Understanding low sensitivity of community-based HIV rapid testing: experiences from the HPTN 071 (PopART) trial in Zambia and South Africa.

INTRODUCTION: Population-wide HIV testing services (HTS) must be delivered in order to achieve universal antiretroviral treatment (ART) coverage. To accurately deliver HTS at such scale, non-facility-based HIV point-of-care testing (HIV-POCT) is necessary but requires rigorous quality assurance (QA). This study assessed the performance of community-wide HTS in Zambia and South Africa (SA) as part of the HPTN 071 (PopART) study and explores the impact of quality improvement interventions on HTS performance. METHODS: Between 2014 and 2016, HIV-POCT was undertaken within households both as part

Child Maltreatment and Migration: A Population-Based Study Among Immigrant and Native Adolescents in Switzerland.

BACKGROUND: Prevalence rates of child maltreatment (CM) can differ substantially between countries and ethnicities. Reasons, however, are complex and not sufficiently understood. METHOD: This epidemiological study examined prevalence and risk factors of various types of CM in a population-based representative sample of native and immigrant adolescents in Switzerland (N = 6,787). RESULTS: The prevalence of CM in general was lowest in the native group, higher in the Western immigrant group, and highest in the non-Western immigrant groups. An immigrant background was related to an overrepresent

[Risk factors for acute respiratory disease hospitalization in children under one year of age].

OBJECTIVE: To evaluate risk factors for acute respiratory disease hospitalizations in children under one year of age. METHODS: A case-control study was conducted in the city of Pelotas, Southern Brazil. Cases were children under one year of age who were hospitalized due to acute respiratory diseases from August 1997 to July 1998. Controls were same-age community children randomly selected without previous respiratory disease hospitalization. A questionnaire about risk factors exposure was applied to the mothers of cases and controls. Univariate, bivariate and multivariate analyses through log

Logistics of community smallpox control through contact tracing and ring vaccination: a stochastic network model.

BACKGROUND: Previous smallpox ring vaccination models based on contact tracing over a network suggest that ring vaccination would be effective, but have not explicitly included response logistics and limited numbers of vaccinators. METHODS: We developed a continuous-time stochastic simulation of smallpox transmission, including network structure, post-exposure vaccination, vaccination of contacts of contacts, limited response capacity, heterogeneity in symptoms and infectiousness, vaccination prior to the discontinuation of routine vaccination, more rapid diagnosis due to public awareness, su

The Association of Preterm Birth With Maternal Nativity and Length of Residence Among Non-Hispanic Black Women.

BACKGROUND: Preterm birth (PTB) is associated with future cardiovascular disease (CVD) risk and disproportionally affects non-Hispanic Black (NHB) women. Limited data exist on the influence of length of US residence on nativity-related disparities in PTB. We examined PTB by maternal nativity (US born vs foreign born) and length of US residence among NHB women. METHODS: We analyzed data from 2699 NHB women (1607 US born; 1092 foreign born) in the Boston Birth Cohort, originally designed as a case-control study. Using multivariable logistic regression, we investigated the association of PTB wit

Characterizing U.S. community well-being indices: a methodological synthesis and measurement systems perspective.

BACKGROUND: The proliferation of community well-being indices (CWBI) reflects growing recognition that both individual experiences and place-based conditions shape population health and quality of life. These indices are increasingly used to inform public health policy, community planning, and population health monitoring. However, there is substantial variation across indices in how community well-being is conceptualized, measured, and reported. Despite their expanding use, few studies have systematically compared these indices as integrated measurement systems. This study aimed to characteri

Poverty in the residential and surrounding counties and its impact on mortality in women with breast cancer.

BACKGROUND: The influence of poverty on cancer outcomes beyond a woman's county of residence remains understudied, despite individuals frequently interacting within a larger ecosocial system. METHODS: A retrospective cohort study was conducted to investigate whether high poverty in residential and surrounding counties is associated with increased mortality. Women from the National Cancer Institute Surveillance, Epidemiology, and End Results database ≥20 years of age, diagnosed with primary breast cancer between 2005 and 2014, and who survived at least 1 year (N = 36,711) were included. Association of neighborhood deprivation and live birth after 8155 first, single euploid frozen embryo transfers: the lived patient experience matters.

STUDY QUESTION: Is there an association between neighborhood deprivation and frozen embryo transfer outcomes? SUMMARY ANSWER: After adjusting for important reproductive characteristics most closely associated with embryo transfer outcomes, residual significant differences in live birth were present with a progressive decline in the probability of live birth as measures of socioeconomic disadvantage increased. WHAT IS KNOWN ALREADY: Residence in a socioeconomically disadvantaged neighborhood is associated with adverse reproductive health outcomes, including decreased fecundability, preeclamps

Residential greenness and adolescent mental health trajectories: A longitudinal pre-registered study.

Exposure to natural environments, such as residential green spaces, has been proposed as a promising approach to health promotion and prevention of mental health issues among adolescents. This study investigated the association between residential greenness and trajectories of adolescent mental health. Participants (n = 245) were part of the Orygen Adolescent Development Study and reported on anxiety and depression symptoms, using the Beck Anxiety Inventory and Center for Epidemiologic Studies-Depression scale, at ages 12, 15, 17, and 19 years. Parent-reports of externalizing behavior was asse

Environmental Stressors, Anemia, and Depressive Symptoms in Pregnancy: Unpacking the Combined Risks.

Chronic exposure to structural violence and environmental hazards may disrupt stress regulation, trigger inflammation, and impair iron metabolism in women. Iron deficiency has been associated with depression, but the combined impact of environmental stressors and anemia on maternal mental health remains understudied. We analyzed associations between 28 neighborhood-level environmental stressors, hemoglobin levels, and depressive symptoms (measured by the Patient Health Questionnaire-9) during early pregnancy, using retrospective data from 1964 pregnant patients (2015-2019) at an urban health c

Living alone, loneliness and mortality in hospitalized older adults: findings from the SAFES cohort.

Population aging is a worldwide phenomenon. Social isolation and loneliness are common factors of social vulnerability among older people and are associated with morbidity and mortality. Loneliness is a subjective state and cannot be directly measured by objective criteria such as marital status or living alone, although the latter may have an indirect link to feeling lonely. The main aim of this study was therefore to investigate the association between loneliness, living alone and survival in a nationwide French cohort including adults aged 75 years or older who are admitted to hospital via

Community-level social capital and postoperative outcomes following intracranial tumor surgery: an exploration of the Social Capital Atlas in neurosurgical oncology.

BACKGROUND AND OBJECTIVES: Explore whether community social capital measures (system of resources available to individuals through community engagement) are related to surgical outcomes among intracranial tumor patients. METHODS: Adults who underwent resection at a single medical center for intracranial tumor was identified and their zip codes were matched to three variables derived from the Social Capital Atlas: economic connectedness, volunteering rate, and civic organizations. The economic connectedness score quantifies the degree to which low-income and high-income community members are f

Exposome-Wide Association Study of Body Mass Index Using a Novel Meta-Analytical Approach for Random Forest Models.

BACKGROUND: Overweight and obesity impose a considerable individual and social burden, and the urban environments might encompass factors that contribute to obesity. Nevertheless, there is a scarcity of research that takes into account the simultaneous interaction of multiple environmental factors. OBJECTIVES: Our objective was to perform an exposome-wide association study of body mass index (BMI) in a multicohort setting of 15 studies. METHODS: Studies were affiliated with the Dutch Geoscience and Health Cohort Consortium (GECCO), had different population sizes (688-141,825), and covered th

Exploring the Determinants of Age-Friendly Neighbourhoods Among Seniors Residing in a Low-Income Urban Community in Singapore: A Participatory Study.

The growing interest regarding the role of the living environment in healthy ageing highlights the need to investigate place-related urban features contributing to health and socio-emotional wellbeing. This study aimed to use a participatory methodology to explore the determinants of an age-friendly neighbourhood built environment that promotes or limits healthy ageing in place among seniors residing in a low-income urban community in Singapore. The study utilised photovoice and semi-structured interviews with 25 community-dwelling seniors aged 65 and above, residing in government-built public

Neighborhood disorder and social cohesion: A longitudinal investigation of links with maternal cardiometabolic risk one year postpartum.

OBJECTIVE: Neighborhood characteristics can influence cardiometabolic health, including during the perinatal period. However, maternal health research has largely examined the influence of objective neighborhood measures, limiting insights into psychological and social processes. We examined associations of perceived neighborhood disorder and social cohesion with maternal cardiometabolic risk 1 year postpartum and explored potential pathways of psychological distress and physical activity. METHODS: A predominantly low-income sample of Black, Latina, and White postpartum women (n = 987) were p

Socioeconomic inequalities in the uptake of postpartum care at home across Dutch neighbourhoods.

BACKGROUND: Postpartum care focuses on prevention of health problems by performing medical check-ups and through enhancing maternal empowerment, the parent-infant interaction and knowledge about mother's own health and that of her newborn. We aimed to investigate whether there was significant clustering within neighbourhoods regarding the uptake of postpartum care and to what extent neighbourhood-level differences are explained by individual socio-demographic factors, pregnancy-related factors and neighbourhood-level determinants (i.e. deprivation and urbanization). METHODS: A nationwide popu

Redlining and Time to Viral Suppression Among Persons With HIV.

IMPORTANCE: Structural racism in the US is evidenced in the discriminatory practice of historical racial redlining when neighborhoods were valued, in part, based on the community's racial and ethnic compositions. However, the influence of these systemic practices in the context of the HIV epidemic is not well understood. OBJECTIVE: To assess the effect of redlining on time to viral suppression among people newly diagnosed with HIV. DESIGN, SETTING, AND PARTICIPANTS: Observational study that included individuals diagnosed with HIV from January 1, 2011, to December 31, 2019, in New Orleans, Lo

Association of Perceived Neighbourhood Walkability with Self-Reported Physical Activity and Body Mass Index in South African Adolescents.

Adolescence is a life stage critical to the establishment of healthy behaviours, including physical activity (PA). Factors associated with the built environment have been shown to impact PA across the life course. We examined the sociodemographic differences in, and associations between, perceived neighbourhood walkability, PA, and body mass index (BMI) in South African adolescents. We recruited a convenience sample (n = 143; 13-18 years; 65% female) of students from three high schools (middle/high and low-income areas). Participants completed a PA questionnaire and the Neighbourhood Environme

Cross-Sectional Study of Location-Based Built Environments, Physical Activity, Dietary Intake, and Body Mass Index in Adult Twins.

We examined relationships between walkability and health behaviors between and within identical twin pairs, considering both home (neighborhood) walkability and each twin's measured activity space. Continuous activity and location data (via accelerometry and GPS) were obtained in 79 pairs over 2 weeks. Walkability was estimated using Walk Score® (WS); home WS refers to neighborhood walkability, and GPS WS refers to the mean of individual WSs matched to every GPS point collected by each participant. GPS WS was assessed within (WHN) and out of the neighborhood (OHN), using 1-mile Euclidean (air1
