Progressive addition lenses--matching the specific lens to patient needs.

BACKGROUND: The objective of this study was to use state-of-the-art methods to measure the optical characteristics of commonly available progressive addition lenses (PALs) and to develop derivatives of the optical measurements that can be used as guidelines in selection of lenses based on patients' visual needs. METHODS: The optics of 28 PALs currently on the market were measured with a Rotlex Class Plus lens analyzer. PALs were specified with plano distance power and a near add of +2.00 D. Data were normalized to plano at the location specified by each manufacturer and acquired from each dat

Anterior segment imaging: ultrasound biomicroscopy.

Ultrasound biomicroscopy technology has become an indispensable tool in qualitative and quantitative assessment of the anterior segment. Advances in soft-ware design and algorithms will improve theoretical understanding of the pathophysiology of anterior segment disorders. Future applications of quantitative techniques will yield important information regarding mechanisms of angle closure, improving understanding of the dynamic functions of the iris, accommodation, presbyopia, and other aspects of anterior segment physiology and pathophysiology.

Potential range of clear vision in monovision.

A possible limitation for monovision contact lens treatment of presbyopia is that an intermediate correction cannot be incorporated into the prescription without compromising distance or near vision. A simple model using a standard prescription strategy for the add and a conservative estimate for the static depth of focus of the eye was used to predict the range of clear vision with monovision lenses. For adds up to 2D, little compromise in visual acuity was predicted for any gaze position.

Indications and risk factors in refractive lens exchange: outcomes and special cases (extremes hyperopes and myopes).

Refractive lens exchange (RLE) has become an effective option for the correction of ametropia and presbyopia in patients over 50 years of age. Unlike conventional cataract surgery, this procedure requires more comprehensive preoperative counseling, as patients often have high expectations. However, it is essential to emphasize that simultaneous vision intraocular lenses and other alternatives provide functional vision, though not perfect. The success of RLE largely depends on the surgeon's ability to select the most appropriate intraocular lens, considering its optical design, the management o…

Topical ocular application of aggrelyte-2A reduces lens stiffness in mice.

Presbyopia is the progressive loss of the ability of the lens to focus on nearby objects due to its increased stiffness. It occurs in the mid-40s and continues to worsen until the mid-60s. The age-associated increase in protein cross-linking in the lens leads to protein aggregation and water insolubility, especially in the nuclear region, contributing to lens stiffness. This study reports the development of aggrelyte-2A (methyl S-acetyl-N-(3,3-dimethylbutanoyl) cysteinate, a derivative of our previously reported aggrelyte-2) for reversing the stiffness of aged lenses. Aggrelyte-2A showed minim…

A biomechanical model for evaluating the performance of accommodative intraocular lenses.

Accommodation alters the shape of the eye lens to change focus from distant to near vision. This function declines with age in the development of presbyopia and most people experience a near total loss of accommodative ability by 55 years. Currently, there are no surgical procedures that correct presbyopia, but considerable work has been done in the development of accommodative intraocular lenses (AIOLs) implanted during cataract surgery. Despite these efforts, AIOLs only restore ∼ 20% of youthful accommodative amplitude and they suffer from high rates of visually-debilitating fibrosis. An imp…

Review of surgical devices using small aperture optics.

Small aperture optics work by blocking unfocused peripheral light rays while allowing central light rays to focus on the retina. This pinhole effect creates an extended depth of focus and has been used in presbyopia correction, improving intermediate and near vision without markedly affecting distance vision. Another beneficial effect of small aperture optics is reducing aberrations caused by irregular corneas or irregular pupils. The first small aperture surgical device was the Kamra corneal inlay used on the nondominant eyes of presbyopic emmetropes. The pinhole concept was also adapted into…

Preparation and Characterization of Biomimetic β-Lens Crystallins Using Single-Chain Polymeric Nanoparticles.

Presbyopia, the inability to focus at arm's length, and cataracts that cloud vision are associated primarily with changes in the mechanical and optical properties of the lens. The optical properties, particularly the refractive index, of the human lens originate from the cytoplasm of the lens fiber, which contains a highly concentrated solution (∼40%) of globular proteins referred to as α, β, and γ crystallins, of which β is the most abundant. In this study, we focus on the synthesis and characterization of a β-crystallin biomimetic in an effort to understand and develop treatments for presbyo…

The relationship between body and ambient temperature and corneal temperature.

PURPOSE: Exposure to elevated ambient temperatures has been mentioned as a risk factor for common eye diseases, primarily presbyopia and cataract. The aim of the present study was to examine the relationship among ambient, cornea, and body core temperature. METHODS: The relation between corneal temperature and ambient temperature was examined in 11 human volunteers. Furthermore, corneal temperature was measured using a thermal camera during elevation of body core temperature in three human volunteers and four rats. RESULTS: A linear relationship between corneal temperature and body temperatu…

Accommodation dynamics in aging rhesus monkeys.

Accommodation, the mechanism by which the eye focuses on near objects, is lost with increasing age in humans and monkeys. This pathophysiology, called presbyopia, is poorly understood. We studied aging-related changes in the dynamics of accommodation in rhesus monkeys aged 4-24 yr after total iridectomy and midbrain implantation of an electrode to permit visualization and stimulation, respectively, of the eye's accommodative apparatus. Real-time video techniques were used to capture and quantify images of the ciliary body and lens. During accommodation in youth, ciliary body movement was bipha…

Advances in cataract surgery and intraocular lenses.

Cataract surgery is one of the most common medical procedures among persons age 65 and older. Once an inpatient procedure, it has become a simple outpatient surgery with extremely low rates of morbidity. With the development of implantable intraocular lenses (IOLs), patients undergoing cataract surgery gained the benefit of corrected pre-existing refractive error, astigmatism, and presbyopia. This article reviews developments in cataract surgery and describes current IOL technology.

Refractive problems of the eye.

Refractive problems of the eye are common. About 25% of adults have detectable myopia, or short-sightedness. Hypermetropia, or long-sightedness, is even more common, with one study estimating about 50% of adults affected. The actual number that have symptomatic refractive problems is far smaller, with about 10% of people needing refractive correction for myopia. The authors give an overview of the common refractive problems of myopia, hypermetropia, presbyopia and astigmatism.
