Urinary excretion of D-glucaric acid in porphyria cutanea tarda.

Twenty patients with porphyria cutanea tarda who had ingested no alcohol for at least 10 days before sampling were found to possess urinary D-glucaric acid levels similar to those of 30 clinically healthy controls. Correlation between urinary excretion of D-glucaric acid on the one hand and plasma or urinary porphyrin concentrations on the other was not statistically significant. These results suggest that the high urinary concentrations of D-glucaric acid found in porphyria cutanea tarda patients by Budillon et al. (Acta Hepato-Gastroenterol. 25 (1978) 267) may have been due to recent consump

Glucose tolerance and glycosuria in Ethiopian porphyria cutanea tarda patients.

Oral glucose tolerance test (OGTT) was performed in 50 Ethiopians with proven porphyria cutanea tarda (PCT) in 30 of whom urine sugar was also checked. The result of the OGTT, interpreted according to WHO criteria, showed that 35 (70%) patients had normal OGTT, 14 (28%) impaired glucose tolerance (IGT) and 1 diabetes mellitus. Seven patients with IGT, the diabetic and one with normal OGTT had glycosuria of variable degree during the OGTT; none had fasting glycosuria. The patient with diabetes and one other with IGT later on developed mild but nonspecific symptoms. In addition, their blood gluc

A review of porphyria and cancer and the missing link with human exposure to hexachlorobenzene.

A literature review shows that porphyria cutanea tarda (PCT), and possibly acute intermittent porphyria also, tend to occur more often than expected among cases of primary liver cancer, especially when cirrhosis is present. Lymphomas and PCT may be related also, although this is less well documented. Furthermore, there is an interesting report on a case of PCT, sarcoma and probable exposure to 2,3,7,8-tetrachlorodibenzo-para-dioxin. Exposure to hexachlorobenzene (HCB) as a cause of cancer in humans does not seem to have been reported, however, but exposure to this compound and other chlorinate

Further discussion concerning porphyria cutanea tarda and TCDD exposure.

Past reports have correlated a diagnosis of porphyria cutanea tarda (PCT) with exposure to 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD). Re-examination of the literature, however, has caused us to question the conclusion of a relationship between acquired PCT and TCDD. The major considerations at issue were: (1) whether the diagnostic label of PCT was applied excessively to cases with findings that are nonspecific; (2) whether the clinical conditions suggestive of PCT were the likely results of exposure to another chemical, for example hexachlorobenzene (HCB); and (3) whether a causal link betwe

Uroporphyrinogen decarboxylase structural mutant (Gly281----Glu) in a case of porphyria.

Uroporphyrinogen decarboxylase deficiency in man is responsible for familial porphyria cutanea tarda and hepatoerythropoietic porphyria. A recent study of a family with hepatoerythropoietic porphyria showed that the enzyme defect resulted from rapid degradation of the protein in vivo. Cloning and sequencing of a complementary DNA for the mutated gene revealed that the mutation was due to the replacement of a glycine residue by a glutamic acid residue at position 281. This base change leads to a protein that is very rapidly degraded in the presence of cell lysate. Characterization of the mutati

Porphyrins and pentachlorophenol in rat-liver mitochondria in hexachlorobenzene-induced porphyria.

This study investigated the extent of impairment in functional parameters of liver mitochondria from rats treated for 60 days with hexachlorobenzene (HCB). A constant amount of mitochondrial uncoupling was found throughout the treatment period. At the same time a nearly constant amount of pentachlorophenol was detected in these mitochondria. In contrast, the level of mitochondrial porphyrins increased progressively. There was good correlation between the concentration of mitochondrial pentachlorophenol and the degree of uncoupling of oxidative phosphorylation.

Correlation of serum and urinary porphyrin levels in porphyria cutanea tarda.

A significant linear correlation was found between serum total porphyrin concentration and 24-hour total urinary porphyrin excretion in 18 patients with porphyria cutanea tarda sampled at diagnosis and during and after treatment on 73 occasions. This confirms that the serum porphyrin level parallels urinary porphyrin excretion and is an appropriate indicator of disease activity useful for monitoring patients in clinical practice.

Role of the polymorphonuclear neutrophils in the phototoxic reaction in porphyria cutanea tarda.

We have compared the superoxide production of polymorphonuclear cells (PMNLs) from healthy donors, when incubated with either control or porphyria cutanea tarda (PCT) sera at 4, 24 and 48 hours after exposure in vivo to UVA light. Serum from UVA-irradiated (1-5 J/cm2) PCT patients generated significantly greater amounts of superoxides from PMNLs than serum from UVA-irradiated (8-12 J/cm2) normal controls. This indicates that serum factors activated by porphyrin plus UVA stimulate neutrophilic granulocytes to liberate superoxides, triggering a series of events that cause tissue damage. The vasc

Low-dose oral chloroquine in the treatment of porphyria cutanea tarda.

Seven patients with porphyria cutanea tarda received a total of ten courses of low-dose oral chloroquine therapy (125 mg chloroquine phosphate twice weekly). Patients were treated for a mean 14.9 months during which time all went into clinical and biochemical remission. Relapse occurred in four patients on a total of six occasions after a mean 17 months. In four patients there was no relapse after a mean 47.3 months. There were no adverse side-effects from the treatment. Low-dose oral chloroquine therapy appears to be a safe, effective and convenient treatment for porphyria cutanea tarda, alth

Variegate porphyria and heavy metal poisoning from ingestion of "moonshine".

A patient with cavitary tuberculosis, hepatic cirrhosis, bullous skin lesions over sun-exposed surfaces, disorientation, and a chronic, as well as recent, history of illicit alcohol consumption was found to have acute variegate porphyria by characteristic fecal and urinary porphyrin studies. Elevated levels of lead and arsenic were found in serum and urine without evidence of heavy metal storage in hair and liver. We suspect that the variegate porphyria was precipitated by the ingestion of heavy metals contained in illicit alcohol. In a patient with disorientation, bullous skin lesions, and a

Decreased hepatic uroporphyrinogen decarboxylase activity in porphyria cutanea tarda.

To test whether reduced hepatic uroporphyrinogen decarboxylase activity is a specific and intrinsic defect in porphyria cutanea tarda, we measured enzymatic activity in the livers of 17 patients with porphyria cutanea tarda, 12 "normal" control patients without liver disease, and 41 patients with other forms of porphyria, alcoholic liver disease, hemochromatosis, or chronic hepatitis. Enzyme activity in all the patients with porphyria cutanea tarda was lower than in the patients without this disease, except for one patient with alcohol-induced fatty liver. Reduction of hepatic iron stores by p

Treatment of hemodialysis-related porphyria cutanea tarda with plasma exchange.

A patient with hemodialysis-related porphyria cutanea tarda was treated with plasma exchange. A rapid clinical response occurred coincidentally with a significant fall in the plasma porphyrin level. The level fell further over the following few months without additional therapeutic intervention, whereafter a slow rise occurred without recurrence of skin disease. We suggest that this form of treatment may be ideal for the patient with porphyria cutanea tarda and chronic renal failure in whom no alternative therapy is available for the cutaneous problem.

[Porphyria cutanea tarda or praematura as a symptom of the hepatotoxicity of hormonal contraception].

It is reported on an evident increase in female patients with porphyria cutanea tarda, seen in dermatological department of Halle/S. since 1975. Our female patients were much younger than patients with p.c.t. usually. 8 women, younger than 40 years in time of manifestation of disease have got contraceptive drugs for many years. Only one women under 40 didn't take contraceptive drugs, but she suffers from a thalassaemia minor. The combination of these diseases we have seen never before. P.c.t. as a follow of orale contraception should be included in calculations of the internal medicine, especi

[Hereditary porphyria cutanea in children. Enzymatic studies (author's transl)].

A girl of 7 years presenting porphyria cutanea tarda was found to have a very low level of erythrocytary uroporphyrinogen decarboxylase. Tests on her brother, parents and maternal grand-parents, who were all free of the disease revealed the same level of enzyme in the mother and grandmother, through whom the hereditary factor was obviously transmitted. This supports evidence provided by juvenile cases and familial forms in the literature that heredity plays an important part in a disease which is often considered to be acquired or symptomatic. Despite clinical and biological resemblances, two

Porphyria cutanea tarda associated with chronic renal disease and hemodialysis.

A fourth case of symptomatic porphyria associated with hemodialysis for chronic renal failure is reported. Subepidermal bullous dermatoses of patients who have undergone hemodialysis have not usually been associated with elevated porphyrin levels. However, this patient and three previously reported cases have been found to have abnormal porphyrin study results in association with skin lesions typical for porphyria cutanea tarda, occurring after hemodialysis. Hemodialysis does not effectively decrease circulating plasma uroporphyrin levels, although some dialysis of uroporphyrin into the dialys
