Patellofemoral arthroplasty and chondrocalcinosis: a twenty year follow-up.

PURPOSE: Calcium pyrophosphate dihydrate (CPPD) deposition, often visible radiographically as chondrocalcinosis, frequently accompanies knee … teoarthritis progression (6 knees), patellar malalignment (7 knees), one implant loosening, and one patella fracture. The 20-year survival rates were 87% for knees without chondrocalcinosis and 90% for those with chondrocalcinosis, with no significant difference in revision timing between groups (log-rank p = 0.64). CONCLUSION: Although chondrocalcinosis became m …

Patellofemoral instability part 2 (Bony procedure for patellar surgical stabilization): State of the art.

Surgery for patellofemoral instability is usually considered in patients with recurrent patellar dislocation and after a first-time patellar dislocation in the presence of either an associated osteochondral fracture or high risk of recurrence due to the presence of several risk factors. Risk factors include demographics such as age, contralateral dislocation, as well as anatomic risk factors (ARF) such as abnormal coronal and rotational alignment, trochlear dysplasia, lateral quadriceps vector, and patella alta. Surgery with soft tissue procedures includes restoring the medial patellar restrai

Polyostotic fibrous dysplasia (McCune-Albright) with rare multiple epiphyseal lesions in association with aneurysmal bone cyst and pathologic fracture.

Fibrous dysplasia, including McCune-Albright syndrome, is a genetic, non-inheritable benign bone disorder that may involve a single or multiple bone, typically occurring in the diaphysis or the metaphysis of long bones. In very rare instances polyostotic fibrous dysplasia present involvement of the epiphysis in long bones. Aneurysmal bone cysts are benign, expansile, lytic bone lesions formed by cystic cavities containing blood, that may occur de novo or secondary to other lesions of bone, including fibrous dysplasia. We report a case of an 18-year-old female with polyostotic fibrous dysplasia

Anterior tibial artery occlusion post total knee arthroplasty: A case report.

RATIONALE: Total knee arthroplasty (TKA) is a globally well-accepted surgery because of its good outcome and safety. Although TKA-associated arterial injuries occur, postoperative anterior tibial artery (ATA) occlusion has not been reported yet. Herein, we present a case of ATA occlusion after TKA. PATIENT CONCERNS: A 42-year-old man with a 6-year history of right knee pain after right patella fracture treated with open reduction was referred to our clinic. Valgus and contracture deformities were detected in the right knee. Severe osteoarthritis (OA) in the right knee and multiple osteochondr…

The Meniscus-Deficient Knee: Biomechanics, Evaluation, and Treatment Options.

Meniscal tears are the most common knee injury, and partial meniscectomies are the most common orthopaedic surgical procedure. The injured meniscus has an impaired ability to distribute load and resist tibial translation. Partial or complete loss of the meniscus promotes early development of chondromalacia and osteoarthritis. The primary goal of treatment for meniscus-deficient knees is to provide symptomatic relief, ideally to delay advanced joint space narrowing, and ultimately, joint replacement. Surgical treatments, including meniscal allograft transplantation (MAT), high tibial osteotomy

First-time traumatic patellar dislocation: a systematic review.

Acute patellar dislocations can result in patellar instability, pain, recurrent dislocations, decreased level of sporting activity, and patellofemoral arthritis. The initial management of a first-time traumatic patellar dislocation is controversial with no evidence-based consensus to guide decision making. Most first-time traumatic patellar dislocations have been traditionally treated nonoperatively; however, there has been a recent trend in initial surgical management. We performed a systematic review of Level I-IV studies to make evidence-based medicine recommendations on how a clinician sho

Nonoperative Management of Minimally Displaced Patellar Sleeve Fractures.

Patellar sleeve fractures primarily occur in the adolescent population from a rapid contraction of the quadriceps with the knee in a flexed position. Several small case reports describe operative reduction and fixation for displaced fractures. However, there is sparse literature on nonoperative management of these injuries. Retrospective review and prospective follow-up of all patients diagnosed with patellar sleeve fracture between 1991 and 2014 at a single institution. Patients with superior pole avulsion fractures, history of prior knee surgery, and fractures managed operatively were exclud

Combination of a miniplate with tension band wiring for inferior patellar pole avulsion fractures.

BACKGROUND: The tension-band technique, with Kirschner wires or cannulated screws, is one of the most commonly used fixation techniques for patellar fractures. However, it may be not useful for inferior patellar pole fractures as it may lead to fragment reduction loss and fixation failure. We present a surgical technique that combines a miniplate with tension band wiring for inferior patellar pole avulsion fractures and report surgical outcomes. PATIENTS AND METHODS: Between June 2012 and May 2016, 17 consecutive patients (mean age: 51 years) with inferior patellar pole fractures (AO/OTA 34-A

Evaluation of an Active Rehabilitation Program With Early Weightbearing and No Immobilization After Tibial Tubercle Distalization.

BACKGROUND: Abnormal patellar height (patella alta) has been reported to be one of the main predisposing factors for recurrent patellar dislocation, and it can be surgically corrected by distalizing tibial tubercle osteotomy (DTTO). Rehabilitation after DTTO often includes limitations on weightbearing and restrictions on knee range of motion by means of bracing, increasing the risk of slow progression of the rehabilitation. HYPOTHESIS: An active rehabilitation program with no restrictions on weightbearing and range of movement would yield a low risk of postoperative complications and a fast r

Knee osteochondral fractures in skeletally immature patients: French multicenter study.

INTRODUCTION: Femoral or patellar osteochondral fractures complicating patellar dislocation are more frequently observed in adolescents than in adults. These incidental lesions of vulnerable joint cartilage are often neglected in the initial phase, which is regrettable given their good capacity for healing after repositioning. The objective of this study was to investigate the characteristics and analyze the results of repositioning these fractures in skeletally immature patients. MATERIAL AND METHODS: This retrospective multicenter study grouped 14 patients and knees (seven females). The mea

Acute patellar dislocation in children and adolescents. Surgical technique.

BACKGROUND: The treatment of acute patellar dislocation in children is controversial. Some investigators have advocated early repair of the medial structures, whereas others have treated this injury nonoperatively. The present report describes the long-term subjective and functional results of a randomized controlled trial of nonoperative and operative treatment of primary acute patellar dislocation in children less than sixteen years of age. METHODS: The data were gathered prospectively on a cohort of seventy-four acute patellar dislocations in seventy-one patients (fifty-one girls and twent

Progress in the treatment of knee osteoarthritis with high tibial osteotomy: a systematic review.

BACKGROUND: High tibial osteotomy (HTO) has been used for over 60 years in clinical practice and mainly comprises two major techniques: closed wedge high tibial osteotomy (CWHTO) and open wedge high tibial osteotomy (OWHTO). However, these have been gradually replaced by total knee arthroplasty (TKA), due to inconsistent clinical results and many complications. With the concept of knee-protection and ladder treatment of osteoarthritis, as an effective minimally invasive treatment for knee osteoarthritis, HTO has once again received attention. METHODS: A systematic literature search was conduc

Ultrasound Detection of Patellar Fracture and Evaluation of the Knee Extensor Mechanism in the Emergency Department.

Traumatic injuries to the knee are common in emergency medicine. Bedside ultrasound (US) has benefits in the rapid initial detection of injuries to the patella. In addition, US can also quickly detect injuries to the entire knee extensor mechanism, including the quadriceps tendon and inferior patellar ligament, which may be difficult to diagnose with plain radiographs. While magnetic resonance imaging remains the gold standard for diagnostic evaluation of the knee extensor mechanism, this can be difficult to obtain from the emergency department. Clinicians caring for patients with orthopedic i

Knee function after patellectomy. A 12- to 48-year follow-up.

We describe 83 knees (69 patients) which had had patellectomy for anterior knee pain (52), patellofemoral osteoarthritis (25) or comminuted fractures (6) between 1942 and 1978. The patients were questioned about their symptoms and the function of the operated knee 14 to 50 years after operation. In the group with anterior knee pain, 76% achieved good results and were satisfied with the operation. Only 54% of the osteoarthritis group had satisfactory relief of pain and most had progressive deterioration of function. Sixteen patients who had had unilateral patellectomy were assessed by dynamomet

Transposition of the sartorius muscle for reconstruction of the extensor apparatus of the knee.

A case of transposition of the sartorius muscle is presented. This method was successfully used for treatment of an exposed knee joint with subtotal loss of the extensor apparatus of the knee. A proximally based muscle flap proved very efficient for coverage of the knee joint and restoration of function.

Patellectomy after fracture. Long-term follow-up results with special reference to functional disability.

Twenty-eight out of a group of 38 patients, who had undergone a patellectomy during the period 1950-70, were investigated clinically and radiologically. The results were compared with the subjective symptoms of the patients to see if there was a correlation. The follow-up period was 7.4 years on average. A good subjective result was reported by only six patients. The predominant subjective symptoms were weakness of the limb and pain on movement and/or exertion. The most usual findings were atrophy of the quadriceps muscle, crepitation, and palpation tenderness. The muscular power of the quadri

Isolated and early-onset cerebral fat embolism syndrome in a multiply injured patient: a rare case.

BACKGROUND: Fat embolism syndrome (FES) is a rare complication that can occur between 12 and 72 h after the initial insult. Isolated cerebral FES without pulmonary symptoms is rarer. Early fracture fixation might prevent FES. We report a case of multiple-fracture with FES despite definite fixation three hours post-injury. CASE PRESENTATION: A 54-year-old man presented with multiple fractures: left femoral shaft (AO B2), left distal radius (AO C3), left comminuted patella, right comminuted 1st metatarsal base and left 2nd-4th metatarsal neck. Because he was stable, we gave him early total care

Extra-articular patellar dislocation with vertical axis rotation.

Extra-articular vertical dislocation is very rare. It occurs when the patella is forced crossing the lateral femoral trochlear surface, and rotated simultaneously along its longitudinal axis at about 90°, resulting in a wedged dislocation against lateral condyle. Reduction has been reported to require general anesthesia and an open procedure. We report a 19-year-old boy with extra-articular patellar dislocation, and a successful reduction was done by manipulation under local anesthesia. We believe that the mechanism of lateral vertical dislocation is the fulcrum mechanism.
