Aggregatibacter actinomycetocomitans prosthetic valve endocarditis causing sequential transient ischaemic attack, rheumatoid factor-positive inflammatory polyarthritis and intracerebral haemorrhage.

Subacute bacterial endocarditis is often associated with non-specific symptoms and multi-system embolic phenomena that can make diagnosis difficult. We present a case of Aggregatibacter actinomycetocomitans (Aa) prosthetic valve endocarditis, initially causing a transient ischaemic attack, inflammatory arthropathy, then intracerebral haemorrhage, the latter leading to diagnosis. Aa should be suspected in prosthetic valve recipients with sequential onset of inflammatory arthropathy and cerebrovascular disease.

Novel in vivo mouse model of shoulder implant infection.

BACKGROUND: Animal models are used to guide management of periprosthetic implant infections. No adequate model exists for periprosthetic shoulder infections, and clinicians thus have no preclinical tools to assess potential therapeutics. We hypothesize that it is possible to establish a mouse model of shoulder implant infection (SII) that allows noninvasive, longitudinal tracking of biofilm and host response through in vivo optical imaging. The model may then be employed to validate a targeting probe (1D9-680) with clinical translation potential for diagnosing infection and image-guided débrid

Nocardia Farcinica infection of a reversed shoulder prosthesis : case report and review.

We present a case of Nocardia farcinica after placement of reverse shoulder prosthesis in a 73-year-old woman. One month after surgery, the patient was admitted to the hospital with a spontaneous drainage of the wound and complaints of aggravating pain in the operated shoulder. There was no history of an immunosuppressive disease or therapy. After cultivation and empiric therapy with flucloxacillin, Nocardia farcinica was found and treated with a combination of intravenous amikacin and ceftriaxone. Eight days after drainage, a rinse and replacement of the polyethylene cup and glenosphere was e

Antibiotic susceptibility among Staphylococcus epidermidis isolated from prosthetic joint infections with special focus on rifampicin and variability of the rpoB gene.

Staphylococcus epidermidis is the most important pathogen in infections related to implanted foreign materials, especially prosthetic joint infections (PJIs). The aim of this study was to investigate the antimicrobial activities of 16 antibiotics against S. epidermidis isolated from PJIs, with special focus on rifampicin and rpoB variability. Ninety-one per cent of the isolates were multiresistant (i.e. resistant to members of more than three classes of antibiotics). Thirty-nine per cent were resistant to rifampicin, associated with one or two single-nucleotide polymorphisms (SNPs) in rpoB. Us

Biofilm formation by Pseudomonas aeruginosa on ossicular reconstruction prostheses.

PURPOSE: Ossicular chain reconstruction may be complicated by prosthesis extrusion. As prostheses are commonly placed in middle ears contaminated with biofilm-forming bacteria, such as Pseudomonas aeruginosa (PA), extrusion may be caused by development of a biofilm on the prosthesis and the host response to this biofilm. The purpose of this experiment was to determine if PA forms biofilm on different ossicular chain reconstruction prostheses to a different degree. METHODS: Prostheses made of titanium, hydroxylapatite (HA), and plastic (23 each) were cultured with PA in broth for 96 hours. Bio

[Nocardia farcinica infection of a vascular prosthesis].

A 61-year-old woman presented with purple-red subcutaneous nodules on her right leg. She had experienced fever up to 40 degrees C for the past 10 days and felt generally weak over the last two months. Four months earlier, a vascular graft had been implanted in her right femoral artery. Based on the diagnosis of skin infection due to implantation of an infected prosthesis, she was hospitalized and treated with an antibiotic regime. During the initial antibiotic treatment, the symptoms deteriorated, and she developed joint, hepatic and CNS abscesses. Finally, a microbiologic culture with an exte

Surgical treatment of methicillin-resistant Staphylococcus aureus infection following infrainguinal arterial reconstruction.

A 64-year-old man was referred to our hospital with Methicillin-resistant Staphylococcus aureus (MRSA) infection following infrainguinal arterial reconstruction. As repeated MRSA sepsis occurred, we decided to remove the infected graft with distal revascularization via circuitous graft tunneling to avoid serious infections and allow limb salvage. An iliofemoro bypass was performed via an extra-anatomical bypass, from just below the iliac crest into the musculus quadriceps femoris using an 8 mm-ringed polyester gelatin polypropylene tube graft, with complete debridement of a groin infection. Overwhelming septic infection with a multi-resistant Staphylococcus aureus (MRSA) after total knee replacement.

INTRODUCTION: The incidence of early deep infection after arthroplasty of the knee is very low but could represent a serious future problem. MATERIAL AND METHODS: The authors report on a 71-year-old woman with gonarthritis who was supplied with a total knee endoprosthesis and developed a local infection followed by septic shock. RESULTS: The infection was evidently caused by a multiresistant Staphylococcus aureus (Staph. aureus). The primary clinical signs closely resembled a necrotizing fasciitis. Systemic and local application of vancomycin led to an improvement of the symptoms at secondar

Povidone iodine demonstrates strong efficacy in reducing Candida biofilm in an in vitro fungal prosthetic infection.

Periprosthetic joint infection (PJI) caused by fungi is rare but can lead to devastating outcomes. However, the best treatment for fungal PJI has not been established. This study aims to identify the optimal surgical irrigation solution to reduce the bioburden of Candida biofilm from prosthetic surfaces. Candida albicans biofilm was cultured on titanium screw caps and treated for 3 min and 10 min using five clinically used irrigation solutions: Normal Saline, Fluconazole solution (2000 µg/mL), Nystatin solution (5000 IU/mL), Chlorhexidine (0.05%), Povidone Iodine (0.35%), and Povidone Iodine (

Preventive Strategies for Driveline Infections in Ventricular Assist Devices: A Systematic Review.

Driveline infection (DLI) is the most frequent infectious complication in patients supported with ventricular assist devices (VADs) and remains a major cause of morbidity and rehospitalization. This systematic review aimed to synthesize the available evidence on preventive strategies to reduce the incidence of DLI in adult VAD recipients. A systematic search of MEDLINE/PubMed, Scopus, CINAHL, Web of Science, the Cochrane Library, and APA PsycInfo up to April 1, 2026, followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. [Bacteriophage Therapy in Orthopaedics and Trauma].

Bacteriophage therapy has re-emerged as a potential adjunctive strategy in the management of orthopedic and trauma-related infections, particularly fracture-related infection (FRI) and periprosthetic joint infection (PJI). These conditions are associated with biofilm formation, repeated surgery, prolonged antimicrobial therapy, impaired functional outcomes, and increasing rates of multidrug-resistant pathogens. The therapeutic rationale of bacteriophages is based on their high specificity for target bacteria, their ability to replicate at the site of infection, and their potential activity aga

Does Postoperative Colonoscopy Increase the Risk for Hip and Knee Periprosthetic Joint Infection? A Systematic Review and Meta-Analysis.

BACKGROUND: Periprosthetic joint infections (PJIs) are a serious complication of hip and knee total joint arthroplasty (TJA), leading to increased morbidity, mortality, reoperations, and strain on health care systems. Hematogenous spread from remote sites is a common cause of PJI, prompting guidelines recommending antibiotic prophylaxis for several procedures. However, evidence on whether colonoscopies increase PJI risk post-TJA remains conflicting. To our knowledge, this study is the first systematic review and meta-analysis to assess the risk of PJI following colonoscopy in patients who have

Apical Abscess on a Zygomaticus Implant Initially Diagnosed as a Cutaneous Carcinoma.

We present a case of an infection on a zygomaticus implant presenting on the skin, mimicking a cutaneous carcinoma, and presenting to a head and neck tumor board. The clinical findings were an intermittently discharging lesion over the zygomatic bone, which resolved upon removing the offending zygomaticus implant. It is essential to be aware that infections on a zygomaticus implant can occur well away from the normal tooth-bearing areas, and having a dentist with knowledge of these implants on a tumor board can prevent misdiagnosis and treatment.

Bacteriophage therapy as an innovative strategy for the treatment of Periprosthetic Joint Infection: a systematic review.

BACKGROUND: Periprosthetic Joint Infection (PJI) following hip and knee arthroplasty is a catastrophic complication in orthopaedic surgery. It has long been a key focus for orthopaedic surgeons in terms of prevention and management. With the increasing incidence of antibiotic resistance in recent years, finding more targeted treatment methods has become an increasingly urgent issue. Bacteriophage Therapy (BT) has emerged as a promising adjunctive treatment for bone and joint infections in recent years. It not only effectively kills bacteria but also demonstrates significant anti-biofilm activi

Lead Extraction-Indications, Procedure, and Future Directions.

Cardiac implantable electronic device (CIED) implantation has steadily increased in the United States owing to increased life expectancy, better access to health care, and the adoption of updated guidelines. Transvenous lead extraction (TLE) is an invasive technique for the removal of CIED devices, and the most common indications include device infections, lead failures, and venous occlusion. Although in-hospital and procedure-related deaths for patients undergoing TLE are low, the long-term mortality remains high with 10-year survival reported close to 50% after TLE. This is likely demonstrat

Infections of cardiac implantable electronic devices: Epidemiology, mechanisms, and preventive measures.

Cardiac implantable electronic device (CIED) infections represent a complication associated with high morbidity and mortality. Despite enormous efforts to prevent them, the rates of infections continue to rise out of proportion to the reported increase in CIED implantation rates. Following extensive research of various prevention strategies and new technologies, several organizations have issued recommendations and consensus papers covering this topic. Our narrative review aims to provide a summary of the existing preventive strategies put forward by the European Heart Rhythm Association conse

Preoperative intra-articular steroid injections within 3 months increase the risk of periprosthetic joint infection in total knee arthroplasty: a systematic review and meta-analysis.

OBJECTIVE: Albeit with no disease-modifying effects, intra-articular steroid injections (IASIs) are still widely used to relieve symptoms of knee osteoarthritis. Previous literature has reported conflicting results regarding the safety of IASI in terms of periprosthetic joint infection (PJI) in total knee arthroplasty (TKA). This study tried to determine whether preoperative IASIs increased the risk of PJI, with different time intervals between the injections and surgery. METHODS: A computerized search of MEDLINE, EMBASE, and Cochrane Library was conducted for studies published before October

Decreased Risk of Periprosthetic Joint Infection with Concurrent Hardware Removal During Conversion Total Hip Arthroplasty Compared to Staged Removal: A Matched Cohort Analysis.

INTRODUCTION: Hardware removal before conversion total hip arthroplasty (cTHA) is a challenging task for the orthopaedic surgeon, although there is little consensus on the timing of hardware removal to mitigate risk of surgery-related complication following cTHA. METHODS: Using a national insurance database, we evaluated patients who underwent hardware removal either on the same day or within 1 year before cTHA, resulting in a total of 7,756 patients. After matching based on demographic factors and comorbidities, both staged and concurrent groups consisted of 2,752 patients. The 90-day and 1-
