Preoperative sleep variability and risk of Anesthesia-related delirium in geriatric surgical patients: The mediating role of intraoperative hemodynamic instability in a single-center study.

BACKGROUND: Older adults undergoing elective surgery are at high risk of anaesthesia-related delirium. Recent evidence points to the coefficient of variation of total sleep time (CV_TST), rather than mean sleep duration alone, as a decisive predictor. We examined whether pre-operative CV_TST is associated with post-anaesthetic delirium and whether intra-operative haemodynamic instability mediates that association. METHODS: In this prospective cohort (n = 122, ≥65 y), wrist actigraphy (ActiGraph wGT3X-BT, ActiGraph LLC, USA) recorded seven nights of sleep before surgery. CV_TST of total sleep

Association between preoperative persistent hyperglycemia and postoperative length of hospital in geriatric hip fracture patients.

BACKGROUND: Metabolic disorders play a significant role in determining the length of hospital stay following hip fracture surgery in elderly patients. Therefore, it is crucial to conduct an in-depth investigation into the relationship between stress-induced hyperglycemia-one of the manifestations of metabolic disorders-and hospitalization duration in this patient population. METHODS: This retrospective cohort study analyzed medical records of patients who underwent hip fracture surgery at a tertiary medical institution between January 2017 and November 2024. LASSO regression identified covari

Preoperative prescription opioid use as an independent predictor of 90-day mortality and adverse events in craniotomy and craniectomy patients.

OBJECTIVE: A growing body of literature suggests that preoperative opioid exposure is an independent predictor of poor outcomes in surgical patients. No outcomes data exist on preoperative opioid use and craniotomies/craniectomies. The objective of this study was to determine the impact of preoperative opioid use on 90-day adverse events after craniotomy or craniectomy. METHODS: A single-center retrospective cohort study of 2445 patients undergoing a craniotomy/craniectomy between January 1, 2013, and October 1, 2018, was conducted. Baseline demographics, pre- and postoperative opioid use (mo

Association of preoperative neutrophil-to-lymphocyte ratio with the risk of postoperative delirium in elderly patients undergoing noncardiac surgery: a systematic review and meta-analysis.

To assess the correlation between preoperative neutrophil-to-lymphocyte ratio (NLR) and risk of postoperative delirium (POD) in older patients undergoing noncardiac surgery. PubMed, Web of Science, Embase, and Scopus were systematically retrieved from inception until February 2023. Two authors independently conducted the selection of literature, data extraction and statistical analysis. In this meta-analysis, Review Manager 5.4 was used for statistical analysis, and the mean difference (MD) and 95% confidence intervals (CIs) of preoperative NLR between the POD group and non-POD group were calc

Preoperative Variables of 30-Day Mortality in Adults Undergoing Percutaneous Coronary Intervention: A Systematic Review.

BACKGROUND AND AIM: Risk adjustment following percutaneous coronary intervention (PCI) is vital for clinical quality registries, performance monitoring, and clinical decision-making. There remains significant variation in the accuracy and nature of risk adjustment models utilised in international PCI registries/databases. Therefore, the current systematic review aims to summarise preoperative variables associated with 30-day mortality among patients undergoing PCI, and the other methodologies used in risk adjustments. METHOD: The MEDLINE, EMBASE, CINAHL, and Web of Science databases until Oct

Preoperative Anxiety: An Important Risk Factor of Postoperative Adverse Events and Increased Reoperation Rates in Patients Undergoing Single-Level Anterior Cervical Diskectomy and Fusion.

INTRODUCTION: With the rising prevalence of mental health disorders, it is important to consider their correlation with surgical outcomes. Anxiety has been associated with adverse events after many surgical procedures, but its relationship relative to anterior cervical diskectomy and fusion (ACDF) regarding medical complications has not previously been assessed in a large database study. METHODS: The 2010-Q3 2022 PearlDiver Mariner161 database was used to identify patients undergoing single-level ACDF. Those with versus without preoperative anxiety were matched 1:1 based on age, sex, and Elix

Significance of preoperative screening of deep vein thrombosis and its indications for patients undergoing urological surgery.

PURPOSE: Preoperative deep vein thrombosis (pre-DVT) is a risk of symptomatic venous thromboembolism (VTE) and a serious postoperative surgical complication. However, little is known about pre-DVT in patients undergoing surgery. This study aimed to investigate the incidence and screening criteria of pre-DVT in patients undergoing urological surgery. MATERIALS AND METHODS: Between 2015 and 2017, 320 patients admitted to our hospital for urological surgery were included in this retrospective study. All patients underwent preoperative D-dimer testing. Patients with elevated D-dimer (≥1.0 μg/mL)

Association between preoperative toe perfusion index and maternal core temperature decrease during cesarean delivery under spinal anesthesia: a prospective cohort study.

BACKGROUND: The main mechanism of body temperature decrease during cesarean delivery under spinal anesthesia is core-to-peripheral redistribution of body heat, attributable to vasodilation. Perfusion index (PI) obtained with a pulse oximeter helps to assess peripheral perfusion dynamics by detecting the change in peripheral vascular tone. This study aimed to examine whether preoperative toe PI could predict the decrease in core temperature induced by spinal anesthesia during cesarean delivery. METHODS: Parturients undergoing scheduled cesarean delivery under combined spinal-epidural anesthesi

Weighing In on the Controversy: Preoperative Imaging in Unicoronal Craniosynostosis Leads to Strategic Changes in Surgical Care.

BACKGROUND: Clinical findings in children with unilateral coronal craniosynostosis are characteristic, and therefore clinicians have questioned the need for confirmatory imaging. Preoperative computed tomographic imaging is a powerful tool for diagnosing associated anomalies that can alter treatment management and surgical planning. The authors' aim was to determine whether and how routine preoperative imaging affected treatment management in unilateral coronal craniosynostosis patients within their institution. METHODS: A retrospective, single-center review of all patients who underwent cran

Lung resections combined with vena cava replacement.

Resection and reconstruction of the superior vena cava in patients with lung cancer is still considered a surgical challenge; the numerous problems related to patient selection, the choice of the most appropriate surgical strategy and technique, the most suitable material for replacement, and the number of potential complications definitively show an impact on short-term outcome and long-term prognosis. However, with the appropriate indications and surgical technique, a clear benefit has been documented in a select group of patients with lung cancer.
