Central corneal thickness in osteogenesis imperfecta and otosclerosis.

The central corneal thickness was measured in 53 patients with osteogenesis imperfecta. The average thickness was found to be 0.443 +/- 0.003 (SEM) mm. This value was significantly lower than the central corneal thickness in 35 patients with otosclerosis (0.515 +/- 0.004 mm) and in 35 sex- and age-matched normal control subjects (0.522 +/- 0.004 mm). No difference in corneal thickness between otosclerosis and controls was observed. Measurements of the central corneal thickness may be of great value in the diagnosis of osteogenesis imperfecta.

Occurrence of malignant hyperpyrexia in a patient with osteogenesis imperfecta.

Probable malignant hyperpyrexia (MH) developed and was successfully treated in a 20-yr-old man during anaesthesia for reduction of a fractured mandible. The sister of this patient had died after an anaesthetic at the age of 14 yr, but malignant hyperpyrexia was not suspected. Subsequent enquiries revealed that the patient and his sister both had osteogenesis imperfecta. This case illustrates the infrequently reported association of malignant hyperpyrexia with osteogenesis imperfecta, and the difficulties in obtaining an adequate personal and family history of previous anaesthetics.

Strangulated diaphragmatic hernia in a patient with osteogenesis imperfecta.

Hernias are common in patients with osteogenesis imperfecta, but to our knowledge there have been no reports of diaphragmatic hernias in such patients. We describe a patient with osteogenesis imperfecta in whom a diaphragmatic hernia contained a strangulated segment of the splenic flexure of the colon. Resection of the necrotic tissue and transverse colostomy resulted in an uneventful recovery.

The prenatal ultrasonographic diagnosis of osteogenesis imperfecta lethalis.

The ultrasonographic features of osteogenesis imperfecta lethalis (OI Type II), including those not reported previously (*), are: In the skull: *Abnormal compressibility of the vault by the transducer, which may alter measurements of biparietal diameter. *Unusually good visualization of the orbits. *Increased visualization of arterial pulsations. *Increased through-transmission of the ultra-sound beam due to extremely poor mineralization. In the remainder of the skeleton. *Diminished visualization and increased through-transmission of the remainder of the skeleton, particularly of the spine, r…

Osteogenesis imperfecta with dominant inheritance and normal sclerae.

Most patients with dominantly inherited osteogenesis imperfecta have blue sclerae and relatively mild symptoms. However, in a small group of families the patients have normal sclerae and this disorder has been classified as Type 4 osteogenesis imperfecta. This paper reports the clinical and radiographical features of 48 patients from 16 families with Type 4 osteogenesis imperfecta and compares the findings with those of the classical disorder with blue sclerae (Type 1 osteogenesis imperfecta). The two types are similar in usually causing a mild disease but with a wide range of severity, and in…

Hearing and middle ear function in osteogenesis imperfecta.

Fifty-five patients with osteogenesis imperfecta (OI) were studied to determine the extent to which the peripheral auditory mechanisms share in the connective tissue lesion. Ninety-two unaffected relatives and 43 control subjects were also studied. Subjects were divided into age groups younger than and older than 30 years. Hearing loss, most frequently sensorineural, occurred in 49% (younger than 30 years) and 94% (older than 30 years) of patients with OI. A sensorineural pattern of hearing loss, here considered characteristic of OI, was observed in 47% of OI subjects irrespective of age, in 4…

Low ocular rigidity in patients with osteogenesis imperfecta.

Sixteen patients with osteogenesis imperfecta (OI) have undergone a thorough eye examination. These patients had statistically significantly lower ocular rigidity measurements than a group of normal volunteers matched on age, sex, and refractive error. In addition, the corneal diameter and length of the eyeball was smaller in OI patients than that in controls. Possible correlations of low ocular rigidity with biochemical changes in scleral collagen await further investigation.

Nursing care of an osteogenesis imperfecta infant and child.

Because the child with osteogenesis imperfecta is hospitalized in the same setting many times throughout his childhood, the nursing staff become well acquainted with him and his family, and are able to assess the child's and parents' understanding of this chronic debilitating disease. The staff can help them to develop a realistic attitude toward their life and future plans. The family should be encouraged to see that the OI child attends a regular school whenever possible so that their active, excellent minds can develop. The family and child should be assured that an OI child can be educated…

Internal fixation of the femur in patients with osteogenesis imperfecta.

The operation of fragmentation and intramedullary rod fixation of the femur with an extensible device has proved valuable for children with osteogenesis imperfecta because it permits these children to remain ambulatory. The use of an extensible rather than a solid rod has greatly decreased the need for reoperation.

"Popcorn" calcifications: a prognostic sign in osteogenesis imperfecta.

"Popcorn" calcifications appear roentgenographically as a collection of scalloped radiolucencies, each with a sclerotic margin and in some instances, with central densities. The radiographs of 46 children with osteogenesis imperfecta were reviewed retrospectively; 52% had "popcorn" calcifications at one or more sites. A study of these children indicated that these lesions are observed only during active skeletal growth, are not present at birth, occur predominantly around the knees and ankles, and are associated with irregularity or absence of the normal horizontal growth plate. Based on multi…

Operative treatment of spine deformity in osteogenesis imperfecta.

There has been little mention in the literature of scoliosis associated with osteogenesis imperfecta. The purpose of this study was to investigate the problem of spine deformity in a series of osteogenesis imperfecta patients and to review the operative treatment of 8 patients. A series of 49 patients (ranging in age from 8 months to 57 years, average 12 years) with osteogenesis imperfecta were studied. Thirty-five (71%) of these patients had scoliosis ranging from 7 degrees to 105 degrees (average 42 degrees). The more severely involved patients (OIC and OITI groups) demenostrated a higher in…

[Treatment of osteogenesis imperfecta with (+)-catechin (author's transl)].

4 girls aged 4--12 years with various forms of osteogenesis imperfecta were treated with (+)-catechin for several months. Bone punch biopsies from the iliac crest were investigated by electron microscopy before and during treatment. The frequency of fractures decreased clinically. There were no radiographic changes in the bones and there were no side effects. Electron microsopy showed a dilated coarse endoplasmatic reticulum with infrequent ribosomes, thin collagen fibrils and decreased predominantly disseminated mineralisation before treatment. Under the influence of treatment electron micros…

Osteogenesis imperfecta. A study of 160 family members.

Osteogenesis imperfecta, a genetic disease characterized by blue sclera, fragile bones, and hearing loss, was studied in 160 descendants of a single, affected individual. One hundred twenty family members in four generations were at risk of inheriting the gene for osteogenesis imperfecta. Fifty-six percent (68/120) had blue sclera, and, of this group, 66% had fragile bones, and 47% of those tested had hearing loss. The onset of hearing loss was usually in the second or third decade, and was primarily of the conductive type. The incidence of sensorineural hearing loss was only slightly less tha…
