Predictors of Receipt of Clinical Preventive Services in Adolescents and Adults with Down Syndrome Accessing Medicaid.

BACKGROUND: Persons with Down syndrome (DS) require preventive care that addresses their age-/gender- and syndrome-specific needs. Yet, adolescents and adults with DS do not receive these preventive care services as recommended. OBJECTIVE/HYPOTHESIS: To identify factors that predict receipt of age-/gender- and syndrome-specific preventive healthcare among adolescents and adults with DS. We hypothesized that more healthcare encounters and greater medical complexity would increase receipt of preventive care due to more opportunities to complete these activities. METHODS: Using Medicaid claims

The Harms of a Penal Colony.

More than just a jail, Rikers has become a site of shifting discourse on punishment and justice in the United States. In the book Life and Death in Rikers Island, Homer Venters argues that the systematic failures of jails to provide appropriate safety and care constitute human rights violations and public health risks. The former chief medical officer and commissioner of correctional health services for the NYC Health and Hospitals system, Venters offers critical insight on the Rikers jail system. "Because jails are chaotic and concealed from outside view," he asserts, "we only become aware of

Changing patterns in sexually transmitted disease syndromes in Kenya after the introduction of a syndromic management program.

OBJECTIVE: The objective of this study was to evaluate patterns in sexually transmitted disease (STD) syndromes after the introduction of an STD syndromic management program. STUDY: We used the HIV sentinel surveillance in patients with STDs (1990-2001) to compute the proportions of STD syndromes (as a proportion of all patients with STDs) before and after the introduction of the syndromic management program. RESULTS: A decline in the proportion of genital ulcer disease (GUD), urethral discharge (UD), and vaginal discharge (VD) was observed from the baseline (1990-1994) to the year 2000 (P <

Adolescents' mental health and the Greek family: preventive aspects.

Preventive mental health measures can be properly planned only if the various factors leading to the adolescent's personality structure are extensively investigated. Starting with the specific attitudes of a couple towards genetic counselling, the disadvantages of urbanization and of the dissolution of the traditional extended family are discussed with regard to their effect on the younger members. Data are produced concerning the child-rearing practices of Greek in comparison to American parents and their effect on the adolescent's emotional life. Extreme dependence on the family, pressure fo

[Work, exclusion, pain, suffering, and gender relations: a survey of female workers treated for repetitive strain injury at a public health clinic].

This article presents the results of a qualitative study based on patient records from a public health clinic and focusing on four therapeutic groups from 1995 to 1997. The study aimed to reconstruct the life and work history of women with repetitive strain injury (RSI) in relation to family, friends, company, workmates, health services, and impacts on daily life. The results corroborate the data in the scientific literature pointing to greater frequency of RSI among women, and allow us to list the needs related to the resulting biopsychosocial distress, suggesting the need for new research an

Stakeholder perspectives on adolescent sexual and reproductive health service utilization in Uganda's Busoga region: A qualitative study.

BACKGROUND: Adolescents in low- and middle-income countries face high rates of unintended pregnancy, sexually transmitted infections (STIs), and unmet sexual and reproductive health (SRH) needs. In Uganda, 24% of girls aged 15-19 years have begun childbearing, yet modern contraceptive prevalence among adolescents remains low (15%). These challenges are particularly severe in the Busoga region, where sociocultural norms, poverty, and limited health system capacity hinder access to adolescent-friendly SRH services. Despite national guidelines, utilization remains low. Few studies have triangulat

Preventive measures against HIV among Uganda's youth: Strategies, implementation, and effectiveness.

Preventing HIV among Uganda's youth is a critical public health priority due to the high prevalence of HIV/AIDS and the disproportionate burden of new infections among young people. This paper examines the preventive measures implemented to combat HIV/AIDS among Uganda's youth, focusing on strategies, implementation efforts, and the effectiveness of interventions. Comprehensive sexuality education programs have been implemented in schools and communities to provide young people with accurate information on HIV transmission, prevention methods, and reproductive health. HIV and Mass Incarceration: Where Infectious Diseases and Social Justice Meet.

More than 1% of all adults in the United States are currently in a jail or prison. This mass incarceration, particularly of African American men, fosters conditions that facilitate the spread of HIV in communities where both HIV and incarceration are endemic. Recognition of the role of mass incarceration in the perpetuation of the HIV epidemic is essential to development of effective HIV prevention policies.

A case of exemplarity: C. F. Rottböll's history of smallpox inoculation in Denmark-Norway, 1766.

Smallpox inoculation was one of the great discoveries of the 18th century and has been written into the grand narrative of medical progress, describing the taming of epidemic disease. Setting the perspective of progress aside, the article explores how this medical innovation was situated in 18th-century society and culture. The aim is to investigate how medical practice was intertwined with social structure and cultural patterns. The article takes its case from a book published in Copenhagen in 1766 by Professor C. F. Rottböll, former Head Physician of the Royal Inoculation House in Copenhagen

Styles of care provided to children in the United States: a comparison by physician specialty.

This study was based on findings from a national survey of physicians that was conducted from 1975 to 1977. The data concern face-to-face encounters with children in the ambulatory care setting. Over one half of the practices of pediatricians consisted of infants and preschoolers, whereas well over 50 percent of the child patients of other types of physicians were 10 to 19 years old. The proportion of visits dealing with a problem already under care was consistently greater for specialists other than primary care physicians; the proportion of visits for preventive care was much lower in the pr

The impacts of migration on maternal and child health services utilisation in Sub-Saharan Africa: evidence from Togo.

OBJECTIVES: Togo is experiencing a growing phenomenon of migration and consequently receives remittances from international migrants back to their relatives. Remittances represent about 9.4% of the gross domestic product (GDP), placing Togo in the top 10 recipients of remittances in the world in 2014. Despite the importance of remittances, information on their health outcomes is inadequate, particularly if remittances have a positive impact on the utilisation of maternal and child health services. The aim of this article is to evaluate the impacts of migration on maternal and child health serv
