Efferent activity was recorded from cranial nerves in the decerebrate dogfish (Scyliorhinus canicula) before and after injection of paralysing drugs. The recordings were made from the mandibular (Vth) and glossopharyngeal (IXth) nerves and the branchial (respiratory) and cardiac branches of the vagus (Xth) nerve. All the respiratory branches (Vth, IXth and Xth) and both cardiac branches fired rhythmic bursts of activity, synchronous with ventilation, which continued (at a higher rate) following paralysis, indicating that they originated in the CNS rather than arising reflexly from stimulation

Listeria monocytogenes (LM) rhombencephalitis is a rare but severe manifestation of neurolisteriosis, typically reported in immunocompromised or elderly patients and often associated with high morbidity and mortality. We describe a 66-year-old immunocompetent man who presented initially with peripheral facial paralysis, an uncommon early manifestation reflecting cranial neuritis, before rapidly developing brainstem dysfunction, respiratory failure, and multiple abscesses. Diagnosis was confirmed by cerebrospinal fluid (CSF) multiplex polymerase chain reaction (PCR) and blood cultures, and desp

Enterovirus D68 (EV-D68) is a cause of severe acute respiratory infections (SARI) and is associated with acute flaccid paralysis. However, data on EV-D68 in Africa, where SARI remains a leading cause of morbidity and mortality among children, remain limited. We conducted a retrospective analysis to detect EV-D68 in archived samples collected from hospitalized children with SARI in Zambia from 2018 to 2020. We identified eight positive samples in 2020, and genetic sequence analysis showed that all sequences belonged to subclade A2. Continuous monitoring is crucial to enhance understanding of th

Guillain-Barre syndrome (GBS) is an acute polyradiculoneuropathy, caused by dysregulated immune response following an infectious or noninfectious event. Although cardiovascular, respiratory, and gastrointestinal systems are commonly involved secondary to neuromuscular paralysis, renal manifestations are rare. Acute kidney injury (AKI) can develop in GBS due to acute tubular necrosis secondary to dysautonomia. Minimal change nephrotic syndrome in GBS may be due to T-cell dysregulation and cytokine release attributed to molecular mimicry. Here, we report the case of GBS with simultaneously devel

A 27-year-old woman presented at 23 weeks' 6 days' gestation who tested positive for the coronavirus disease 2019 (COVID-19). Despite mechanical ventilation and paralysis, she remained hypoxic and was emergently cannulated for veno-venous extracorporeal membrane oxygenation (VV-ECMO). The patient ambulated while intubated and on VV-ECMO. She was decannulated and extubated. An ultrasound demonstrated an appropriately grown fetus without abnormalities. She was discharged to home and gave birth to a healthy baby girl at 39 weeks' gestation. Using VV-ECMO, this patient and her fetus survived acute

A 52-year-old woman with Charcot-Marie-Tooth disease presented with severe dyspnea due to bilateral diaphragmatic paralysis severely compromising respiratory function. There was little in the available literature to guide us regarding management of this unusual condition, and after deliberation, we decided to treat her with a staged plication of bilateral hemidiaphragms. Postoperatively, she demonstrated very good symptomatic relief supported by objective evidence, including improvement in lung function tests. We describe our management of this difficult condition, including the surgical and a

This report describes the case of a 39-year-old male patient who presented to the emergency room with complaints of impaired balance, diplopia, and nasal voice. The patient had a history of upper respiratory tract infection. The initial physical examination revealed ataxia, ophthalmoplegia, and areflexia, which are consistent with the classic triad of Miller Fisher syndrome, considered a benign variant of Guillain-Barré syndrome. The patient developed peripheral facial paralysis during hospitalization. He underwent a treatment with immunoglobulin for five days, resulting in near complete resol

A 49-year-old woman with amyotrophic sequelae of poliomyelitis experienced progressive left upper limb weakness and breathing discomfort while walking that had developed over one year prior to presentation. She had flaccid quadriplegia which was more marked in the left upper limb, with the C4- and C5-innervated muscles being most severely affected. Chest radiographs double exposed at maximal inspiration and expiration revealed poor respiratory movements in the left hemidiaphragm. Phrenic nerve conduction study demonstrated conspicuous diaphragmatic paralysis on the left side. The electrophysio

Dress code: Emergency nurses at University College London Hospitals NHS Trust donned protective clothing last month as part of a simulated terrorist attack on the London underground. Forty 'casualties' were treated for poisoning with Sarin, the agent used on the Tokyo underground in 1995. They 'presented' with symptoms that included muscle spasm, eye irritation, a runny nose, faecal incontinence, vomiting and respiratory paralysis. A&E modern matron Debra Glastonbury said: 'We were able to get our first decontamination tent up in four minutes. And by the time the second one was up, we were in …

A 67-year-old woman had fever, myalgias, progressive weakness, and respiratory insufficiency. In 9 days, flaccid areflexic quadriparesis and bulbar palsy developed. She died 26 days after the onset of her illness. Serum and cerebrospinal fluid serology were positive for West Nile virus. Neuropathological study showed changes consistent with a viral encephalomyelitis, similar to poliomyelitis. The brainstem showed neuronal loss and multiple foci of necrosis. The spinal cord showed severe loss of anterior and posterior horn neurons. Immunohistochemistry identified West Nile virus antigens in the

BACKGROUND: Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a variant form of stiff person spectrum disorders. Paroxysmal respiratory failure and paroxysmal sympathetic hyperactivity syndrome (PSH) had rarely been reported in PERM cases.
CASE PRESENTATION: We report a PERM case with acute onset of limb stiffness followed by rapid progression to severe sound-induced myoclonus with respiratory failure (oxygen saturation decreased to 30% with generalized cyanosis). The patient also exhibited ptosis, restricted rightward and downward eye movement, peripheral facial paralysis,

STUDY DESIGN: Case report and narrative review.
OBJECTIVE: To explore the therapeutic role of surgical and nonsurgical treatment of diaphragmatic paralysis secondary to spinal cord and nerve root compression.
SUMMARY OF BACKGROUND DATA: Phrenic nerve dysfunction due to central or neuroforaminal stenosis is a rare yet unappreciated etiology of diaphragmatic paralysis and chronic dyspnea. Surgical spine decompression, diaphragmatic pacing, and intensive physiotherapy are potential treatment options with varying degrees of evidence.
METHODS: The case of a 70-year-old male with progressive dysp

A toddler presented to the emergency department with persistent stridor and barky cough for 4 weeks and progressive dysphagia for 1 week. During this time, he had sought medical attention 6 additional times and had been treated for pneumonia, wheezing and croup, receiving antibiotics and several courses of steroids without improvement. On the final presentation, airway imaging did not reveal a foreign body. However, bedside laryngoscopy demonstrated bilateral vocal cord paralysis. Further imaging revealed an intracranial posterior fossa mass which provided the unifying diagnosis for his persis

BACKGROUND: Unilateral diaphragmatic paralysis or paresis (UDP) in adults is an often overlooked disease which relevantly impairs the patient's lung function and quality of life. Particularly in idiopathic UDP, there is no evidence for conservative therapy and only little evidence for surgical therapy.
METHODS: The method involves retrospective single-center analysis of patients with UDP persistent for at least 1 year who were operated by diaphragmatic resection, plication, and augmentation with a polypropylene mesh. The patients were tested for lung and diaphragmatic function, six-minute wal

BACKGROUND: Neuralgic amyotrophy is characterised by pain in the neck or shoulder region, followed by neuropathy of both motor and sensory nerves of the brachial plexus. The incidence of this condition is estimated at 1/1000 per year. In a rare variant of the syndrome, involvement of both phrenic nerves can occur, leading to diaphragmatic paralysis and severe orthopnoea.
CASE DESCRIPTION: A 67-year-old woman was referred to us with acute orthopnoea. Imaging studies showed bilateral diaphragmatic paralysis, and electromyography (EMG) confirmed neuropathy of both phrenic nerves. The diagnosis w

We report the case of a 49-years-old patient who presented to the accident and emergency department with sudden onset dyspnea associated with acute shoulder pain. He was breathless at rest with supine hypoxemia. He had an amyotrophic left shoulder with localized paresis of the shoulder. Both hemi-diaphragms were elevated on chest X-rays. Pulmonary function tests showed a restrictive pattern and both phrenic nerve conduction velocities were decreased. At night, alveolar hypoventilation was evidenced by elevated mean capnography (PtcCO2: 57mmHg). Neuralgic amyotrophy, Parsonage-Turner syndrome w

A method is described for performing needle electromyography of the diaphragm which is safe and causes little discomfort. It provides valuable information concerning neuropathies and myopathies which may affect the diaphragm, and complements information derived from phrenic nerve conduction studies. Firing patterns of motor unit potentials during spontaneous respiration identify upper motor neurone disorders causing respiratory insufficiency.
