A 3-year-old boy with Q fever received several kinds of antibiotics including minocycline, but spiking fever and positive PCR of Coxiella burnetii continued for several months. He became asymptomatic and his abnormal laboratory data normalized after the administration of gamma interferon three times a week.

Valvular endocarditis is the most serious complication of chronic Q fever, an infectious disease due to Coxiella burnetii. We describe a 37-year-old farmer with a 3-week history of fevers and hepatitis, in whom Q fever was diagnosed. The diagnosis was based on the findings of characteristic "ring" granulomas on a bone marrow biopsy specimen and confirmed by complement-fixation antibody tests to Coxiella burnetii. Unusual aspects of this case included (1) relatively low complement-fixation antibody titers, (2) prolonged prothrombin time, (3) false-positive results of a serologic test for the human immunodeficiency virus (HIV), and (4) ring granulomas that progressed to atypical granulomas in biopsy specimens.

A 70-year-old retired fisherman presented with fever, chills and headache for four days. The clinical presentation was mild, including a self-limiting fever, pneumonia and elevated liver enzyme levels. Acute Q fever was proved by a four-fold rise of phase II IgG antibodies. Interestingly, the man denied any animal contact or travelling over the past year. This case may suggest that Q fever should be added to the list of differential diagnoses for acute febrile disease in Taiwan.

Neurological complications of Coxiella burnetii infection (Q fever) are rare, although the occurrence of headache, paresthesias, and transient focal deficits has been reported. We report the case of a patient with a relapsing demyelinating polyradiculoneuritis as an aftermath of C. burnetti endocarditis and pneumonia.

Q fever was diagnosed in 443 patients in Northern Ireland between 1962 and 1989. From 1986 onwards there was an increase, which peaked in 1989 with 107 cases of whom 47 were infected in Ballycastle, Co Antrim. There were three outbreaks and 21 clusters of patients with Q fever. We report a case of Q fever endocarditis in a patient who presented with a slight pyrexia and acute cardiac failure due to aortic incompetence. The diagnosis was made by detecting high titres of serum IgG and IgA antibody against Coxiella burnetii phase I antigens and confirmed by demonstrating C. burnetii on the excised aortic valve using immunofluorescence and electron microscopy. In April and May, 1981, an outbreak of Q fever occurred in a university department which used gravid sheep for fetal respiratory studies. During the subsequent investigation 91 people were studied and 28 were found to have complement-fixing antibodies to the phase II antigen of Coxiella burneti in their serum. This report describes the clinical manifestations and pathological findings in 5 patients with serologically diagnosed acute Q fever. Each patient presented with headache, malaise, spiking fever, and hepatitis. Percutaneous biopsy of the liver in 4 patients revealed granulomatous changes with many lesions containing a dense fibrin ring surrounding a central lipid vacuole. Biopsy of the bone marrow in the fifth patient revealed similar abnormalities. These lipogranulomas should be considered characteristic of Q fever.

A patient with nephrotic syndrome and Q-fever endocarditis (confirmed serologically and ultrastructurally) was found to have mesangio-capillary glomerulonephritis with parietal deposits of C3 and IgM and some IgM in the mesangium. Elution studies showed that IgM antibodies reactive against insoluble Coxiella antigens were present in the kidney. Review of the literature suggests that this type of immune complex nephritis may be associated with Q-fever. Possible reasons for the variability of the nephritis associated with infective endocarditis are discussed.
