A prospective study of 12 patients with the diagnosis of polymyositis or dermatomyosiois was initiated to determine the characteristics of the esophageal manometric patterns in this group. All of the patients were women between the ages of 30 to 80 years and met three of four of Bohan's diagnostic criteria for these diseases. Biopsy material from the muscles of 40 patients with either polymyositis or dermatomyositis was examined with the immunoperoxidase method to compare the distribution of immunoglobulin (Ig)-containing cells in the inflammatory infiltrates and elsewhere in the muscle tissue. A 2-year study was undertaken which identified 60 patients who had an abnormal electromyogram (EMG), muscle biopsy, and/or clinical examination suggestive of myopathy. The clinical records were reviewed, and 12 patients were found to fulfill the criteria of Bohan and Peter for definite diagnosis of polymyositis or dermatomyositis. A controlled, prospective, double-blind, therapeutic trial of azathioprine was conducted in the initial therapy of polymyositis. Sixteen patients received 60 mg prednisone per day plus either azathioprine (2 mg/kg of body weight per day) or placebo for a period of 3 months. Creatine phosphokinase (CPK) levels fell to normal slightly sooner in the placebo group, but not significantly so. A case of the nodular form of polymyositis is reported in which muscle necrosis and inflammation, and fibrosis of subcutaneous fat were observed. Although the patient had a short history, the presence of extensive fibrosis with acute lesions indicated that the disease had been present for some time. Type-specific antibodies to the polymorphic types of collagen have been used to study their distribution in polymyositis. Dramatic increases were observed in the staining with antibodies to Type III collagen and, to a lesser extent, with antibodies to Types I, IV, and V. The changes in capillary and endomysial basement membrane collagens (Types IV and V) as well as the proliferation of perimysial Type III collagen are discussed in relation to the muscle atrophy observed during the disease.

The "tumoral" calcifications seen with polymyositis and dermatomyositis are frequently large and painful. Many surgeons believe such calcifications should not be removed surgically, arguing that the surgical trauma induces further calcification that is probably ever more severe. However, we believe that surgical removal of these large masses is less traumatic than cyclical, spontaneous extrusion of the calcium with secondary healing. We present 2 patients in whom chronically draining and painful dystrophic calcifications were treated surgically without recurrence.

Muscle breakdown and repair were measured by metabolic balance techniques in a patient with polymyositis who was being treated with prednisolone and azathioprine. Changes in body muscle mass that had been estimated from nitrogen and phosphorus balances correlated with antropometric assessments of thigh muscle mass and quadriceps strength. A high-passage laboratory strain of coxsackievirus B1 produced a unique myositis that predominantly and profoundly affected hip extensors and, to a lesser extent, hindquarter knee flexors when inoculated into a strain of random-bred newborn mice of Swiss origin (COH mice). Muscle biopsy specimens of a patient with polymyositis showed crystalline structures resembling picornavirus aggregates within muscle cells. The patient's serum reacted in an indirect immunofluorescence assay with autologous muscle cells. A strongly positive immunofluorescence staining was also noted when a section of muscle tissue was reacted with coxsackievirus A9 antiserum, and a weakly positive reaction was noted with coxsackievirus B2 antiserum. No staining was observed after treatment with antiserum to poliovirus type 1 or echovirus types 11 and 22.

Radioimmunoassay of serum myoglobin was done in 53 patients with polymyositis syndromes and other conditions. Serum myoglobin values in 33 healthy subjects ranged from 4 to 77 [mean 33.3 +/- 19.8 (SD)] ng/ml. Fifty percent of polymyositis patients had elevated serum myoglobin levels (greater than 80 ng/ml). Serum myoglobin values in polymyositis patients fluctuated more sensitively than CPK and GOT. Combined estimation of myoglobin and CPK offers advantages for the detection of muscle injury and the prediction of disease exacerbation.
