Operative exposure and management of axillary vessel injuries.

INTRODUCTION: Axillary vessel injuries are uncommon and challenging injuries encountered by trauma surgeons. Proximity of this vessel to other adjacent veins including the axillary vein, brachial plexus and the osseous structures of the shoulder and upper arm account for a large number of associated injuries. MATERIALS AND METHODS: Systematic review of the literature, with emphasis on the diagnosis, treatment and outcomes of these injuries, incorporating the authors' experience. CONCLUSIONS: Although uncommon, axillary arterial injuries can result in significant morbidity, limb loss and mort

Injuries in Whitewater Paddling.

In brief Whitewater paddlers have very few injuries other than bruises and minor scrapes, and major injuries are the exception rather than the rule. Many of the injuries occur to capsized boaters, who are susceptible to hematomas, contusions, and abrasions from bumping into unseen rocks and obstructions as the river current carries them downstream. Two major chronic complaints are tendinitis of the wrist extensors and low back pain. Major injuries include head injuries, fractures, and shoulder dislocations and subluxations. As with other water sports, drowning is the major cause of fatalities.

[What information do we need to treat the injured shoulder of a hockey player?].

Successful patient care of the injured shoulder of a hockey player involves interdisciplinary cupertino. Images allow an interpretation of a static dimension but may not be sufficient enough to develop a surgical treatment strategy, since all injuries to the shoulder have its origin in a dynamic pathology with some kind of external impact. This article classifies various traumatic shoulder problems and the mechanism of injury, it outlines clinical tests and stresses key facts an imaging report should include in order to plan conservative or surgical treatment. Basic treatment options as perfor

Shoulder Osteoarthritis.

Glenohumeral osteoarthritis has proven to be a major contributor to shoulder joint pain and dysfunction in the elderly. There are several conditions about the shoulder that contribute to the development of glenohumeral osteoarthritis, which includes traumatic injuries, rotator cuff pathology, glenohumeral instability, glenoid dysplasia, and osteonecrosis. When glenohumeral osteoarthritis pain is refractory to conservative treatment, intra-articular injections and surgery can be performed. The radiologist should be aware of the varying types of shoulder arthroplasties, what preoperative finding

Shoulder arthroscopy in children and adolescents.

Arthroscopy is increasingly being used to manage a wide range of pathologies in the pediatric population. Knee arthroscopy is an efficacious treatment method for skeletally immature patients, and an increasing number of shoulder conditions can be managed with minimally invasive techniques. Special considerations are needed with regard to anatomy, anesthetic technique, equipment, and patient positioning when performing shoulder arthroscopy in a child or an adolescent. Several shoulder ailments can be managed arthroscopically in this patient population, including infection, contractures resultin

Traumatic surfing injuries in New Zealand: a descriptive epidemiology study.

BACKGROUND: New Zealand (NZ) has nearly 14,000 km of coastline and a surfing population of approximately 315,000 surfers. Given its popularity, surfing has a high frequency of injury claims, however, there remains a lack of data on traumatic surfing-related injuries from large population studies. The primary purpose of this study was to examine traumatic surfing injuries in NZ specific to injury incidence, duration, location, type, mechanism of injury and associated risk factors. METHODS: A sample of self-identified surfers currently living in NZ participated in an online retrospective cross-

In-Season Management of Acute and Subacute Sports Foot Injuries.

In athletes, foot injuries present with a variety of mechanisms, severity, and implications for return to play. Although potentially given less attention than knee and shoulder injuries by the team physician, foot injuries are common and thus require knowledgeable consideration. In this article, we review the anatomy, presentation, workup, and management of several of the most common athletic foot injuries, including turf toe, Lisfranc injuries, Jones fractures, and navicular stress fractures. The goal is to provide the team physician with the information necessary to evaluate and manage these…

Posterior sternoclavicular joint injuries in skeletally immature patients.

The management of sternoclavicular injuries in skeletally immature patients has not been well described. The purpose of this study was to describe the authors' experience treating this rare and potentially life-threatening injury. All skeletally immature patients who underwent treatment for a medial clavicular physeal fracture or sternoclavicular dislocation between 2003 and 2011 were identified using ICD-9 diagnostic codes. Sternoclavicular injuries with posterior displacement were isolated from this cohort for a thorough chart review. Patients were contacted to complete brief phone surveys a

Defining the terms acute and chronic in orthopaedic sports injuries: a systematic review.

BACKGROUND: Although many authors report on acute injuries and chronic injuries in the orthopaedic literature, the actual terms are seldom explicitly defined. HYPOTHESIS: Much of the literature pertaining to sports injuries that are acute or chronic does not define these terms. It is believed that definitions will provide clarity and specificity in future literature. STUDY DESIGN: Systematic review. METHODS: A systematic review of 116 articles was conducted to determine whether and how the terms acute and chronic were defined as they pertain to several commonly treated conditions: Achilles

Bilateral posterior shoulder dislocation after electrical shock: A case report.

INTRODUCTION: Posterior dislocation of the shoulder is a rare and commonly missed injury. Unilateral dislocations occur mostly due to trauma. Bilateral posterior shoulder dislocations are even more rare and result mainly from epileptic seizures. Electrical injury is a rare cause of posterior shoulder dislocation. Injury mechanism in electrical injury is similar to epileptic seizures, where the shoulder is forced to internal rotation, flexion and adduction. PRESENTATION OF CASE: This report presents a case of bilateral posterior shoulder dislocation after electrical shock. We were able to find

Soft-tissue coverage of the shoulder and brachium.

Injuries to the upper extremity often involve bone and soft-tissue elements, as do injuries to the lower extremity. Proper reconstruction of the soft-tissue deficits in concert with bone restoration is essential to final recovery. Better understanding of the blood supply and newer concepts of managing severely injured limbs has improved the ability to manage these challenging problems.

[Significance of a transthoracic imaging technic in shoulder arthrography].

Even in this era of sonography, arthrography of the shoulder is an important diagnostic investigation, making it possible to exclude unrecognized connective tissue lesions in patients with chronic shoulder pain resistant to therapy. Sometimes standard X-ray techniques do not yield all the information the surgeon needs about ventral or dorsal localization of a rotator-cuff rupture. An additional approach to the joint is needed: a transthoracic lateral-view X-ray provides the surgeon with adequate information to help in selection of the most appropriate operative approach to the shoulder.

Luxatio erecta: case series with review of diagnostic and management principles.

We reviewed 11 cases of luxatio erecta (inferior shoulder dislocation) managed acutely at our institutions to gain insight into the diagnostic and management principles of this condition. We then compared our findings with those in the current literature. Luxatio erecta requires careful clinical and radiographic evaluation and a high index of suspicion for associated injuries, as they occur frequently and can be significant given their tendency to be associated with higher energy trauma. Our results indicate that the majority of patients return to preinjury level of shoulder function, despite
