Sialolithiasis of an Accessory Parotid Gland Causing Mid-Cheek Pain.

A 52-year-old woman presented with a slightly painful swelling of the right cheek area without postprandial aggravation of the symptom last 1 week. On physical examination, an approximately 1.5 cm sized firm, slightly tender mass was palpated on the right mid-cheek region, and she did not show any abnormal findings on neurologic examination. Computed tomography showed a well-enhanced lesion with low density of internal portion in right cheek area, and tiny calcification was noted within internal low-density portion of cheek mass. These findings suggested sialolithiasis of an accessory parotid

Diagnosis of unknown sarcoidosis associated to erythema nodosum with 18F-FDG PET/CT.

A 76-year-old woman was referred to our centre to perform (18)F-FDG PET/CT with the clinical suspicion of vasculitis. Whole body PET was negative for vasculitis but it depicted moderate hypermetabolism in several lymph nodes of the mediastinum and intense uptake of the tracer in the parotid glands. Since the patient referred skin lesion on both legs a particular acquisition of the lower extremities was performed which showed diffuse uptake on the perimalleolar region of both legs. On the basis of the PET/CT findings that were suggestive for sarcoidosis the patient performed bronchoalveolar lav

Melioidosis of the head and neck.

Melioidosis is a potentially deadly infection that can affect any organ system. Reports of melioidosis of the ENT/head and neck region are relatively uncommon. Four cases are presented: (i) parotid abscess evolving into necrotising fasciitis, (ii) acute sinusitis and parapharyngeal cellulitis resulting in upper airway obstruction, (iii) acute suppurative lymphadenitis (iv) and chronic suppurative otitis media causing meningoencephalitis. Three of the four cases are believed to be unique, as a literature review of melioidosis in ENT/head and neck is also presented. Some practical issues of mana

Surgical management of deep dermatophytosis in a patient with CARD9 deficiency.

Deep dermatophytosis is a dermal infection caused by Dermatophytes. It can cause deeper dermal dermatophytosis, Majocchi's granuloma, dermatophytic pseudomycetoma or a widespread infection. CARD9 deficiency is a known risk factor in the Mediterranean region, first reported in 1964 in Morocco. We report a case of 23-year-old man with a scarring alopecia who presented with subcutaneous abscesses topped off with a large ringworm infection. Mycotic analysis revealed a Trichophyton Rubrum deep dermatophytosis. The molecular study revealed a CARD9 mutation confirming dermatophytosis with parotid and

[Fifty years' retrospection of systemic study on salivary gland tumors].

Salivary gland tumors are a group of the most common tumors in the oral and maxillofacial region with obvious characteristics of clinical, histopathologic, and therapeutic aspects. During the past 50 years, our research group performed a systemic study on salivary gland tumors including imaging diagnosis, fine needle aspiration cytology, FT-mid-IR fiber optics spectroscopy, histopathologic features and biological behaviour of the tumors, functional parotid surgery, and (125)I seed implant branchytherapy. More reasonable principles of management for various types of the tumors were put forward

Basal cell carcinoma metastatic to the parotid: report of a new case and review of the literature.

Basal cell carcinoma is the most common of the cutaneous malignancies, accounting for 65 to 75% of all skin cancers. The natural history of this disease is one of chronic local invasion. Metastatic basal cell carcinoma is a rare clinical entity, with a reported incidence of only 0.0028 to 0.5%. Approximately 85% of all metastatic basal cell carcinomas arise in the head and neck region. We present a case of basal cell carcinoma that spread to the parotid gland in a man who had multiple lesions on his scalp and face. We also review the literature on metastatic basal cell carcinoma of the head an

Absorbed doses with intraoral radiography. Function of various technical parameters.

Both the maxillary incisor and mandibular molar regions of a Rando phantom were radiographed using specific technical parameters. The parotid gland and thyroid gland doses were measured using thermoluminescence dosimetry. Tube voltage, total filtration, mAs, collimation, and cone length were varied while keeping the other factors constant. Increased tube voltage or total filtration generally resulted in a slightly increased absorbed dose. Absorbed dose was decreased as the beam was more tightly collimated or the cone length was increased.

NUT carcinoma of the head and neck: A case report and literature review.

Nuclear protein in testis (NUT) carcinoma is a rare and highly aggressive cancer, characterized by rearrangements involving the NUT gene located on chromosome 15q14. In this report, we present the case of a 52-year-old female diagnosed with primary parotid NUT carcinoma. Despite undergoing surgery, adjuvant chemotherapy, and incomplete regional radiotherapy, the patient succumbed to the disease after an overall survival duration of 7 months. We retrospectively discuss patient clinical and pathological features, as well as therapeutic approaches of NUT carcinoma of the head and neck.

A Giant Schwannoma Extending from Medial Portion of Middle Cranial Fossa to Parapharyngeal Space and Deep Parotid Space.

Trigeminal schwannomas are rare tumours comprising 0.2% of all intracranial tumours and 0.5% of all head and neck tumours. Patients with trigeminal schwannomas presented with facial hypoesthesia and pain. We presented a case with left bulging oropharynx. The CT scan showed a 3.8x2.6x4.9cm left parapharyngeal tumour compressed to the oropharynx and middle cranial fossa. We performed 3 ways in two times of operation to excise the whole tumour. We chose the transoral approach for parapharyngeal space, trans-parotid approach for deep parotid part and the endoscopic endonasal trans-pterygoid approa

Regional Responses in Radiation-Induced Normal Tissue Damage.

Normal tissue side effects remain a major concern in radiotherapy. The improved precision of radiation dose delivery of recent technological developments in radiotherapy has the potential to reduce the radiation dose to organ regions that contribute the most to the development of side effects. This review discusses the contribution of regional variation in radiation responses in several organs. In the brain, various regions were found to contribute to radiation-induced neurocognitive dysfunction. In the parotid gland, the region containing the major ducts was found to be critical in hyposaliva

Gross failure rates and failure modes for a commercial AI-based auto-segmentation algorithm in head and neck cancer patients.

PURPOSE: Artificial intelligence (AI) based commercial software can be used to automatically delineate organs at risk (OAR), with potential for efficiency savings in the radiotherapy treatment planning pathway, and reduction of inter- and intra-observer variability. There has been little research investigating gross failure rates and failure modes of such systems. METHOD: 50 head and neck (H&N) patient data sets with "gold standard" contours were compared to AI-generated contours to produce expected mean and standard deviation values for the Dice Similarity Coefficient (DSC), for four common

A parotid abscess out of control resulting in craniocervical necrotising fasciitis in the context of diabetes mellitus-a case report and review of the literature.

BACKGROUND: Necrotising fasciitis is an aggressive life-threatening infective process rarely making an appearance in the head and neck region and its development secondary to parotid abscess is exceptionally rare and scarcely reported in the literature. This case report serves to guide otolaryngologists with respect to its recognition and offers an alternative approach to craniocervical necrotising fasciitis with multiple neck explorations, use of antimicrobial impregnated packing enabling delayed reconstruction with lower morbidity. CASE DESCRIPTION: A 76-year-old female with a body mass ind

Studies on kallikrein in the duct systems of the salivary glands of the cat.

By correlating immunofluorescence light microscopy with electron microscope studies and with kallikrein concentrations under various conditions, we have made the following observations and conclusions about kallikrein in the submandibular and other salivary glands.1. In the submandibular gland, specific immunofluorescence to kallikrein was observed in the luminal region of the striated ducts particularly, but also in the outer epithelial cells of the stratified epithelial collecting ducts. Sympathetic nerve stimulation resulted in a reduction in intensity of specific fluorescence and in its in

A Method for Sensitivity Analysis of Automatic Contouring Algorithms Across Different MRI Contrast Weightings Using SyntheticMR.

BACKGROUND: Currently, a majority of institution-specific automatic MRI-based contouring algorithms are trained, tested, and validated on one contrast weighting (i.e., T2-weighted), however their actual performance within this contrast weighting (i.e., across different repetition times, TR, and echo times, TE) is under-investigated and poorly understood. As a result, external institutions with different scan protocols for the same contrast weighting may experience sub-optimal performance. PURPOSE: The purpose of this study was to develop a method to evaluate the robustness of automatic contou

Apparent Diffusion Coefficient Map-Based Radiomics Features for Differential Diagnosis of Pleomorphic Adenomas and Warthin Tumors From Malignant Tumors.

PURPOSE: The magnetic resonance imaging (MRI) findings may overlap due to the complex content of parotid gland tumors and the differentiation level of malignant tumor (MT); consequently, patients may undergo diagnostic lobectomy. This study assessed whether radiomics features could noninvasively stratify parotid gland tumors accurately based on apparent diffusion coefficient (ADC) maps. METHODS: This study examined diffusion-weighted imaging (DWI) obtained with echo planar imaging sequences. Eighty-eight benign tumors (BTs) [54 pleomorphic adenomas (PAs) and 34 Warthin tumors (WTs)] and 42 MT

Management to prevent high-sited recurrence of pharyngo-laryngeal cancers.

The development of reconstructional techniques in cervico-facial surgery has led to the proposal of surgical solutions, in conjunction with radiotherapy, for the extensive epidermoid carcinomas of the oropharynx and hypopharynx that were formerly rejected by the surgeon. However these stage III or IV epidermoid carcinomas carry a heavy proportion of "high" recurrences, i.e. situated in the parapharyngeal, oropharyngeal and rhinopharyngeal regions. The reasons for this are: the lymphatic continuity between the oro- and hypopharynx and the parapharyngeal regions and the base of the skull; the li

Survival outcomes of patients treated with hypofractionated stereotactic body radiation therapy for parotid gland tumors: a retrospective analysis.

BACKGROUND: to review a single-institution experience with the management of parotid malignancies treated by fractionated stereotactic body radiosurgery (SBRT). FINDINGS: Between 2003 and 2011, 13 patients diagnosed with parotid malignancies were treated with adjuvant or definitive SBRT to a median dose of 33 Gy (range 25-40 Gy). There were 11 male and two female patients with a median age of 80. Ten patients declined conventional radiation treatment and three patients had received prior unrelated radiation therapy to neighboring structures with unavailable radiation records. Six patients wer

A cadaveric study of auricular lymphatics and implications for sentinel lymph node biopsy.

Malignant cutaneous tumors of the auricle are known to have a high rate of spread to the regional lymph nodes, and, for this reason, removal of the lymph nodes, for diagnostic or therapeutic purposes, is often required. Recent experience with sentinel node biopsy in cutaneous tumors of the head and neck has questioned the traditional lymphatic pathways and prompted a new study. Lymphatic pathways from the auricle were demonstrated by India ink injection of five auricles in three cadavers followed by block dissection and Spalteholz clearing of en bloc specimens. Lymphatics descend adjacent to t

High-risk cutaneous squamous cell carcinoma of the head and neck.

Nonmelanoma skin cancers (squamous cell and basal cell carcinomas) occur at an epidemic rate in many countries with the worldwide incidence increasing. The sun-exposed head and neck are the most frequent sites for these cancers to arise and in most patients diagnosed with a cutaneous squamous cell carcinoma, local treatment is usually curative. However, a subset is diagnosed with a high-risk cutaneous squamous cell carcinoma. High-risk factors include size (>2 cm), thickness/depth of invasion (>4 mm), recurrent lesions, the presence of perineural invasion, location near the parotid gland, and
