Resolution of a Cystic Endodontic-Periodontal Lesion Utilizing Combined Apicoectomy and Guided Tissue Regeneration: A Case Report.

INTRODUCTION: Endodontic-periodontal combined lesions pose a treatment challenge for clinicians, especially when the lesion is persistent and does not show signs of healing after initial endodontic treatment. This case report describes interdisciplinary management of a non-healing combined endodontic-periodontal lesion related to a peri-apical cyst, through a combined approach of apicoectomy and guided tissue regeneration (GTR) with enamel matrix derivative (EMD). Most documented cases have been treated in a sequential manner, with minimal literature present on the effects of a combined approa

Rapid canine retraction through distraction of the periodontal ligament.

The process of osteogenesis in the periodontal ligament during orthodontic tooth movement is similar to the osteogenesis in the midpalatal suture during rapid palatal expansion. A new concept of "distracting the periodontal ligament" is proposed to elicit rapid canine retraction in 3 weeks. It is called dental distraction. Fifteen orthodontic patients (26 canines, including 15 uppers and 11 lowers) who needed canine retraction and first premolar extraction were included. At the time of first premolar extraction, the interseptal bone distal to the canine was undermined with a bone bur, grooving

Sirtuin 1 regulates matrix metalloproteinase-13 expression induced by Porphyromonas endodontalis lipopolysaccharide via targeting nuclear factor-κB in osteoblasts.

Porphyromonas endodontalis lipopolysaccharide (P.e LPS) is an important initiating factor for periapical inflammation and bone destruction. Matrix metalloproteinase-13 (MMP-13) has been shown to participate in the formation and diffusion of periapical bone lesion in chronic apical periodontitis. Sirtuin 1 (SIRT1) is a key regulator of inflammation in mammalian cells which suppresses the release of inflammatory mediators. This study aimed to explore the role of SIRT1 in regulating MMP-13 expression induced by P.e LPS in osteoblasts. P.e LPS stimulated MMP-13 expression in MC3T3-E1 cells. Knockd

High prevalence of radiolucent periapical lesions amongst patients with inherited coagulation disorders.

Apical periodontitis (AP) is an inflammatory lesion around the apex of a tooth caused by bacterial infection of the pulp canal system. AP appears radiographically as a radiolucent periapical lesion (RPL). The elective treatment for teeth with AP is root canal treatment (RCT). No study is available about the frequency of RPL and RCT in patients with inherited coagulation disorders (ICD). The aim of this study was to investigate the prevalence of RPL and RCT in patients with ICD and control subjects. In a cross-sectional study, the radiographic records of 58 patients with haemophilia A, haemophi

Alveolar bone-loss area localization in periodontitis radiographs based on threshold segmentation with a hybrid feature fused of intensity and the H-value of fractional Brownian motion model.

BACKGROUND AND OBJECTIVE: Periodontitis involves progressive loss of alveolar bone around the teeth. Hence, automatic alveolar bone-loss (ABL) measurement in periapical radiographs can assist dentists in diagnosing such disease. In this paper, we propose an effective method for ABL area localization and denote it as ABLIfBm. METHOD: ABLIfBm is a threshold segmentation method that uses a hybrid feature fused of both intensity and texture measured by the H-value of fractional Brownian motion (fBm) model, where the H-value is the Hurst coefficient in the expectation function of a fBm curve (inte

Detection of bacteria in endodontic samples by polymerase chain reaction assays and association with defined clinical signs in Italian patients.

BACKGROUND/AIMS: The presence of selected bacteria (Enterococcus faecalis, Porphyromonas gingivalis, Prevotella intermedia, Tannerella forsythensis, Treponema denticola) in infected root canals was studied using polymerase chain reaction (PCR) assays, and the association of bacteria with clinical signs of endodontic disease was assessed. The null hypothesis, that no difference could be observed between clinical signs of apical periodontitis and a specific bacterial strain, was tested. METHODS: Microbial samples were obtained from 62 teeth in 54 patients with endodontic disease. For each tooth

Ferroptosis in endodontic pathobiology: Emerging evidence, mechanistic rationale, and translational uncertainties.

INTRODUCTION: Ferroptosis is an iron-dependent form of regulated cell death characterized by lipid peroxidation and impaired antioxidant defense. Its defining biochemical features, including iron-redox imbalance and lipid peroxide accumulation, overlap with biological conditions present in infected, hypoxic, and cytokine-rich endodontic tissues. This narrative review examined the potential role of ferroptosis in endodontic pathobiology, with emphasis on separating direct endodontic evidence from indirect or extrapolated oral evidence. METHODS: Electronic searches were performed in PubMed, Sco

Microsurgical management of radicular cyst using guided tissue regeneration technique: A case report.

BACKGROUND: Radicular cyst is a lesion of odontogenic origin that arises from epithelial remains due to periapical periodontitis caused by inflammatory reactions generated at the apex of affected teeth with infected or necrotic pulps. The therapeutic management of radicular cysts is controversial. There is only one case report of enucleation of a radicular cyst managed with microsurgery and apicoectomy, but without the use of the guided tissue regeneration (GTR) technique in the same surgical procedure. The present clinical case describes the management of a radicular cyst with microsurgical a…

Management of infected radicular cyst associated with immature maxillary permanent lateral incisor: a conservative surgical approach.

Radicular cysts are the most common cystic lesions which affect the jaw. Traumatic dental injuries cause injury to the periodontal ligament and dental pulp often leads to pulpal necrosis. The necrosed pulp eventually becomes the nidus of infection and irritates the periapical epithelial cell remnants, which in turn develops into a cyst eventually. This case report presents the successful conservative surgical management of a large infected radicular cyst which was associated with traumatised, necrotic, permanent maxillary lateral incisor with open apex by Partsch II surgical procedure followed

A case of bacteremia caused by Dialister pneumosintes and Slackia exigua in a patient with periapical abscess.

Dialister pneumosintes and Slackia exigua are both obligatory anaerobe and known to be associated with periodontal diseases and other oral infection. We report a case of blood stream infection caused by D. pneumosintes and S. exigua. This occurred in a 78-year-old female patient that presented with general weakness and fever. We revealed that she had a periapical abscess. The blood culture was positive for D. pneumosintes and S. exigua; however, identifying them was challenging. Ultimately, 16S rRNA sequencing was used to identify the organisms. The patient recovered after being treated with c

Application of fluorescence microscopy on hematoxylin and eosin-stained sections of healthy and diseased teeth and supporting structures.

Destruction of dental tissue and supporting structures is usually microscopically assessed by routine hematoxylin and eosin (HE)-stained sections. This short communication is concerned with the potential role of fluorescence microscopy of HE-stained sections to study morphological aspects of intact and pathological teeth in dental research. This methodology improves the visualization of the anatomical structures of the intact teeth, especially anatomical features and periodontal ligament spatial distribution. This technique also improves the visualization of the root and bone resorption and th
