Treatment of metachronous colorectal cancer metastases in the Netherlands: A population-based study.

BACKGROUND: This study aimed to describe the treatment of metachronous colorectal cancer metastases in a recent population-based cohort. METHOD: Patients with stage I-III colorectal cancer (CRC), diagnosed between January 1st and June 30th, 2015 who were surgically treated with curative intent were selected from the Netherlands Cancer Registry. Follow-up was at least 3 years after diagnosis of the primary tumour. Treatment of metachronous metastases was categorized into local treatment, systemic treatment, and best supportive care. Overall survival was estimated using Kaplan-Meier method. RE

Adenocarcinoma of the rectosigmoid colon seeding into pre-existing anal fistula.

This paper reports a rare case of rectosigmoid colon carcinoma metastasizing to anal fistula. The patient is a 57-year old man with a 7-year history of a fistula in ano. Colonoscopy revealed a rectosigmoid carcinoma. He underwent high-anterior resection. Two years and 3 months later, a subcutaneous tumor was found at the external opening of fistula. Trans perineal tumor excision plus fistulectomy was performed. Pathology revealed that the colon cancer and the perianal tumor were both moderately differentiated adenocarcinoma. Many reports support the concept of tumor cell implantation in mucosa

Prolapsed pedunculated lipoma: a cause of rectosigmoid intussusception.

We report the case of a male in his early 30s who presented with a large prolapsed anal mass, a single episode of rectal bleeding and a 1 day history of lower abdominal pain. Contrast-enhanced CT imaging of the abdomen and pelvis revealed a 4.4 cm lipomatous mass protruding through the anal canal, originating from the sigmoid colon, which had intussuscepted into the rectum and anus. The patient was successfully managed with flexible sigmoidoscopy and excision of the mass. Histopathological examination confirmed the diagnosis of a colonic lipoma.

Combined Robotic Transanal Transection Single-Stapled Technique in Ultralow Rectal Endometriosis Involvement Associated With Parametrial and Vaginal Infiltration.

OBJECTIVE: To describe a combined robotic and transanal technique used to treat ultralow rectal endometriosis in a 36-year-old patient with multiple pelvic compartments, which was responsible for infertility, dyspareunia, left sciatic pain, and severe dyschezia. DESIGN: Surgical video article. SETTING: The achievement of a perfect bowel anastomosis in patients with low rectal endometriosis could be challenging owing to technical and anatomic limitations [1]. By allowing a right angle rectotomy with a single-stapled anastomosis, the transanal transection single-stapled technique overcomes the

Natural Orifice Transanal Endoscopic Rectopexy for Complete Rectal Prolapse: Prospective Evolution of a New Technique and Short-term Outcomes.

BACKGROUND: Endoluminal surgery for the management of rectal prolapse remains largely experimental. OBJECTIVE: To evaluate the evolution and short-term outcomes of a new endoluminal technique for the management of complete rectal prolapse. DESIGN: This was a prospective study. SETTINGS: This study was conducted at a single tertiary care teaching center. PATIENTS: A total of 29 patients were included. The first 12 patients underwent the procedure with our initial technique, and the last 17 patients were subjected to the new modified procedure. The follow-up duration was 3 years for the olde

Using transanal irrigation in the management of low anterior resection syndrome: a service audit.

Low anterior resection syndrome (LARS) is a collection of symptoms that can occur as a result of a low anterior resection for bowel cancer. Transanal irrigation (TAI) can be used to manage these symptoms. This article describes a retrospective audit of 15 patients who were using TAI to manage symptoms of LARS. The aim of the audit was to ascertain whether the use of TAI improved outcomes for these patients. The data suggest that TAI has reduced both the frequency of bowel movements and episodes of faecal incontinence. Those patients using higher volumes of water seem to have experienced more b

Late gastrointestinal toxicity after radiotherapy for rectal cancer: a systematic review.

PURPOSE: Although the multimodal cancer treatment techniques have greatly improved over the years, irradiation-induced late gastrointestinal toxicity remains a great concern as it may highly affect the quality of life of a patient. The aim of this study was to define the prevalence of late gastrointestinal toxicities. METHODS: Electronic databases of Cochrane Library, Embase, Web of Science, CENTRAL and PubMed were searched until September 2019. We used the following keywords: radiotherapy, radiation therapy, irradiation, rectal cancer, gastrointestinal toxicity, adverse effects, late effects

Adenocarcinoma within anorectal fistulae: different clinicopathological characteristics between Crohn's disease-associated type and the usual type.

Adenocarcinoma within anorectal fistulae is rare and is sometimes associated with Crohn's disease. Crohn's disease-associated adenocarcinoma within anorectal fistulae has a poor prognosis; however, little is known about the clinicopathological differences between Crohn's disease-associated adenocarcinoma within anorectal fistulae and usual adenocarcinoma within anorectal fistulae. We retrospectively searched patients' charts and pathology archives at Tokyo Yamate Medical Center and Tokyo Medical and Dental University Hospital for adenocarcinoma within anorectal fistulae. Clinical and pathologi

Primary and secondary adenocarcinomas associated with anal fistulae.

PURPOSE: The purpose of this study was to evaluate the clinical features, pathology, and etiology of adenocarcinoma in patients with anal fistulae. METHODS: We identified seven patients diagnosed with adenocarcinoma associated with anal fistulae from a retrospective chart review. RESULTS: Five patients were diagnosed with primary adenocarcinoma associated with anal fistulae. Two patients were diagnosed with secondary adenocarcinoma associated with anal fistulae originating from rectal cancer on the proximal side. The primary adenocarcinomas included cancers arising from long-standing anal fi

Occlusion of a postoperative rectoabdominocutaneous fistula with fibrin clot: a case report.

A 56 year-old rectal cancer patient who developed a chronic rectoabdominocutaneous fistula postoperatively was treated with fibrin clot, and the fistula healed completely. Occlusion of chronic postoperative fistulas with fibrin clot appears to be a useful technique.

[Squamous epithelial carcinoma of the anorectal junction in Crohn disease with an anal fistula of many years' duration].

A squamous-cell carcinoma of the anorectal junction had developed from a chronic anal fistula in a 27-year-old woman with Crohn's disease who had been suffering from anal and rectovaginal fistulas for many years. In view of their cancerogenic potential, chronic fistulas associated with Crohn's disease must be followed up closely by clinical examination, endoscopy and biopsy. If surgery is indicated, resection of such fistulas should be considered at an early stage.

[The EEA stapler in low rectal anastomoses: preliminary results (author's transl)].

In the past two years, we have been using the EEA stapler for anastomoses of the extra-peritoneal rectum. With this technique, we have been able to preserve the anal sphincter in all cases of tumor at or above 6 cm from the anal verge. The results of 24 consecutive cases demonstrate the safety of the mechanical anastomoses of the mid-rectum with no death, nor anastomotic dehiscence in 15 cases. Complications occurred in the lowest locations which would have been otherwise treated by an abdomino-perineal excision or pull through procedure: 1 death from peritonitis and stress ulcer 3 temporary l
