Inherited retinal disease pathway in the UK: a patient perspective and the potential of AI.

BACKGROUND: Inherited retinal diseases (IRDs) are the leading cause of blindness in young people in the UK. Despite significant improvements in genomics medicine, the diagnosis of these conditions remains challenging, and around 40% do not receive a definite genetic diagnosis after extensive genetic testing. This survey aims to investigate the experience of individuals affected by IRDs, their relatives, friends and caregivers, focusing on their care and diagnostic journey. Additionally, it explores the potential acceptability of artificial intelligence (AI) technologies, such as Eye2Gene, that…

Uncharted territory: retinal vasculitis and cryoglobulinemia in Behçet's disease - a case report.

Behçet's disease (BD) is a systemic inflammatory disorder characterized by recurrent oral and genital ulcers, skin lesions, and ocular involvement, often presenting with retinal vasculitis as a severe complication. Although mixed cryoglobulinemia, typically associated with hepatitis C virus (HCV) infection, is well-documented in other autoimmune diseases, its coexistence with BD is exceedingly rare. This report details the case of a 56-year-old male presenting with BD complicated by HCV-related mixed cryoglobulinemia, manifesting as retinal vasculitis, purpuric skin lesions, and systemic vascu

Interpericyte tunnelling nanotube loss in very early diabetic retinal disease.

AIMS/HYPOTHESIS: Interpericyte tunnelling nanotubes (IP-TNTs) regulate microvascular blood flow by coordinating pericyte-pericyte communication across distant capillaries in the retina. Perfusion abnormalities are observed in preclinical diabetic retinopathy, and the state of IP-TNTs in this condition is unknown. METHODS: Using high-resolution confocal microscopy with isolated perfusion labelling of human donor retina, we investigated changes to IP-TNTs, pericytes and capillaries in the macular vasculature of individuals with preclinical diabetic retinopathy (n=7) and control individuals (n=1

Retinal Microvascular Correlates of Cerebral Small Vessel Disease in Older Age.

BACKGROUND AND OBJECTIVES: Cerebral small vessel disease (cSVD), characterized by pathologic changes in the structure and function of small brain vessels, is detectable on brain MRI in the absence of clinical symptoms. However, imaging cerebral small vessels themselves in vivo remains costly and challenging. There is growing interest in investigating whether retinal microvascular imaging features could be proxies for changes in the brain microvasculature. Using a multipronged approach, we explored the relation of retinal microvascular characteristics with MRI markers of cSVD (MRI-cSVD). METHO

Branch Retinal Vein Occlusion Revealing Previously Undiagnosed Systemic Disease.

Branch retinal vein occlusion (BRVO) is a common cause of vision loss and frequently reflects an underlying systemic vascular disorder. Early systemic assessment is fundamental for preventing ocular and cardiovascular complications. This report describes the clinical case of a 61-year-old woman who presented with sudden, painless visual field loss in the left eye. Fundoscopic and angiographic findings confirmed superotemporal BRVO with macular edema. Intravitreal anti-vascular endothelial growth factor (VEGF) therapy was initiated, and referral to a primary care physician for systemic evaluati

Altered Outer Retinal Structure, Electrophysiology and Visual Perception in Parkinson's Disease.

BACKGROUND: Visual biomarkers of Parkinson's disease (PD) are attractive as the retina is an outpouching of the brain. Although inner retinal neurodegeneration in PD is well-established this has overlap with other neurodegenerative diseases and thus outer retinal (photoreceptor) measures warrant further investigation. OBJECTIVE: To examine in a cross-sectional study whether clinically implementable measures targeting outer retinal function and structure can differentiate PD from healthy ageing and whether these are sensitive to intraday levodopa (L-DOPA) dosing. METHODS: Centre-surround perc

Deep Learning Analysis of Retinal Structures and Risk Factors of Alzheimer's Disease.

The importance of early Alzheimer's Disease screening is becoming more apparent, given the fact that there is no way to revert the patient's status after the onset. However, the diagnostic procedure of Alzheimer's Disease involves a comprehensive analysis of cognitive tests, blood sampling, and imaging, which limits the screening of a large population in a short period. Preliminary works show that rich neurological and cardiovascular information is encoded in the patient's eye. Due to the relatively fast and easy procedure acquisition, early-stage screening of Alzheimer's Disease patients with

Retinal small vessel pathology is associated with disease burden in multiple sclerosis.

BACKGROUND: Alterations of the superficial retinal vasculature are commonly observed in multiple sclerosis (MS) and can be visualized through optical coherence tomography angiography (OCTA). OBJECTIVES: This study aimed to examine changes in the retinal vasculature during MS and to integrate findings into current concepts of the underlying pathology. METHODS: In this cross-sectional study, including 259 relapsing-remitting MS patients and 78 healthy controls, we analyzed OCTAs using deep-learning-based segmentation algorithm tools. RESULTS: We identified a loss of small-sized vessels (diame

Network Biology and Medicine to Rescue: Applications for Retinal Disease Mechanisms and Therapy.

Inherited retinal degenerations (IRDs) are clinically and genetically heterogenous blinding diseases that manifest through dysfunction of target cells, photoreceptors, and retinal pigment epithelium (RPE) in the retina. Despite knowledge of numerous underlying genetic defects, current therapeutic approaches, including gene centric applications, have had limited success, thereby asserting the need of new directions for basic and translational research. Human diseases have commonalities that can be represented in a network form, called diseasome, which captures relationships among disease genes,

Beyond VEGF: Targeting Inflammation and Other Pathways for Treatment of Retinal Disease.

Neovascular eye diseases include conditions such as retinopathy of prematurity, proliferative diabetic retinopathy, and neovascular age-related macular degeneration. Together, they are a major cause of vision loss and blindness worldwide. The current therapeutic mainstay for these diseases is intravitreal injections of biologics targeting vascular endothelial growth factor (VEGF) signaling. Lack of universal response to these anti-VEGF agents coupled with the challenging delivery method underscore a need for new therapeutic targets and agents. In particular, proteins that mediate both inflamma

Artificial intelligence in retinal disease: clinical application, challenges, and future directions.

Retinal diseases are a leading cause of blindness in developed countries, accounting for the largest share of visually impaired children, working-age adults (inherited retinal disease), and elderly individuals (age-related macular degeneration). These conditions need specialised clinicians to interpret multimodal retinal imaging, with diagnosis and intervention potentially delayed. With an increasing and ageing population, this is becoming a global health priority. One solution is the development of artificial intelligence (AI) software to facilitate rapid data processing. Herein, we review re…

Is RPGR-related retinal dystrophy associated with systemic disease? A case series.

BACKGROUND: Ciliopathies responsible for retinitis pigmentosa can also cause systemic manifestations. RPGR is a ciliary gene and pathogenic variants in RPGR cause a retinal ciliopathy, the commonest cause of X-linked recessive retinitis pigmentosa. The RPGR protein interacts with numerous other ciliary proteins present in the transition zone of both motile and sensory cilia, and may play an important role in regulating ciliary protein transport. There has been a growing, putative association of RPGR variants with systemic ciliopathies: mainly sino-respiratory infections and primary ciliary dys

Characterization of retinal function and structure in the MPTP murine model of Parkinson's disease.

In addition to well characterized motor symptoms, visual disturbances are increasingly recognized as an early manifestation in Parkinson's disease (PD). A better understanding of the mechanisms underlying these changes would facilitate the development of vision tests which can be used as preclinical biomarkers to support the development of novel therapeutics for PD. This study aims to characterize the retinal phenotype of a mouse model of dopaminergic dysfunction and to examine whether these changes are reversible with levodopa treatment. We use a 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (

Inferred retinal sensitivity in recessive Stargardt disease using machine learning.

Spatially-resolved retinal function can be measured by psychophysical testing like fundus-controlled perimetry (FCP or 'microperimetry'). It may serve as a performance outcome measure in emerging interventional clinical trials for macular diseases as requested by regulatory agencies. As FCP constitute laborious examinations, we have evaluated a machine-learning-based approach to predict spatially-resolved retinal function ('inferred sensitivity') based on microstructural imaging (obtained by spectral domain optical coherence tomography) and patient data in recessive Stargardt disease.
