Volar PEEK plate for distal radius fracture: analysis of adverse events.

BACKGROUND: The aim of the present retrospective study was to evaluate complications following carbon-fiber-reinforced polyetheretherketon (CFR-PEEK) volar plating in distal radius fracture in a large cohort of patients. As an alternative to conventional metallic devices, CFR-PEEK plates have been introduced over the last few years. METHODS: We performed a retrospective study including all patients who were treated for distal radius fracture using a volar fixed-angle plate DiPHOS-RM (Lima Corporate, Udine, Italy). All CFR-PEEK plates implanted were reviewed between May 2012 and December 2017.

Role of a spanning plate as an internal fixator in complex distal radius fractures.

OBJECTIVE: Minimally invasive temporary internal wrist arthrodesis as an alternative treatment method in complex distal radius fractures. INDICATIONS: Complex distal radius fractures with dorsal and/or palmar comminution and little to no reconstruction possibilities, radiocarpal ligamentous injury, need for early weight bearing through the affected wrist (walking aids). CONTRAINDICATIONS: Complex hand injuries limiting the possibility to safely secure the plate at either the metacarpal or the radial shaft. SURGICAL TECHNIQUE: Percutaneous or open reduction and fixation of the distal radius …

Acute Carpal Tunnel Syndrome in Inpatients With Operative Distal Radius Fracture.

Acute carpal tunnel syndrome (CTS) may occur concomitantly with distal radius fracture (DRF) and is often managed with carpal tunnel release (CTR). Carpal tunnel syndrome may also develop postoperatively after DRF fixation. The authors sought to determine the rate of CTS with DRF, prophylactic CTR, and postoperative development of CTS. Furthermore, they also sought to identify predictors of these. The Nationwide Inpatient Sample database was queried (2002 to 2014) to identify adult inpatients undergoing surgical fixation of DRFs. They were segregated by the presence of CTS and further stratifi

Treatment of the distal radius fractures with percutaneous pinning: evolution to the HK2 system.

Displaced fractures of the distal radius often require reduction and fixation to improve outcome in active patients. The volar locking plate gained ground over pinning, which was the first surgical solution described. But pinning methods have since evolved with fewer complications. The authors present a clinical study of distal radius fracture fixation using the HK2 technique, where subchondral pins are connected to intra-focal pins. The results are satisfactory with a lower complication rate compared to other pinning methods. This technical improvement relaunches the pinning technique, with i

[The Malunited Distal Radius Fracture - Extraarticular Correction without Bone Graft].

BACKGROUND: In open wedge osteotomies of the distal radius, bone grafts increase initial stability and enhance bone formation but are frequently associated with complaints and complications at the donor site. Previous studies have suggested that bone grafting may probably be unnecessary. PATIENTS AND METHODS: Between 2013 and 2016, 19 patients with symptomatic extra-articular fractures of the distal radius, malunited in extension, underwent corrective osteotomy using a palmar locking implant without an additional bone graft. 3 patients dropped out for implant related problems. One patient ref

Which immobilization is better for distal radius fracture? A prospective randomized trial.

INTRODUCTION: The aim of this study was to determine whether an above-elbow cast (AEC) is better than a below-elbow cast (BEC) at maintaining the initial reduction in the orthopaedic management of a distal radius fracture (DRF). METHODS: It is a prospective randomized study carried out in a single emergency trauma department. There were 72 patients older than 55 years of age (55-96) with a distal radius fracture treated orthopaedically. They were randomized into two groups: group B (AEC 32 patients) and group A (BEC 40 patients). Randomization was done by a computer program. Four subgroups we

Predictors of Carpal Tunnel Release After Open Distal Radius Fracture.

The purpose of this investigation was to determine the incidence and identify the predictors of carpal tunnel release (CTR) after open fractures of the distal radius (DRF). Patients with clinical symptoms of persistent median nerve neuropathy that required CTR were analyzed for risk factors. One hundred thirty-nine open DRFs (107 grade I, 23 grade II, 9 grade III) met the inclusion criteria. The incidence of CTR was 13.7% in all open DRFs (19 out of 139). Multivariable logistic regression analysis identified four predictors: male sex [odds ratio (OR) = 8.8, p = .001], type III Gustilo and Ande

Developing an instrument for the measurement of grip ability after distal radius fracture.

BACKGROUND: Distal radius fracture (DRF) often causes reduced ability to perform activities of daily living (ADLs). There is a need for recurrent evaluation of the ability to perform ADL in the early stage of rehabilitation as a complement to traditional functional measures. AIM: Develop a short and simple performance test for use in clinical practice through modification of the Sollerman Grip Function Test (GFT). METHOD: DRF patients and controls performed the GFT. The tasks that discriminated patients from controls were calculated for sensitivity to change and correlated to the QuickDASH q

Radius and Ulna Stress Fractures in a Patient with Neuromyelitis Optica: A Case Report.

INTRODUCTION: Hemangiomas are benign tumors characterized by proliferation of blood vessels. A few hemangiomas are aggressive, characterized by bone expansion and extraosseous extension. These benign tumors may be mistaken for metatasis resulting in unnecessary biopsies, which have a high risk of hemorrhage. These hemangiomas can spread not just into the paraspinal soft tissues but also into the epidural region of the spinal canal causing cord compression and paraparesis. These clinical symptoms can be relieved by surgical decompression of the posterior elements, embolization or radiotherapy.

Fat drops in wrist tendon sheaths on MRI in conjunction with a radius fracture.

A case is presented in which fat drops are seen on MRI inside extensor compartment 2 and 3, in conjunction with a radius fracture. The occurrence of traumatic tendon sheath fat-fluid levels has been sparsely reported on CT and MR imaging. This case is the first report of post-traumatic tendon sheath fat drops. Although the clinical relevance of tendon sheath fat drops seems to be limited, it is important to detect and correctly diagnose these fat drops in order to provide an accurate and complete radiologic report.

Common treatments and procedures used for fractures of the distal radius and scaphoid: A review.

The distal radius and the scaphoid are the most commonly injured carpal bones among both active adults and the osteoporotic elderly. The purpose of surgical treatment is to restore form and function to the wrist. Depending on the nature of the fracture, either topical procedures or invasive surgery can be applied. This article critiques the treatments currently used for fixation of wrist fractures in order to drive the development of new materials to improve patient outcomes.

Defining the role of intramedullary nailing for fractures of the distal radius: a systematic review.

This article is a systematic review of the published literature about the biomechanics, functional outcome and complications of intramedullary nailing of fractures of the distal radius. We searched the Medline and EMBASE databases and included all studies which reported the outcome of intramedullary (IM) nailing of fractures of the distal radius. Data about functional outcome, range of movement (ROM), strength and complications, were extracted. The studies included were appraised independently by both authors using a validated quality assessment scale for non-controlled studies and the CONSORT

Primary Wrist Hemiarthroplasty for Irreparable Distal Radius Fracture in the Independent Elderly.

Background Volar plating for acute distal radius fractures (DRF) in the elderly has been recommended. Some studies have suggested that open reduction with internal fixation (ORIF) in this situation results in frequent complications. Our purposes were to provide a definition of irreparable DRF in independent elderly patients and to review the results of a preliminary retrospective series of wrist hemiarthroplasty (WHA) in this patient population. Materials Between 2011 and 2014, 11 consecutive independent elderly patients (12 wrists) with irreparable intra-articular DRF were treated with primar…

[Growth behaviour after fractures of the proximal radius: differences to the rest of the skeleton].

BACKGROUND: Fractures of the proximal end of the radius in the growth phase have three characteristics: the head of the radius articulates with two joint partners and is therefore indispensable for an undisturbed function of the elbow. The blood supply of the proximal end of the radius is via periosteal vessels in the sense of a terminal circulation which makes it extremely vulnerable. Severe trauma caused either by accidents or treatment, can result in partial or complete necrosis with deformity of the head and neck region of the radius. INJURY PATTERNS: Radioulnar synostosis and chronic epi

Multipurpose external fixation for unstable comminuted intraarticular fracture of distal radius.

BACKGROUND: Most of the Intraarticular distal radius fractures are complex and unstable. They are caused by high-energy injuries. The optimal and appropriate treatment remains a topic of controversy. Many different methods of treatment have been advocated such as closed reduction maintained by cast, K-wire fixation with cast, and opened reduction with internal fixation with plate and screws or external fixation with or without K-wire augmentation. OBJECTIVE: To study the efficacy of the TU Model External Fixator for the treatment of unstable intraarticular fracture of distal radius. This was …
