Retrospective biological dosimetry of absorbed radiation.

Frequencies of chromosomal translocations in human peripheral blood lymphocytes irradiated in vitro by 200 kV X rays have been estimated by … lted in a small but significant reduction in the yield of exchanges, thus suggesting repair of some radiation damage during the period between the two fractions. Retrospective biological dosimetry was attempted with FISH assay of five radiation workers with chronically accumulated doses of approximately 500 mSv received over a period of 2-3 decades. Among t …

Radiation therapy for extensive actinic keratosis.

Actinic keratosis in the most common premalignant keratotic tumour of sun-exposed skin. A 66-year-old man developed a large actinic keratosis on his scalp, which did not respond to conventional treatment. Fractionated radiotherapy with a cumulative dose of 28 Gy resulted in complete remission without relapse during a 14-month follow-up. In older subjects in whom conventional treatment fails, low-dose fractionated radiation therapy is an effective alternative method.

Skin cancer and solar UV radiation.

Ultraviolet (UV) radiation in sunlight is the most prominent and ubiquitous physical carcinogen in our natural environment. It is highly genotoxic but does not penetrate the body any deeper than the skin. Like all organisms regularly exposed to sunlight, the human skin is extremely well adapted to continuous UV stress. Well-pigmented skin is clearly better protected than white Caucasian skin. The sun-seeking habits of white Caucasians in developed countries are likely to have contributed strongly to the increase in skin cancer observed over the last century. Skin cancer is by far the most comm…

Radiation treatment of benign mesenchymal disease.

The benign mesenchymal diseases, for which radiation is often quite effective in halting progression or achieving complete and permanent resolution, include neoplastic and nonneoplastic processes (eg, giant cell tumor of bone to keloid). Radiation oncologists have been reluctant to employ radiation in the management of patients with benign disease for several reasons: (1) the small but nonzero risk of late appearance of radiation-induced malignant tumors; (2) were radiation subsequently required to be employed in the same region of the body for treatment of a separate and independent neoplasm,…

Hormonal therapy in chronic radiation colitis.

Severe gastrointestinal bleeding is a rare complication of radiation therapy that requires frequent transfusions. This case report describes a patient with severe bleeding from radiation colitis after treatment of bladder cancer. During 5 months of therapy with multiple drugs, the patient needed 26 units of packed red cells. A subsequent hormone therapy consisting of an estrogen-progesterone combination significantly reduced the need for blood transfusions and hospitalization. We conclude that hormones might provide a promising new additional symptomatic therapy for bleeding radiogenic colitis…

Radiation-induced cerebral meningiomas. Case reports.

Two cases are presented of meningiomas following external cranial irradiation in which several features clearly indicate a causal relationship between radiotherapy and tumour development. In one patient multiple recurred meningioma was diagnosed 30 years after low-dose X-ray treatment of Tinea capitis; in the other patient the meningioma was diagnosed approximately 18 years after high-dose radiation therapy for astrocytoma. The features distinguishing radiation-induced meningiomas from other meningiomas are reviewed. The use of radiation therapy only in those cases in which there is clear evid…

Hyperbaric oxygen treatment for radiation proctitis.

PURPOSE: Our objective was to assess, retrospectively, the efficacy of hyperbaric oxygen treatment in radiation proctitis in all patients who have completed treatment for this disease at the Fremantle Hyperbaric Oxygen Unit. This unit is the only one of its kind in Western Australia. METHODS AND MATERIALS: Patients were assessed by a review of hospital records, blood bank records, and clinic review (if this was convenient), and all patients responded to a telephone survey. Patients were questioned regarding radiation proctitis symptoms and the degree to which each had improved. RESULTS: Most…

[Palliative radiation therapy for multiple myeloma].

PURPOSE: Radiation therapy is a useful palliative modality for refractory lesions of multiple myeloma. It has been reported that total doses of 10 to 20 Gy are usually adequate to obtain some degree of pain relief. However, there are many patients who need additional doses to obtain sufficient pain relief. In this study, we retrospectively analyzed the records of patients with multiple myeloma irradiated at our department, in an attempt to develop an effective treatment policy for this disease. MATERIALS AND METHODS: Twenty-nine patients with 53 lesions were treated between 1968 and 1993. Tot…

Chromosome painting of radiation-induced micronuclei.

In this report, in situ hybridization with whole chromosome painting probes was used to paint radiation-induced micronuclei (MN) in three lymphoblastoid cells lines to investigate the frequency of radiation-induced MN. The results obtained for four different chromosomes showed that there was a significant deviation of the numbers of signal-positive MN from that expected on the basis of DNA proportionality. Restriction of the analysis to three chromosomes showed that the deviations arose primarily from chromosome 7, which was underrepresented in the numbers of signal-positive MN in the group of…

Bullous pemphigoid induced by radiation therapy.

A rare complication occurring in a female patient who underwent conservative surgery and radiation therapy for breast cancer is described. Three weeks after the completion of radiotherapy, a diffuse bullous pemphigoid eruption developed in the irradiated area, spreading thereafter to the whole body. Although systemic cutaneous side effects have been reported after radiation therapy, this is the first occurrence of bullous pemphigoid ever reported in a female patient following treatment for breast cancer. Having made the diagnosis, an effective therapeutic regimen including nicotinamide and tet…

Surgery for radiation-induced valvular disease.

The effects of radiation on the heart have been well described including acute and chronic pericarditis, myocardial fibrosis, accelerated arteriosclerosis of the coronary arteries. However, valvular dysfunction secondary to mediastinal irradiation has received less attention. We report two cases who developed valvular dysfunction associated with coronary artery disease possibly caused by mediastinal irradiation and a review of the literature regarding surgery for radiation induced valvular disease. Both patients underwent aortic valve replacement and coronary artery bypass graft surgery. With …

Radiation exposure in paranasal sinus imaging.

Plain films of the paranasal sinus were the imaging technique most frequently used before the advent of computed tomography. With improved resolution, computed tomography imaging has become the standard for evaluating patients with disease of the paranasal sinuses. With the widespread availability of computed tomography scanning, patients are likely to undergo multiple examinations during the treatment of their disease. Concern has been raised over the radiation exposure to the patient, in particular, with regard to radiation-induced cataracts. The purpose of this study is to measure the radia…

Beta-particle dosimetry in radiation synovectomy.

Beta-particle dosimetry of various radionuclides used in the treatment of rheumatoid arthritis was estimated using Monte Carlo radiation transport simulation coupled with experiments using reactor-produced radionuclides and radiachromic film dosimeters inserted into joint phantoms and the knees of cadavers. Results are presented as absorbed dose factors (cGy-cm2/MBq-s) versus depth in a mathematical model of the rheumatoid joint which includes regions of bone, articular cartilage, joint capsule, and tissue (synovium) found in all synovial joints. The factors can be used to estimate absorbed do…

Patient radiation doses from enteroclysis examinations.

Data relating to patient dose have been acquired for enteroclysis examinations (small bowel enemas) performed at the John Radcliffe Hospital, Oxford, on 23 adult patients. Dose-area products, fluoroscopy times and the number of radiographs taken are used to compare the examination procedure at the John Radcliffe Hospital with enteroclysis and barium follow-throughs performed elsewhere. The mean dose-area product for the 23 examinations was 6.8 Gy cm2 and the mean effective dose was estimated to be 1.5 mSv. These doses are intermediate between those arising from barium meals and barium enemas p
