Carbon dioxide accumulation during analgosedated colonoscopy: comparison of propofol and midazolam.

AIM: To characterize the profiles of alveolar hypoventilation during colonoscopies performed under sedoanalgesia with a combination of alfentanil and either midazolam or propofol. METHODS: Consecutive patients undergoing routine colonoscopy were randomly assigned to sedation with either propofol or midazolam in an open-labeled design using a titration scheme. All patients received 4 μg/kg per body weight alfentanil for analgesia and 3 L of supplemental oxygen. Oxygen saturation (SpO₂) was measured by pulse oximetry (POX), and capnography (PcCO₂) was continuously measured using a combined dedi

Comparison of Dexmedetomidine and Midazolam in Conscious Sedation During Dental Implant Surgery: A Randomized Clinical Trial.

PURPOSE: Conscious sedation in oral surgical procedures provides comfort both for patients and practitioners. Midazolam is a sedative agent commonly used in this manner. Dexmedetomidine is also a decent sedative agent, with its analgesic and mild respiratory effects, which is usually preferred in intensive care units. Dexmedetomidine has been recently used in dental surgeries. This study aimed to investigate the analgesic and respiratory properties of midazolam and dexmedetomidine in conscious sedation during dental implant procedures and to compare these two drugs in terms of ease of use and

Safety and Feasibility of Analgosedation for Electrochemotherapy of Liver Lesions.

Interventional Oncology treatments grant low-risk mini-invasive alternatives to surgery for cancer patients. Percutaneous ablative therapies represent a cornerstone for treatment of liver cancer patients. Among these, a newly emerging one is represented by electrochemotherapy. Improvements in analgesia and sedation can nowadays offer optimal support for ablative procedures, serving as a valid alternative to general anesthesia. The intention of this retrospective monocentric study is to report our preliminary experience on feasibility and safety of electrochemotherapy for treatment of complex l

Ciprofol vs propofol for gastrointestinal endoscopy sedation: a systematic review and meta-analysis.

BACKGROUND: Ciprofol, a propofol derivative, is increasingly used for sedation in China. However, the specific benefits of ciprofol in gastrointestinal endoscopic sedation have not been fully evaluated. This meta-analysis aimed to compare the efficacy and safety of ciprofol with propofol in gastrointestinal endoscopy. METHODS: Four public databases were searched for the relevant literature to February 1, 2025. Studies were excluded based on predefined criteria, and the characteristics and outcome data of each included study were collected. Subsequently, meta-analysis and trial sequential anal

Breathe-squeeze: pharmacodynamics of a stimulus-free behavioural paradigm to track conscious states during sedation☆.

BACKGROUND: Conscious states are typically inferred through responses to auditory tasks and noxious stimulation. We report the use of a stimulus-free behavioural paradigm to track state transitions in responsiveness during dexmedetomidine sedation. We hypothesised that estimated dexmedetomidine effect-site (Ce) concentrations would be higher at loss of responsiveness (LOR) compared with return of responsiveness (ROR), and both would be lower than comparable studies that used stimulus-based assessments. METHODS: Closed-Loop Acoustic Stimulation during Sedation with Dexmedetomidine data were an

[New possibilities with retroflexed functional endoscopy].

BACKGROUND: Anterograde transnasal flexible endoscopy of the pharynx, larynx, and proximal trachea without sedation has been established since the 1990s. Retroflexed endoscopic functional analysis has recently been introduced. OBJECTIVES: Adding retroflexed functional endoscopy from the oral cavity up to the duodenum to anterograde diagnostics allows the precise demonstration of how different factors interact in a complex way, e. g., in case of dysphagia or pathological reflux. MATERIALS AND METHODS: A 70-cm-long high-definition endoscope without a channel system is equipped with a 200° retr

Propofol-induced sedation diminishes the strength of frontal-parietal-occipital EEG network.

The level of conscious experience can be effectively and reversibly altered by the administration of sedative agents. Several studies attempted to explore the variations in frontal-parietal network during propofol-induced sedation. However, contradictory outcomes warrant further investigations. In this study, we implemented the Neural Gas algorithm-based delay symbolic transfer entropy (NG-dSTE) for investigation of frontal-parietal-occipital (F-P-O) network using scalp EEG signals recorded during altered levels of consciousness. Our results show significant disruption of the F-P-O network dur

Ultrasonographic evaluation of relative gastrointestinal layer thickness in cats without clinical evidence of gastrointestinal tract disease.

The objectives of this study were (1) to measure normal thickness values of the muscularis, submucosal, mucosal and serosal layers in each gastrointestinal (GI) segment (gastric fundus, body and pyloric antrum; duodenum; jejunum; ileum; colon), and (2) to calculate a ratio of muscularis and mucosal layer thickness to aortic diameter measured at the level of the celiac artery (Musc:Ao and Muc:Ao) in each GI segment in a sample of healthy cats. Ultrasonographic examination of the GI tract was performed, and measurements of the individual layers in each GI segment were obtained in 38 healthy cats

Sedation in PACU: the role of propofol.

In critically ill patients, adequate sedation increases comfort, minimizes stress response and facilitates diagnostic and therapeutic procedures. Propofol (2-, 6-diisopropylphenol) is an intravenous sedative-hypnotic agent popular for sedation in the Intensive Care Unit. The favorable propofol pharmacokinetic, characterized by a three compartment linear model, allows rapid onset and short duration of action. The emergence time from sedation with propofol varies with the depth and the duration of sedation and the patient's bodyweight. Propofol causes hypotension, particularly in volume depleted

Propofol. An update of its use in anaesthesia and conscious sedation.

Propofol is an intravenous sedative hypnotic agent which rapidly and reliably causes loss of consciousness. It is also associated with a quick and 'smooth' recovery, which distinguishes it from many of the more traditional anaesthetic regimens. Like other intravenous agents, propofol is both a cardiovascular and a respiratory depressant; however, the risk of these effects can be lessened by appropriate dosage adjustment or patient management. Anaphylaxis with propofol is rare. Propofol anaesthesia in day case surgery is consistently associated with a quicker early recovery than other intraveno

A double-blind placebo controlled trial of oral midazolam as premedication before flexible sigmoidoscopy.

OBJECTIVE: We aimed to determine the efficacy of oral midazolam as premedication to improve tolerance of flexible sigmoidoscopy. METHODS: Ninety-nine patients were randomized to receive 7.5 mg of oral midazolam solution mixed with apple juice or placebo (apple juice), 20 min before sigmoidoscopy. Patients' anxiety and pain experienced before sedation, before the procedure, and during the procedure were assessed using a 10-cm visual analog scale (VAS) by both patients and physicians (0 = no pain, anxiety, 10 = severe pain, anxiety). Extent of sedation, amnesia, overall tolerance, adequacy of s

Percutaneous studies of the upper urinary tract in children, with special emphasis on infants.

Of 101 antegrade pyelography and/or percutaneous nephrostomy procedures performed within a 6-year period, 18 involved patients less than 6 months of age. The procedures in this age group were characterized by ease of performance -without sedation in the neonates and with sedation in the children -and by the use of smaller needles and catheters. The procedures enabled differentiation between obstructed and nonobstructed dilated systems with pressure-flow studies, depiction of the intimate anatomy of complicated urologic abnormalities, temporary drainage, decompression of pyonephrosis, endourolo

The effects of preoperative anxiety on intravenous sedation.

Anxiety is known to cause feelings of uneasiness, tension, and nervousness, and previous studies have noted that anxiety and its effects may have an effect on out-patient sedation for patients undergoing surgical procedures. In this study, we assess the effects of anxiety on 25 outpatients undergoing intravenous sedation for third molar extraction. Before the procedure, subjects completed the State-Trait Anxiety Inventory, and intraoperative patient movement was assessed using a subjective scale. We found that patients with a high level of preoperative anxiety had a greater degree of average i
