Butorphanol tartrate: an intravenous analgesic for outpatient surgery.

Many surgical procedures in otolaryngology, particularly those in the areas of facial plastic surgery, are best performed under a local anesthetic. The majority of these procedures may be done on an outpatient basis and require an intravenous analgesic of some form. This analgesic must be safe, predictable, easy to administer, have a rapid onset, and provide good amnesia. A short recovery period is imperative. Diazepam has been widely used in the outpatient setting, since it generally fulfills the requirements listed. However, the recovery time is often prolonged and unpleasant for the patient

The Prevalence of Psychiatric Disorders Among Elective Plastic Surgery Patients.

Purpose: Psychiatric disorder is one of the predictors of poor outcome in cosmetic plastic surgery patients. A US study in 1960 showed that 72.4% of 98 cosmetic plastic surgery patients had a psychiatric disorder. In our study, we predict that the prevalence of psychiatric disorders will be statistically significant among patients seeking elective plastic surgery in comparison with the general US population. Methods: We conducted a retrospective review study of 1000 adult patients seeking elective plastic surgery at The University of Kansas Medical Center Plastic Surgery Department from 2011 t

Robotic Multiport Buccal Mucosa Graft Ureteroplasty: Tips and Tricks.

Robotic buccal mucosa graft ureteroplasty (RU-BMG) has become a cornerstone in the armamentarium of robotic ureteral reconstruction. Previously, long-segment strictures of the proximal and mid-ureter were relegated to aggressive reconstructive options such as ileal ureter substitution and renal autotransplantation. RU-BMG has emerged as an option for repair with comparatively minimal morbidity and excellent success rates. We herein present a detailed description of our pre- and postoperative considerations, operative techniques, and tips and tricks. The accompanying video provides multiple exa

Periodontal procedures to enhance esthetics.

Dentistry today can provide excellent esthetics for the patient. Periodontal procedures have transcended the era where the treatment and scope are limited to pocket reduction and disease control. Periodontal plastic surgery will prove to be an excellent adjunct when properly applied in conjunction to elective esthetic dentistry.

"Botched": A Case Report of Silicone Embolism Syndrome After Penile and Scrotal Injection.

INTRODUCTION: Silicone has been commonly used for both major and minor plastic and reconstructive surgery for decades. Due to the high costs associated with minor cosmetic procedures and plastic surgery, the unauthorized use of silicone injections by laypersons has become increasingly common. Improper or illegal subcutaneous injectable silicone has caused significant pulmonary complications and neurological complications, which can range from mild chest pain, hypoxia, and respiratory failure to coma and altered mental status. CASE REPORT: We present a patient who had a rare complication of re

A Review of Hand Neuroanatomy, Anesthesia, and Bedside Blockade.

Many hand-related pathologies can be appropriately managed at the bedside without operative escalation, thereby reducing resource allocation, healthcare costs, and exposure to general anesthesia. Plastic surgery trainees have variable experience and knowledge in managing hand pathologies outside the operating room. A comprehensive clinical reference for hand anesthesia has yet to be seen in the current plastic surgery literature. This review aims to guide the safe and effective administration of regional anesthesia in the hand and wrist for bedside procedures. This review was guided by clinica

Nasal Bone Fractures: Differences Amongst Sub-Specialty Consultants.

BACKGROUND: Nasal fractures constitute the largest proportion of facial trauma each year, however, there is no consensus management. In this study, we investingated the role of the consultant and the functional and aesthetic outcomes of procedures performed to address nasal bone fractures. METHODS: A retrospective chart review of patients who sustained nasal bone fractures was conducted from 8/1/14 through 1/23/18. Categorical variables were analyzed using chi-squared testing and Fisher's exact test, where appropriate, while continuous variables were compared using Mann-Whitney U testing. RE

Oral and maxillofacial surgery: a new wrinkle.

A number of facial altering surgical procedures have been presented. Several areas are important to note. Most of these procedures were devised by practitioners lost to antiquity and individuals in a plethora of different fields, including orthopedics, dermatology, oral surgery, plastic surgery, and otolaryngology. Just as in the other specialties that perform these procedures, not all surgeons of the specialties perform the entire scope of their specialty. However, dentistry now has the capability to change the jaws, soft tissue drape of the face, neck, nose, and eyes, as well as smile lines

Perioperative Hypothermia in Medium- and Large-Volume Plastic Surgery: Clinical and Surgical Implications and Practical Prevention Strategies.

Inadvertent perioperative hypothermia (IPH), defined as a core temperature below 36 °C, is a common complication of elective procedures under general anesthesia and has traditionally been treated as an anesthetic concern. In medium- and large-volume plastic surgery, including abdominoplasty, large-volume liposuction, combined and postbariatric body contouring, and autologous and microsurgical reconstruction, wide body-surface exposure, cold tumescent solution, prolonged operative time, and combined anesthesia amplify this risk and make it directly relevant to surgical outcomes. This narrative

Glucagon-Like Peptide-1 Agonists: A Practical Overview for Plastic and Reconstructive Surgeons.

BACKGROUND: Glucagon-like peptide-1 (GLP-1) agonists, such as exenatide, liraglutide, dulaglutide, semaglutide, and tirzepatide, effectively manage type 2 diabetes by promoting insulin release, suppressing glucagon secretion, and enhancing glucose metabolism. They also aid weight reduction and cardiovascular health, potentially broadening their therapeutic scope. In plastic surgery, they hold promise for perioperative weight management and glycemic control, potentially impacting surgical outcomes. METHODS: A comprehensive review was conducted to assess GLP-1 agonists' utilization in plastic s

The Burden of Plastic Surgery in Rural Kenya: The Kapsowar Hospital Experience.

PURPOSE: Both governmental and nongovernmental training programs are expanding efforts to train the next generation of plastic surgeons who will work in low- and middle-income countries (LMICs). Sufficient training is dependent on acquiring the appropriate skillset for these contexts. Few studies have characterized the spectrum of practice of plastic surgeons in LMICs and their relative disparity. METHODS: We performed a retrospective review on all patients who received plastic surgery at a single institution in rural western Kenya from 2021 to 2023. Data such as diagnoses, procedures, and ho
