[Small brain infarcts in arterial hypertension and atherosclerosis (pathogenesis and criteria of morphologic diagnosis].

Small infarcts, few millimeters in size, localized in the deep regions of brain hemispheres, cerebellum and pons were found in 36 patients with arterial hypertension and atherosclerosis. These infarcts develop in the intracerebral arteries having the diameter up to 500 microns. Two types of these infarcts are distinguished: hypertensive and atherosclerotic. Arterial stenosis and obliteration are at the basis of hypertensive infarcts. Reduction of circulation due to the atherosclerotic stenosis is the cause of the atherosclerotic infarcts.

Brainstem cysticercosis simulating cystic tumor lesion. A case report.

The authors report the case of a 37 year-old man with a solitary cysticercus cyst in the brainstem (pons) successfully removed through a suboccipital craniectomy. Surgery in neurocysticercosis has been indicated in patients with hydrocephalus and/or large cystic lesions. Cystic lesions in the brainstem and spinal cord may have indication for surgery for two reasons: (1) diagnosis; and (2) treatment. Aspects related to differential diagnosis and therapeutic alternatives are discussed.

Dorsolateral pontine hemorrhage producing pure sensory stroke.

Pure sensory stroke predominantly results from lacunar infarction in the posteroventral nucleus of the thalamus. We report the history of a young nonhypertensive male presenting with pure sensory stroke due to hemorrhage of a small arteriovenous malformation in the dorsolateral part of the pons. In contrast to the extent of the lesion, only minor degree of objective sensory loss was observed. Furthermore, our observation is unique in that it is the first presentation of this syndrome with that particular location.

fMRI reveals brainstem and cortical networks contribute to the instruction-dependent modulation of stretch-evoked motor responses.

Reliable noninvasive measurement of human brainstem activity during motor control remains challenging due to small anatomical structures and physiological noise, yet it is essential for understanding descending contributions to rapid feedback control. We used brainstem-optimized whole-brain functional magnetic resonance imaging (fMRI) to examine whether task-dependent modulation of stretch-evoked motor responses in humans is associated with changes in activation within reticulospinal regions of the human brainstem. A behavioral validation experiment (N = 10), in which participants were instruc…

Pentylenetetrazole-Induced Seizures in Wistar Male Albino Rats with Reference to Glutamate Metabolism.

BACKGROUND AND PURPOSE: Epilepsy is a common and heterogenous neurological disorder characterized by recurrent spontaneous seizures. Animal models like rats play a crucial role in finding of mechanism of epilepsy in different brain regions. i.e., cerebral cortex, cerebellum, hippocampus, and pons medulla. Glutamate is an important excitatory neurotransmitter in the central nervous system and also glutamate plays a vital role in neuronal development and memory. The process of neuronal death evolved by glutamate receptor activation, has been hypothesized in both acute and chronic degenerative di…

[A patient with multiple sclerosis who developed bilateral optic neuritis and central trigeminal myelin lesion].

A 70-year-old woman was admitted to our hospital due to bilateral optic neuritis and left facial sensory disturbance that became exacerbated … The central-peripheral myelin transitional zone at the trigeminal nerve is located 1-6 mm from the pons, where central myelin changes to the peripheral myelin. This patient showed a high T2 signal at the trigeminal nerve 3 mm from the pons on MRI, suggesting the involvement of a central trigeminal myelin lesion. Findings of a central trigeminal myelin …

Paraoxonase 1 L55M, Q192R and paraoxonase 2 S311C alleles in atherothrombosis.

Increased oxidative stress is known to play a role in the pathogenesis of atherosclerosis, and polymorphisms in genes encoding for enzymes involved in modulation of oxidant stress, such as paraoxonases (PONs), provide a potentially powerful approach to study the risk of disease susceptibility. Aim of our study is to investigate the possible association among PONs polymorphisms, clinical and metabolic factors, and atherothrombotic events in an Italian population. We evaluated in 105 subjects, with or without atherosclerotic risk factors, the presence of PON1 L55M, PON1 Q192R, and PON2 S311C gen…

Increased limbic and brainstem activity during migraine attacks following olfactory stimulation.

OBJECTIVE: Migraine patients have dysfunctional cortical olfactory processing and very often report hypersensitivity and phobic symptoms to odors during acute headache attacks. However, imaging data of how the brain processes associate migraine symptoms, such as photophobia, phonophobia, or osmophobia, are rare. METHODS: The present study aimed to explore neuronal processing in response to olfactory stimulation (rose odor) in migraine patients in and outside acute headache attacks. Using event-related fMRI we studied 20 migraine patients and compared behavioral and imaging data with sex- and …

Sequential gadolinium-DTPA enhanced MRI studies in neuro-Behçet's disease.

Sequential gadolinium-DTPA (Gd-DTPA) enhanced MR images were obtained before and after steroid therapy in a case of neuro-Behçet's disease. Multiple scattered lesions, which could not be detected on pre- and post-contrast CT, were demonstrated mainly in the white matter of the pons and/or the cerebrum with both T1- and T2-weighted images. Some of these lesions, however, were not enhanced at all by infusion of Gd-DTPA. The Gd-DTPA infusion study demonstrated marked enhancement in the white matter of the pons and cerebrum. Some lesions not seen with T2-weighted images were also strongly enhanced…

Effect of thiamine deficiency on brain serotonin turnover.

Serotonin turnover has been investigated in regional brain areas of rats made thiamine deficient by pyrithiamine (PT). Following intracisternal injection of [14C]5-hydroxytryptamine ([14C]5-HT), a marked increase in the accumulation of [14C]5-hydroxyindoleacetic acid ([14C]5-HIAA) was found in the medulla-pons, hypothalamus and cerebral cortex. [14C]5-HT levels were normal in all of the brain areas except the cerebral cortex which had an increase of 58%. The ratio of [14C]5-HIAA/[14C]5-HT was significantly increased in every brain region of PT-treated rats except the cerebral cortex. Part of t…

Chronic Lymphocytic Inflammation With Pontine Perivascular Enhancement Responsive to Steroids: An Acute Presentation.

Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a rare disease with an unknown etiology which most commonly results in subacute diplopia and ataxia. Diagnosis is achieved through a triad of the following findings: lymphocytic pleocytosis with increased CD4+ T cells on cerebrospinal fluid (CSF) analysis; perivascular punctate and curvilinear hemorrhages in the pons, medulla, or cerebellum on magnetic resonance imaging (MRI) with contrast; and the cessation of symptoms after the initiation of corticosteroids. Here, we report the case of…
