How recent psychological distress relates to session quality in psychotherapy: The mediating roles of the real relationship and working alliance.

OBJECTIVE: To examine how clients' recent psychological distress (PD) relates to perceived psychotherapy session quality (SQ), and to test whether this association is statistically accounted for by clients' ratings of the real relationship (RR) and the working alliance (WA). METHODS: Secondary analyses combined data from two studies of adults (N = 1175) receiving individual psychotherapy. Following a session, clients completed measures of anxiety and depressive symptoms (7-day recall), RR, WA (task, goal and bond), and SQ. A covariate-adjusted path model was performed. RESULTS: PD was associ

Nonverbal empathy and perceived therapist warmth and competence: An experimental video vignette study.

OBJECTIVES: This study examined how nonverbal empathy affects perceptions of therapists' competence and warmth. It also looked at the influence of the Big Five, self-rated empathy, and secure attachment, on these perceptions. METHOD: A between-subjects online randomized controlled trial involved 257 participants (average age 41.57 years, with 66.1% identifying as female), who were randomly assigned to view one of two video clips. These videos depicted a therapist demonstrating either high (E + ) or low (E-) nonverbal empathy during a simulated therapy session. Nonverbal empathy was conveyed t

Predicting patient outcomes from initial in-session interactions within cognitive behavioral therapy and person-centered experiential therapy for severe depression.

OBJECTIVE: Cognitive behavioral therapy (CBT) and person-centered experiential therapy (PCET) are the most frequently delivered individual psychological therapies in the UK National Health Service Talking Therapies program. However, many patients do not benefit from treatment and motivational processes early in therapy may influence outcomes. This study assessed specific, in-session interaction-types at the first session of CBT and PCET for severe depression that may predict therapeutic outcomes. METHOD: Trained raters assessed active engagement in specific interactions from 103 first session

The impact of the Memory Support Intervention on therapist memory for treatment contents.

OBJECTIVE: Given that therapist memory for treatment is likely to be sub-optimal and may affect the quality of treatment, this secondary analysis aimed to evaluate the impact of the Memory Support Intervention on therapist treatment recall in the context of delivering cognitive therapy (CT) for major depressive disorder (MDD). METHOD: Adults (N = 172) with MDD were randomized to receive CT + Memory Support or CT-as-usual. Therapist demographics were comparable across conditions (N = 19; Mean age = 28.5 years; 79.0% female; 42.1% Caucasian). The amount of memory support delivered by therapists

Guarding and flow in the movements of people with chronic pain: A qualitative study of physiotherapists' observations.

BACKGROUND: Among the adaptations of movement consistently associated with disability in chronic pain, guarding is common. Based on previous work, we sought to understand better the constituents of guarding; we also used the concept of flow to explore the description of un/naturalness that emerged from physiotherapists' descriptions of movement in chronic pain. The aim was to inform the design of technical systems to support people with chronic pain in everyday activities. METHODS: Sixteen physiotherapists, experts in chronic pain, were interviewed while repeatedly watching short video clips

Ability to move between self-states and emotional experiencing and processing as predictors of symptomatic change.

OBJECTIVES: Contemporary relational theories consider clients' ability to move between multiple self-states and clients' ability to experience and process emotions to be two therapeutic processes inherently connected and fundamental to growth and change in psychotherapy. The current research aimed to empirically explore these theoretical assumptions by (1) examining the between- and within-client temporal association between clients' levels of ability to move between multiple self-states and clients' emotional experiencing and processing over the course of therapy, and by (2) testing whether t

Sexual minority men want provider behavior consistent with attitudes and norms during patient-provider interactions regarding HIV prevention.

HIV is a health problem for sexual minority men in the United States. One factor among many that contributes to this HIV disparity is poor patient-provider interactions. We focused on specific provider behavior preferred by sexual minority men during patient-provider interactions about HIV prevention. We interviewed 20 HIV-negative sexual minority men who endorsed 1+ psychosocial HIV risk factor. We used follow up interviews and conventional content analysis. Among our sample, 55% identified as White; 50% as bisexual, (Mage = 28.45). Findings suggested even some providers knowledgeable about s

Therapists' Perceptions of Their Encounter With Sex Offenders.

Despite the increasing interest in therapists' responses to their encounter with sex offenders, there is a lack of research on their subjective perceptions of this encounter and on their experience working with this client population. The study presented in this article is part of a larger qualitative research project conducted among 19 social workers (12 were women and 7 were men; their ages ranged from 30 to 66 years; 15 of them were Jewish and 4 were Arab). In-depth semi-structured interviews were conducted to examine their attitudes toward and perceptions of their encounter with sex offend

Narrative self-constitution and vulnerability to co-authoring.

All people are vulnerable to having their self-concepts shaped by others. This article investigates that vulnerability using a theory of narrative self-constitution. According to narrative self-constitution, people depend on others to develop and maintain skills of self-narration and they are vulnerable to having the content of their self-narratives co-authored by others. This theoretical framework highlights how vulnerability to co-authoring is essential to developing a self-narrative and, thus, the possibility of autonomy. However, this vulnerability equally entails that co-authors can under

The relationship between intraoperative teamwork and management skills in patient care.

BACKGROUND: Optimal team performance in the operating room (OR) requires a combination of interactions among OR professionals and adherence to clinical guidelines. Theoretically, it is possible that OR teams could communicate very well but fail to follow acceptable standards of patient care and vice versa. OR simulations offer an ideal research environment to study this relationship. The goal of this study was to determine the relationship between ratings of OR teamwork and communication with adherence to patient care guidelines in a simulated scenarios of malignant hyperthermia (MH). Heart rate variability and the anxious client: cardiac autonomic and behavioral associations with therapeutic alliance.

This exploratory study was designed to investigate the link between a client's heart rate variability (HRV) and the forming of a therapeutic alliance (TA) during psychotherapy. Change in HRV is associated with many psychological and physiological situations, including cardiac mortality. Cardiac effects were evaluated during therapy in 30 symptomatically anxious clients using HRV during six weekly 1-hour therapy sessions (S1-S6). Therapeutic index (TI), a measure of TA, was evaluated using skin conductance resonance between client and therapist. The Working Alliance Inventory provides a subject

Phobias of attachment-related inner states in the psychotherapy of adult survivors of childhood complex trauma.

The clinical case described in this article illustrates the value of taking into account the dynamics of disorganized attachment in the assessment of attachment-related phobias (phobia of attachment and phobia of attachment loss) during the psychotherapy of chronically traumatized patients. These seemingly opposite phobias typically coexist in the same patient, appear as phobias of both inner states (affect phobias) and relational experiences, and are linked to dissociated representations of self-with-other. Theory and research on attachment disorganization provide a clinician-friendly concept
