INTRODUCTION: Diagnosis of retroperitoneal fibrosis is generally delayed and revealed by various non-specific signs. We report the case of an isolated lymphedema of the lower limb revealing retroperitoneal fibrosis complicating a metastatic squamous cell carcinoma.

OBJECTIVE: To evaluate the efficacy of tamoxifen in the treatment of idiopathic retroperitoneal fibrosis in one patient and to review the results reported in the literature.
METHODS: A 68-year-old man with idiopathic retroperitoneal fibrosis and obstructive acute renal failure was admitted to our department. Bilateral ureteral stents were placed and tamoxifen 20 mg daily p.o. was started.

Retroperitoneal fibrosis has been observed in two patients with Parkinson's disease treated with bromocriptine. The patients complained of abdominal or lower back pain and presented with various degrees of renal insufficiency, with anuria in one. Laboratory evaluation furthermore showed an increased sedimentation rate and inflammatory anemia. Four cases of idiopathic retroperitoneal fibrosis are reported. Magnetic resonance and computerized tomographic images were obtained for diagnosis in each case, and followup scans were obtained at various intervals after therapy. In all 4 cases magnetic resonance imaging was equal or superior to computerized tomography in the diagnosis of retroperitoneal fibrosis, outlining the extent of involvement, preoperative planning and assessing response to medical or surgical therapy.

Retroperitoneal fibrosis is associated with Riedel's thyroiditis, in which an unexpectedly high proportion of the plasma cells have been reported to contain IgA and lambda light chains. Retroperitoneal fibrosis in a 12-year-old boy is reported. This was associated with a fibrotic mass in the neck which resolved spontaneously. Right-sided ureteric obstruction responded to treatment with steroids.

An 8-year-old boy with symptoms of relapsing polychondritis for five years developed retroperitoneal fibrosis and ureteral obstruction. In addition to involvement of the cartilage of his joints, ear, and nose, there was significant arterial involvement. The patient had a very good response to adrenal corticosteroid therapy. After one year of follow-up, he has had a definite increase in joint mobility and has normal renal function.
