Congenital dislocation of the knee.

Congenital dislocation of the knee (CDK), first described in 1922, is a rare condition sometimes associated with other congenital malformations. There is a total anterior dislocation with recurvatum and upward displacement of the proximal tibia. This review covers our ten year experience at the hospital San Juan de Dios, in Caracas, Venezuela, a children's hospital with three orthopaedic services. We have seen 22 affected knees in 14 patients. Newborn patients were treated on traction until reduction was achieved, followed by serial plaster of Paris casts until, optimally, 90 degrees of flexio

Assessment and management of pes cavus in Charcot-Marie-tooth disease.

The sequential approach to evaluating the cavus foot is integrated with a description and assessment of the various treatment options. Decision making in the treatment of these cases is complicated by the progressive neurologic condition that underlies many of these deformities. An effort is made to recommend the most appropriate surgical intervention based on the nature of the deformity and its rigidity. Although these principles apply to all cavus feet, the deformity in Charcot-Marie-Tooth disease is the most difficult to treat and the most prone to recurrence because of the progressive natu

[Pavlik's method of reduction treatment of congenital hip dislocation].

In 68 patients with 101 congenital dysplasias of the hip results of treatment by Pavlik harness were retrospectively analysed. Of the 101 there were 61 dislocations. After six weeks of treatment with the Pavlik harness reduction was achieved in 48 of the 61 dislocated hips (80%). However, because of persisting dysplasia 50% of all cases had to be treated with an abduction device for a few months. There were no complications, in particular osteonecrosis of the femoral head was not observed. Provided well controlled, the Pavlik harness is a good, safe and easy method for treating congenital disl

Management of a patient with a diagnosis of bilateral plantar fasciitis and Achilles tendinitis. A case report.

This case report describes an approach for determining when fabricated orthoses might be useful in the treatment of a patient with foot-related problems. The patient is of special interest because she is an aerobics instructor, and her type of injury is relatively common. Particular emphasis in the report is placed on hypothesizing the cause of the patient's complaints and then basing treatment on that hypothesis.

Juvenile idiopathic scoliosis followed to skeletal maturity.

The authors reviewed 43 patients with the onset of idiopathic scoliosis between 4 and 9 years who were followed to skeletal maturity to document the natural history, effects of bracing, and factors associated with progression. Average age at onset was 7.1 years for female and 6.7 for male patients. Twelve were treated with observation only; six improved, five progressed, and one refused an orthosis and required surgery. Thirty-one patients wore an orthosis; five patients improved or were unchanged, and 26 progressed, including 13 who subsequently required surgery. All patients treated with obs

Lessons learned in the management of Legg-Calvé-Perthes disease.

In the assessment of Legg-Calvé-Perthes disease, true lateral radiographs of the femoral head are required. Lack of head sphericity does not preclude good range of motion, which may mislead one into expecting a favorable outcome in very young patients; nor does head flattening prevent successful surgical containment and restoration of sphericity in some older children. Slings and springs, broomstick casts, soft tissue releases, and day or night bracing all have a place in obtaining and retaining a good range of abduction. If orthotic management is to be employed, it must be consistently effect
